Third molars vary in their positions, which can complicate their surgical extraction. Various classifications, scales and indexes have been developed to predict this difficulty. Studies published between 2012-2024 in PubMed, Scielo, Elsevier and Redalyc were reviewed, focusing on tools to predict surgical difficulty. The Winter, Pell and Gregory, Sánchez Torres classifications; the Modified Romero Ruiz Scale, the Pederson, Koerner Index and the "New Index of Difficulty for the Extraction of Impacted Mandibular Third Molars" were compared. These tools use parameters such as angulation and depth to assess difficulty. Although periapical and panoramic radiographs are useful for surgical planning, they have limitations. Despite this, they provide a solid basis for the evaluation of surgical difficulty in the extraction of third molars.
