The structural problems of the health system, the economy and the social security system, make difficult the access to health services particularly for the elderly. In the State in Mexico, 42 per cent of the elderly don´t have access to the social security health services, the 33.1 per cent are pensioners, the 57 per cent are beneficaries of insured workers or pensioners. The elderely that don´t have access to the social security suffered the consequences of not having quality health services, in particular those facing chronic diseases which become catastrophic experiences, like those in he end stage of a chronic kidney disease with a high risk to die. In the State of Mexico, 21 per cent of the elderly have a chronic kidney disease and 30 per cent have high levels of colesterol (hypercholesterolemia). The high prevalence of chronic diseases among the elderly has deteriorated, therefore the need for help to realice básic daily activities has increased as well.
