Granulomatosis with polyangiitis is an autoimmune systemic granulomatous vasculitis that affects mainly the respiratory tract and kidney. However, it can affect nearly any organ, including the orbit. When it manifests as localized disease, is extremely rare and establishing the differential diagnosis of the eye lesions can be a challenge. We present a granulomatosis with polyangiitis case in a 42-year-old woman.
