Sin embargo, a largo plazo se asocia a mayor riesgo de reintervención y complicaciones, como migración, endofugas, acodamiento y trombosis. Por esta razón, es necesario un seguimiento estricto y seriado para evaluar la permeabilidad de la endoprótesis y la presencia de endofugas. La Sociedad Americana de Cirugía Vascular recomienda el seguimiento de estos pacientes con angiotomografía computarizada seriada, esto aumenta la exposición a altas dosis de radiación y contraste yodado. El ultrasonido Doppler es una herramienta segura, pero poco eficaz para asegurar el adecuado seguimiento de estos pacientes. En los últimos años, el desarrollo de microburbujas aplicadas como medio de contraste ecográfico nos brinda una herramienta que permite realizar un análisis poco invasivo, en tiempo real, dinámico, altamente sensible y específico, sin uso de contraste yodado ni radiación ionizante para la identificación y caracterización de endofugas. La siguiente revisión tiene como objetivo describir los principios ecográficos del ultrasonido contrastado, su utilidad clínica y el protocolo de evaluación en aquellos pacientes en seguimiento postoperatorio de reparación endovascular de aneurisma de aorta.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Endovascular abdominal aortic aneurysm repair (EVAR) accounts for 85% of all procedures for aneurysmal lesions in the abdominal aorta and is associated with lower perioperative mortality and morbidity, 1.2 and 11.7% respectively, compared with open repair, 4.6 and 26.4% respectively. However, in the long term, it is associated with a higher risk of reintervention and complications, such as migration, endoleaks, kinking and thrombosis. For this reason, strict and serial follow-up is necessary to evaluate the patency of the endoprosthesis and the presence of endoleaks. The Society for Vascular Surgery recommends monitoring these patients with serial computed tomography angiogram, which increases exposure to high doses of radiation and iodinated contrast. Doppler ultrasound is a safe but ineffective tool to ensure adequate surveillance of these patients. In recent years, the development of microbubbles applied as an ultrasound contrast medium provides us with a tool that allows us to perform a non-invasive, real-time, dynamic, highly sensitive and specific flow analysis, without the use of iodinated contrast or ionizing radiation for identification and characterization of endoleaks. The following review aims to describe the ultrasound principles of contrast-enhanced ultrasound, its clinical applications and the evaluation protocol for surveillance in EVAR patients.]]></p></abstract>
