The expert consensus group was formed in response to the growing need for a comprehensive approach to addressing polycystic ovary syndrome (PCOS) and its association with insulin resistance (IR). The main objective is to offer effective and practical guidance on the diagnosis and treatment of PCOS and IR through a review of the scientific literature and a discussion of clinical experiences. A two-day meeting was held in December 2021 and two days in December 2023, during which each expert presented their perspectives on the collected information. The information was subjected to discussion and consensus through an evaluation questionnaire, which was subsequently referenced and admitted as evidence. Using the Rotterdam 2023 criteria is recommended for diagnosing PCOS. Regarding the diagnosis of IR in the Mexican population, the HOMA-IR index or the triglycerides/glucose index is recommended. Comprehensive treatment, including lifestyle changes and insulin sensitizers, such as myoinositol and metformin, are recommended as first-line treatment. Timely diagnosis and appropriate treatment of IR in patients with PCOS could decrease the risk of associated complications in the short and long time.
