We present the case of a young patient, immunocompetent and without risk factors, with a condition compatible with mononucleosis syndrome (cervical lymphadenopathy, fever, malaise), which was characterized by persistent symptoms, despite multiple antibiotic treatments. During its approach infection was ruled out due to the most common causes of fever and Epstein-Barr infection, for which serological tests were performed where it was corroborated an etiology that is unusual that is presented as infectious mononucleosis. By identifying the infectious agent and because it was associated with haematological alterations, organomegalias and myositis during its evolution, the patient was a candidate for antibiotic treatment, with notable improvement when he started it, which led to his total recovery.
