Abstract

Introduction:

the gastrointestinal tract is a site frequently affected by HIV; however, in clinical practice some normal findings in endoscopy may not be, which leads these patients to not receive timely treatment for gastrointestinal pathologies. There is no study in Mexico that reports endoscopic and histopathological findings in HIV patients.

Objective:

Describe the endoscopic and histopathological findings of patients undergoing endoscopic procedures.

Material and Methods:

It is a retrospective descriptive observational study in patient patients with HIV who were referred for endoscopic procedures such as: panendoscopy, colonoscopy, rectosigmoidoscopy, ERCP. The study was conducted in the period from December 2017 to March 2019 in the Gastrointestinal endoscopy service of the Dr. Manuel Gea González General Hospital. Sociodemographic, clinical and paraclinical data were taken. 29 were made Panedoscopias, 23 colonoscopies, 8 Rectosigmoidoscopias, 2 ERCP e n 62 patients taking biopsy in all studies except for ERCP.

Results:

the average age was 36.05 (SD 10.092 years). The most frequent endoscopic diagnosis in the esophagus, stomach and duodenum was no abnormalities in 69% followed by Grade C- and D erosive sofagitis Angels 18%, when reviewing pathology slides findings were: esofago without abnormalities 66%, candidiasis that phage and erosive esophagitis 15%; In stomach chronic gastropathy in 56% and Gastritis by H. Pylori in 16%; In duodenum normal benign changes 76% and nonspecific chronic duodenitis 14%. In colon the most frequent diagnosis by Colonoscopy and Rectosigmoidoscopy was abnormal in more than 80% of cases (non- specific chronic colitis and nonspecific proctitis). When reviewing the histopathology report, the most reported finding in colonoscopy was non- specific chronic colitis and CMV with 33% each and in rectosigmoidoscopy It was CMV. In ERCP 1 patient was normal and another had HIV cholangiopathy

Conclusions:

endoscopic and pathological findings they agree with those reported in the literature, drawing attention in our study to a considerable number of opportunistic infections, especially in the colon and rectum. The most frequent opportunists were candida and cytomegalovirus. The agreement between the endoscopist and pathologist was very good only in esophagus and rectum; In Colon, the discordance between endoscopist and pathologist is very evident in our center. Therefore, in patients with HIV/AIDS who require endoscopy it would be important to always consider taking b iopsies in a protocolized way and to make a better diagnosis and treatment in these patients.
