Avulsion fracture of the distal tendon of the biceps brachii (TDB) is rare, with an incidence of 5.35 per 100,000/year. It is observed in athletes performing a forced eccentric brachial biceps contraction. The injuries are evaluated with the “hook test”. Tendon repair is performed in acute injuries involving > 50%. Usually, reinsertion through an incision is performed by fixation to the second cortex with a “button” type device. There is no consensus on the best repair method; surgical complications are heterotopic ossification, rigidity, external cutaneous neuropraxia of the antebrachial nerve, and injury to the posterior interosseous branch of the radial nerve.
