Diffuse alveolar hemorrhage is a rare entity with high mortality. Clinically it is characterized by severe dyspnea that is accompanied by hypoxemia, decreased hemoglobin, and hemoptysis. The most frequent causes are vasculitis, rheumatological, pharmacological diseases, elevation of left ventricular pressure (heart failure), among others. The clinical case of a 26-year-old man with diagnoses of lupus erythematosus, antiphospholipid syndrome, chronic pulmonary thromboembolism, and chronic heart failure and over coagulation syndrome who develops diffuse alveolar hemorrhage and high-risk pulmonary thromboembolism is described, in order to exemplify the challenges for the treatment of diffuse alveolar hemorrhage.
