Large cell neuroendocrine carcinoma is a malignant neoplasm with a low incidence. In this report we present a case of a large cell neuroendocrine carcinoma in the stomach and a brief description of the multidisciplinary approach. We present a patient with epigastric pain, early satiety and weight loss with one month of evolution, contrasted CT scan report with a polypoid tumor lesion in the stomach measuring 13.6 × 8.8 cm, who underwent an upper endoscopy, finding a tumor that extended and deformed the middle of the gastric body to the lesser curvature, with intrinsic and extrinsic tumor growth, a cerebroid appearance and poorly defined borders, with areas of ulceration. Histopathology reported invasive, ulcerated, large cell neuroendocrine carcinoma, grade 3 (WHO). Cycles of chemotherapy based on etoposide and carboplatin were started. According to literature, neuroendocrine carcinoma is a rare and aggressive tumor that is generally diagnosed incidentally in advanced stages. The prognosis is poor, with a survival of 11 to 12 months in advanced stages, with surgery reaching a 5-year survival of up to 25.6%.
