Abstract

CARRASCO-ALAMILLA, Romeo A.; FLORES-RAMIREZ, Carlos; LOPEZ-RIOS, Miguel R.  and  REYES-CHAVEZ, Sonia E.. Catheter-directed thrombolysis of the iliocaval segment in the prevention of the post-thrombotic syndrome. Rev. mex. angiol. [online]. 2019, vol.47, n.3, pp.16-21.  Epub Oct 25, 2021. ISSN 2696-130X.  https://doi.org/10.24875/rma.m19000004.

Objective:

To Compare the treatment with catheter-directed thrombolysis (CDT) of the iliac segment vs anticoagulation and its effect in the prevention of the post-thrombotic syndrome (PTS).

Methods:

Observational study of ambispective cohort in patients with deep vein thrombosis (DVT) of the iliac segment of the Central Hospital of the State of Chihuahua, during the period from March 2014 to June 2016. Patients were divided in two groups, one treated with anticoagulation and the other with CDT + anticoagulation to evaluate its effect on the prevention of SPT.

Results:

Of the 14 patients studied in each group, it was found that 5 (35.7%) had received anticoagulation plus CDT treatment for 2 years follow-up and had developed PTS classified by a score of 5 or more in the Villalta scale, against 6 (42.9%) patients who developed PTS of the other group treated with anticoagulation only, without existing statistically significant difference (T = 0.14; p = 0.70).

Conclusions:

Treatment of iliac segment DVT focuses on the reduction of the thrombotic load through different mechanisms for the prevention of squealae and decrease morbidity. In this study we could not demostrate the utility of invasive treatment for the prevention of PTS.
