Meningiomas are usually slow-growth tumors that arise from arachnoid cells. Extension of a cerebellopontine angle (CPA) meningioma to the external auditory canal is uncommon. A 64-year-old woman presented with unilateral and progressive hearing loss and tinnitus. A tumor was observed protruding into the external auditory canal. Magnetic resonance imaging showed a CPA tumor with extension to the middle ear, and external auditory canal. A subtotal resection (85%) was performed. Finally, on discharge from the hospital, she presented only a right sixth cranial nerve palsy. Meningiomas of the pontocerebellar angle can present with unilateral and progressive hearing loss and may extend to the external auditory canal.
