<abstract abstract-type="short" xml:lang="es"><p><![CDATA[Los Trastornos de la Ingestión Alimentaria de la Infancia o la niñez (TIAI) son de común presentación en las consultas de profesionales de la salud. Sin embargo, a menudo son confundidos con los Trastornos de la Conducta Alimentaria (TCA), Anorexia Nerviosa y Bulimia Nervosa, aunque estos permanecen como entidades diagnósticas separadas tanto en el DSM-IV-TR como en el ICD-10. En comparación con los TCA y con otros trastornos del comportamiento infanto-juvenil, los TIAI están escasamente investigados. Los términos utilizados para su descripción, la diversidad en la prevalencia, y las dificultades en la clasificación, afectan la identificación de estos problemas en la práctica clínica. Por ello, este artículo tiene como objetivo el repasar la literatura disponible sobre estos trastornos para así contribuir a una mejora en el entendimiento de los TIAIs.]]></p></abstract>

<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Palabras clave:</b> Trastornos de la Ingesti&oacute;n Alimentaria de la Infancia o la Ni&ntilde;ez, Revisi&oacute;n, Clasificaci&oacute;n.</font></p>          <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>              <p align="justify"><font face="verdana" size="2"><b>Introducci&oacute;n</b></font></p>              <p align="justify"><font face="verdana" size="2">Durante la infancia es com&uacute;n la presencia de periodos donde predomina el rechazo a la comida y los alimentos. Este comportamiento puede ser considerado como parte del desarrollo normal del ni&ntilde;o. La literatura refleja el hecho de que la mayor parte de los padres relatan que sus hijos, en un momento u otro de su crecimiento, presentan problemas relacionados con la ingesti&oacute;n de alimentos (Harris &amp; Booth, 1992). De hecho, los problemas relacionados con los trastornos de la ingesti&oacute;n alimentar&iacute;a de la infancia o la ni&ntilde;ez (TIAI) son motivo frecuente de consulta con profesionales de la salud (Graham, 1991).</font></p>  	    <p align="justify"><font face="verdana" size="2">A diferencia de los TIAI donde existe irregularidad en el proceso de ingesti&oacute;n oral de alimentos y l&iacute;quidos, los trastornos del comportamiento alimentario en adolescentes y adultos (TCA), como la Anorexia y Bulimia Nervosa, se caracterizan fundamentalmente por el rechazo a mantener un peso corporal igual o por encima del valor m&iacute;nimo establecido por talla y peso con la alteraci&oacute;n de la percepci&oacute;n de la silueta corporal (DSM&#45;IV, APA, 2000).</font></p>          <p align="justify"><font face="verdana" size="2">Los TIAI pueden presentarse con sintomatolog&iacute;a de exceso o d&eacute;ficit de peso o &iacute;ndice de masa muscular, malnutrici&oacute;n, estre&ntilde;imiento, c&oacute;licos, retrasos del desarrollo infantil, periodos de depresi&oacute;n y otros tipos de s&iacute;ntomas f&iacute;sicos o psicol&oacute;gicos que se encuentran directa o indirectamente relacionados con la conducta y el proceso alimentario. En el desarrollo de esta sintomatolog&iacute;a participan un complejo y entramado grupo de factores, relacionados con las caracter&iacute;sticas del ni&ntilde;o y de los padres y el ambiente que les rodea. Factores relacionados con el ni&ntilde;o incluyen su temperamento, su estado de salud, o su experiencia durante el proceso de aprendizaje alimentario. Relacionado con los padres habr&iacute;a que incluir, los conocimientos sobre la alimentaci&oacute;n, sus habilidades educativas, su salud f&iacute;sica y emocional, o su relaci&oacute;n de pareja. Todas estas circunstancias van a jugar un papel contribuyente e impactar en las relaciones paterno/materno&#45;filiales, hasta el punto de llegar a convertirse en un contexto de c&iacute;rculos viciosos donde los condicionamientos negativos mantienen los s&iacute;ntomas de los TIAI del ni&ntilde;o (Kedesdy &amp; Budd, 1998).</font></p>          <p align="justify"><font face="verdana" size="2">Aunque la prevalencia del TCA es considerablemente inferior a los TIAI (Ramsay &amp; Veroff, 1998), los estudios cl&iacute;nicos de estos &uacute;ltimos, son realmente escasos. Por ejemplo, llama la atenci&oacute;n el hecho de que estos trastornos no sean citados en el Manual Pr&aacute;ctico de Nutrici&oacute;n en Pediatr&iacute;a elaborado por el Comit&eacute; de Nutrici&oacute;n de la Asociaci&oacute;n Espa&ntilde;ola de Pediatr&iacute;a (Mu&ntilde;oz &amp; Su&aacute;rez, 2007) donde s&oacute;lo se citan los diagn&oacute;sticos de Anorexia Nervosa y Bulimia Nervosa en adolescentes. La situaci&oacute;n actual que atraviesan los TIAI, tanto en la literatura como en la pr&aacute;ctica cl&iacute;nica, parecen asemejarse a la posici&oacute;n que ocupaban los TCA, en los a&ntilde;os sesenta. Por ello, el presente art&iacute;culo tiene como objetivo el repasar los aspectos m&aacute;s importantes de los TIAI citados en la literatura, para as&iacute; contribuir a una mejora en el entendimiento de estos problemas.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p> 	         <p align="justify"><font face="verdana" size="2"><b>Conceptos b&aacute;sicos</b></font></p> 	         <p align="justify"><font face="verdana" size="2">El comportamiento humano en la ingesti&oacute;n de alimentos es un proceso complejo que requiere una perfecta integraci&oacute;n de varios sistemas biol&oacute;gicos y psicol&oacute;gicos que interact&uacute;an de forma efectiva y sin&eacute;rgica con el entorno ambiental, (Dovey, 2010; Kervin 1999). Una m&iacute;nima dificultad en cualquiera de estos sistemas puede crear un 'efecto domino' resultando en la formaci&oacute;n de los TIAI (Kedesdy &amp; Budd, 1998). Para entender los TIAI es primordial primero repasar las definiciones utilizadas en la literatura as&iacute; como las cifras de incidencia y las dificultades en su clasificaci&oacute;n.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><i>Complejidad del vocabulario: Definiciones</i></font></p>          <p align="justify"><font face="verdana" size="2">La primera complejidad en la investigaci&oacute;n de estos trastornos se deriva de la adaptaci&oacute;n de la terminolog&iacute;a anglosajona cuyo vocabulario distingue entre el acto de 'dar de comer/alimentar' (feeding) y el de 'comer/alimentarse' (eating). El primero refleja un acto din&aacute;mico y de relaci&oacute;n estrecha entre el que alimenta (padres, cuidadores etc.) y el que es alimentado (generalmente ni&ntilde;os hasta que son capaces de comer aut&oacute;nomamente). El segundo implica el desarrollo de la habilidad y la capacidad de ingerir la alimentaci&oacute;n. Como consecuencia las descripciones utilizadas para describir los TIAI var&iacute;an desde "rechazo a la comida", "falta de apetito", "neofobia', 'fobia a la comida", o "Anorexia infantil".</font></p>  	    <p align="justify"><font face="verdana" size="2">El siguiente problema que afecta a la definici&oacute;n de los TIAI, es derivado del modelo m&eacute;dico utilizado hasta la fecha para su identificaci&oacute;n, lo que significa que cuando la proporci&oacute;n peso/talla y el &iacute;ndice de masa corporal es adecuado a las escalas de desarrollo establecidas, independientemente de las dificultades alimentarias del ni&ntilde;o, la patolog&iacute;a no se describe. Sin embargo, la evidencia cl&iacute;nica resalta que la distinci&oacute;n centrada en detalles de peso/talla es insuficiente, lo que sugiere la necesidad de revisar los manuales diagn&oacute;sticos existentes.</font></p>  	    <p align="justify"><font face="verdana" size="2">Hasta la fecha hay muy pocos art&iacute;culos sobre el TIAI que describan los comportamientos observables en estos ni&ntilde;os. Teniendo en cuenta esta ausencia en la literatura, Dovey, Farrow y Martin (2009) han descrito los principales s&iacute;ntomas cl&iacute;nicos asociados al comportamiento de rechazo a la comida y alimentos (food refusal). Estos autores recomiendan el definir los TIAI como un t&eacute;rmino general que abarque a todos aquellos ni&ntilde;os que presenten una inhabilidad y/o falta de inter&eacute;s o motivaci&oacute;n para la ingesti&oacute;n de alimentos (Dovey &amp; Martin, 2010). El problemas es que esto abarcar&iacute;a muchas conductas que no tendr&iacute;an porque denominarse como trastornos.</font></p>  	    <p align="justify"><font face="verdana" size="2"><i>Prevalencia: Diversidad de cifras</i></font></p>  	    <p align="justify"><font face="verdana" size="2">Los &iacute;ndices de prevalencia de los TCA, Anorexia y Bulimia Nervosa, en la poblaci&oacute;n general adulta est&aacute;n establecidos sobre el 0.3% (Hoek &amp; Van Hoeken, 2003; Favaro, Ferrara, Santonastaso, 2004) y el 1.1% (Garfinkel et al., 1995; Hudson, Hiripi, Pope &amp; Kessler, 2007) respectivamente. En comparaci&oacute;n con estas cifras, los &iacute;ndices de prevalencia de los TIAI son considerablemente m&aacute;s elevados, ya que se estima que afectan al 25% de los ni&ntilde;os en alg&uacute;n momento de su desarrollo (Ramsay &amp; Veroff, 1998). Su elevada incidencia podr&iacute;a ser explicada por la falta de criterios claros que definan el trastorno.</font></p>          <p align="justify"><font face="verdana" size="2">Sin embargo, estos &iacute;ndices de prevalencia var&iacute;an considerablemente dependiendo de la definici&oacute;n que se utilice y de la fuente de informaci&oacute;n. Por ejemplo, cuando los padres son los encuestados estos &iacute;ndices se pueden elevar hasta un 50.9% (Coulthard &amp; Harris, 2003), a diferencia de la identificaci&oacute;n de estos trastornos por profesionales de la salud donde 1.4% de los ni&ntilde;os entre 3 y 12 meses de edad son diagnosticados con rechazo a la comida, v&oacute;mitos y c&oacute;licos (Dahl &amp; Sundelin, 1986). La prevalencia tambi&eacute;n varia dependiendo de el grupo de ni&ntilde;os que se estudie, por ejemplo la literatura describe el hecho de que los TIAI alcanzan una alta prevalencia en ni&ntilde;os con retraso cognitivo (80%), con discapacidades f&iacute;sicas (26%&#45;90%); con dificultades para aprendizaje (23%&#45;43%) y ni&ntilde;os prematuros (10%&#45;49%) (Crist, McDonnell, &amp; Beck, 1994; Douglas &amp; Byron, 1996; Linscheid, Budd, &amp; Rasnake, 2003; Manikan &amp; Perman, 2000; Palmer, Thompson, &amp; Linscheid, 1975).</font></p>      <p align="justify"><font face="verdana" size="2">Por &uacute;ltimo, varios autores han se&ntilde;alado que entre el 3% y el 10% de los ni&ntilde;os que experimentan trastornos severos y persistentes del comportamiento alimentario tambi&eacute;n sufren un deterioro general en su salud (Dahl &amp; Sundelin 1992; Lindberg, Bohlin, &amp; Hagekull, 1991; Reau, Senturia, Lebailly, &amp; Christoffel, 1996). Estos ni&ntilde;os est&aacute;n en riesgo de malnutrici&oacute;n y posteriores p&eacute;rdidas significativas de peso, que puede impactar en su desarrollo f&iacute;sico, cognitivo y emocional (Budd et al, 1992; Riordan, Iwata, Wohl, y Finney, 1980).</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p> 	         <p align="justify"><font face="verdana" size="2"><b>Clasificaci&oacute;n</b></font></p> 	         ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Los dos manuales de diagn&oacute;stico utilizados por los profesionales, el Manual Estad&iacute;stico y Diagn&oacute;stico de los Trastornos Mentales (DSM&#45;IV, APA, 2000) y la Clasificaci&oacute;n Internacional de Enfermedades (ICD&#45;10, World Health Organization, WHO 1992) incluyen una categor&iacute;a para los trastornos de la ingesti&oacute;n alimentaria de la infancia y la ni&ntilde;ez. As&iacute; en el DSM&#45;IV este diagn&oacute;stico es parte de la categor&iacute;a de los 'Trastornos de la Ingesti&oacute;n y de la Conducta Alimentaria de la Infancia o la Ni&ntilde;ez donde se encuentran los trastorno de la ingesti&oacute;n alimentaria de la infancia o la ni&ntilde;ez, trastornos de pica y trastorno de rumiaci&oacute;n. En la Clasificaci&oacute;n Internacional de Enfermedades versi&oacute;n 10, este diagn&oacute;stico es parte de "Otros trastornos emocionales y del comportamiento que aparecen habitualmente en la ni&ntilde;ez y en la adolescencia". Ambos manuales establecen el criterio sobre la base de la existencia de un fallo persistente para la ingesti&oacute;n de alimentos con dificultades significantes para ganar peso siempre y cuando no est&eacute; asociado a problemas m&eacute;dicos o trastornos mentales y clasifican estos problemas en categor&iacute;as y apartados claramente separados de los TCA. Sin embargo ninguno de estos manuales especifica cu&aacute;l ha de ser la severidad de los s&iacute;ntomas para llegar a este diagn&oacute;stico (Davies et al., 2006; Bryant&#45;Waugh, Markham, Kreipe, &amp; Walsh, 2010)</font>.</p> 	         <p align="center"><font face="verdana" size="2"><img src="/img/revistas/rmta/v3n2/a2f1.jpg"></font></p> 	         <p align="justify"><font face="verdana" size="2">Varios estudios han destacando el hecho de que el criterio propuesto por los manuales profesionales es ambiguo e insuficiente para hacer frente a la extensa gama de TIAI cuyo origen es claramente multifactorial (Manikan &amp; Perman, 2000). Resulta obvio que los criterios actuales basados exclusivamente en cifras de peso/altura est&aacute;n excluyendo a muchos ni&ntilde;os que padecen otros tipos de TIAI. Las clasificaciones ofrecidas por algunos autores (Burklow, Phelps, Schultz, McConnell &amp; Rudolph, 1998; Chatoor &amp; Ganivan, 2003; Dovey et al., 2009; Kedesdy &amp; Budd, 1998; Lindscheid, Budd &amp; Rasnake, 2003), y que encajan m&aacute;s adecuadamente con lo hallado en la pr&aacute;ctica cl&iacute;nica est&aacute;n aun siendo estudiadas para su inclusi&oacute;n en los nuevos manuales oficiales. Dovey y colegas (2009) expandiendo los trabajos de Cha&#45;toor y Ganiban (2003) han sugerido una clasificaci&oacute;n alternativa que se ajusta a la pr&aacute;ctica cl&iacute;nica y que identifica diferentes subtipos de 'rechazo de alimentos/comida': (1) consecuentes de complicaciones m&eacute;dicas&#45;biol&oacute;gicas; (2) dependientes del aprendizaje (3) de caracter&iacute;sticas selectivas; (4) basados en ansiedad y miedos condicionados y (5) basados en el desarrollo del apetito y la experiencia de autonom&iacute;a. Estos autores han presentado evidencias de casos cl&iacute;nicos (Dovey et al, 2010a) y han desarrollado estrategias para la intervenci&oacute;n cl&iacute;nica (Dovey et al., 2010b).</font></p>      <p align="justify"><font face="verdana" size="2">Bryant&#45;Waugh y colegas (2010) han completado una revisi&oacute;n de la literatura sobre los criterios diagn&oacute;sticos en los TIAI destacando los numerosos problemas referidos a la terminolog&iacute;a y a limitadas l&iacute;neas de investigaci&oacute;n abiertas. Estos autores recomiendan cambios en el futuro DSM&#45;V sugiriendo la inclusi&oacute;n de aspectos diet&eacute;ticos, comportamentales y de relaci&oacute;n familiar. No cabe la menor duda de que un sistema de clasificaci&oacute;n actualizado debe de tener en cuenta las caracter&iacute;sticas multifactoriales de estos trastornos y algunos autores ya han empezado a proclamar la necesidad de aplicar un modelo bio&#45;psicosocial para su entendimiento global (Silverman et al., 2010).</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p> 	         <p align="justify"><font face="verdana" size="2"><b>Etiolog&iacute;as multifactoriales</b></font></p> 	         <p align="justify"><font face="verdana" size="2">El consenso actual es que los TIAI emergen de una combinaci&oacute;n de factores de car&aacute;cter m&eacute;dico, fisiol&oacute;gico, social, ambiental y socio&#45;familiar que forman un entramado muy complejo (Birch, McPhee, Steinberg, &amp; Sullivan, 1999; Burklow, 1998; Byars, 2003; Casey y Rozin, 1987; Heffer &amp; Kelly, 1994; Kerwing, 1999; Ramsay &amp; Veroff, 1998; Rommel, DeMeyer, Feenstra, &amp; Veereman&#45;Wauters, 2003, Silverman 2010; Skuse, Wolke &amp; Reilly, 1992). La literatura tambi&eacute;n establece que los TIAI pueden ser explicados y entendidos desde la teor&iacute;a del aprendizaje. Conceptos como el condicionamiento cl&aacute;sico y operante ayudan a definir espec&iacute;ficamente el acto de 'comer' como un comportamiento que incluye una interacci&oacute;n con el entorno y dentro del tradicional esquema de antecedentes&#45;conducta&#45;consecuentes (ABC serian sus siglas en ingles).</font></p>          <p align="justify"><font face="verdana" size="2">Un ejemplo de c&oacute;mo las etiolog&iacute;as biol&oacute;gicas y conductuales interact&uacute;an para el desarrollo y el mantenimiento de estos problemas lo encontramos en ni&ntilde;os que padecen reflujo gastroesof&aacute;gico (RGE). Esta condici&oacute;n se diagnostica cuando el esf&iacute;nter esof&aacute;gico inferior (EEI) no se cierra adecuadamente o permanece abierto permitiendo que los contenidos del est&oacute;mago asciendan hacia el es&oacute;fago. Este problema produce una regurgitaci&oacute;n de los alimentos ingeridos y de los &aacute;cidos del est&oacute;mago causando una sensaci&oacute;n de quemaz&oacute;n en el pecho o en la garganta y un dolor epig&aacute;strico (McColl, Junghard, Wiklund &amp; Revicki, 2005). Un ni&ntilde;o con este problema m&eacute;dico sufre dolor f&iacute;sico asociado a la ingesti&oacute;n de alimentos. Esta asociaci&oacute;n (alimento/dolor) es aprendida a trav&eacute;s de condicionamientos cl&aacute;sicos y operantes, que resulta muy resistente al cambio (Bolles, Hayward &amp; Crandall., 1981; Garcia, Hankins &amp; Rusiniak, 1974; Rozin &amp; Kalat, 1971). Adem&aacute;s, como la n&aacute;usea juega un papel crucial en la adquisici&oacute;n del gusto (Garc&iacute;a et al., 1974) este problema impacta en el desarrollo del gusto, limitando la variedad de alimentos que acepta el ni&ntilde;o. Como resultado de la asociaci&oacute;n entre comer y dolor el ni&ntilde;o puede desarrolla conductas para evitar la ingesti&oacute;n de alimentos, por ejemplo, pelearse con la cuchara cuando se le ofrece comida, dar patadas a la mesa etc, que pueden ser reforzadas por el &eacute;xito obtenido en evitar la comida. En un estudio realizado por Kerwin (2004) se encontr&oacute; que RGE hab&iacute;a sido diagnosticada en el 56 % de los ni&ntilde;os que presentaban TIAI de car&aacute;cter severo.</font></p>      <p align="justify"><font face="verdana" size="2"><i>Conductas problem&aacute;ticas a la hora de las comidas</i></font></p>  	    <p align="justify"><font face="verdana" size="2">Las conductas negativas y problem&aacute;ticas a la hora de las comidas pueden surgir tanto si el ni&ntilde;o ingiere la comida de forma oral como si lo hace a trav&eacute;s de sondas nasog&aacute;stricas (Burklow, McGrath, Allred, &amp; Rudolph, 2002).</font></p>          ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Crist, Nappier &amp; Phillips (2001) encontraron que la mayor&iacute;a de los padres (51%) de su estudio informaron de que sus ni&ntilde;os exhib&iacute;an conductas problem&aacute;ticas como por ejemplo, levantarse de la mesa sin consentimiento y antes de terminar la comida, rechazar la comida cuando est&aacute; en la mesa pero reclamar alimentos cuando todos han terminado, pataletas a la hora de las comida etc. Adem&aacute;s el 21% de estos padres indic&oacute; que sus hijos presentaban cuatro o m&aacute;s de estos comportamientos problem&aacute;ticos y oposicionales. Las diferencias en los patrones de comportamiento a la hora de las comidas entre los ni&ntilde;os con TIAI y sin ella depende en la funci&oacute;n, la intensidad y la frecuencia del comportamiento problem&aacute;tico.</font></p>      <p align="justify"><font face="verdana" size="2">Burklow y colegas (1998) estudiaron los factores com&oacute;rbidos relacionados con los TIAI y encontraron que los 'problemas de conducta a la hora de las comidas' era el factor que se asociaba m&aacute;s frecuentemente a estos trastornos (85%) incluso con m&aacute;s frecuencia que los problemas neurol&oacute;gicos (73%), las deficiencias estructurales (57%), los problemas cardio&#45;respiratorios (7%) o las disfunciones metab&oacute;licas (5%). Estos autores encontraron que la mayor&iacute;a (80%) de los ni&ntilde;os de su estudio, que inclu&iacute;a 103 participantes, exhib&iacute;an conductas dif&iacute;ciles y desafiantes asociadas con las comidas y esto era concomitante con su trastorno alimentario. Estas cifras encajan con los resultados de un estudio reciente en una cl&iacute;nica de TIAI donde el escrutinio inicial revel&oacute; que el 74% de los ni&ntilde;os all&iacute; atendidos exhib&iacute;a conductas negativas asociadas a la ingesti&oacute;n de alimentos con una gran elevada frecuencia e intensidad aunque este aspecto s&oacute;lo hab&iacute;a sido mencionado en un 6% de los informes de otros profesionales de la salud que hab&iacute;an derivado los casos (Martin &amp; Dovey 2010).</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p> 	         <p align="justify"><font face="verdana" size="2"><b>Diagn&oacute;stico y tratamiento multiprofesional</b></font></p> 	         <p align="justify"><font face="verdana" size="2">El origen y el car&aacute;cter multifactorial de los TIAI conlleva la necesidad de incluir la presencia de equipos multidisciplinarios para establecer su diagn&oacute;stico y tratamiento (Arvedson &amp; Brodsky, 2002; Manikam &amp; Perman, 2000; Martin, Southall, Shea &amp; Marr, 2008; Puntis 2007).</font></p>          <p align="justify"><font face="verdana" size="2"><i>Equipo para la valoraci&oacute;n cl&iacute;nica</i></font></p>  	    <p align="justify"><font face="verdana" size="2">El diagn&oacute;stico y tratamiento de estos trastornos es un proceso complejo que ha de incluir la evaluaci&oacute;n de varios profesionales de la salud, cada uno desempe&ntilde;ando un papel espec&iacute;fico.</font></p>  	    <p align="justify"><font face="verdana" size="2">Los pediatras valoran de forma objetiva el crecimiento del ni&ntilde;o con una completa exploraci&oacute;n cl&iacute;nica y antropom&eacute;trica. Los recientes avances tecnol&oacute;gicos ofrecen modelos sofisticados para el estudio de la composici&oacute;n corporal y los &iacute;ndices de masa corporal. McCarthy, Cole, y Fry (2006) publicaron las primeras curvas pedi&aacute;tricas que inclu&iacute;an los percentiles de masa corporal. Esto ha permitido comparar las medidas de crecimiento de un ni&ntilde;o con su poblaci&oacute;n de referencia. Una de las tareas de los pediatras (y en algunos pa&iacute;ses de los diet&oacute;logos y expertos en nutrici&oacute;n) consiste en vigilar la nutrici&oacute;n y el desarrollo infanto&#45;juvenil. Ellos son los encargados de evaluar el estado nutricional del ni&ntilde;o identificando patolog&iacute;as debidas a carencias nutricionales. Los requerimientos nutricionales se definen como la cantidad de energ&iacute;a y nutrientes que se necesitan para mantener el crecimiento, la salud y un determinado nivel de actividad f&iacute;sica. Algunos ni&ntilde;os precisan de nutrici&oacute;n enteral y parenteral, que incluye la administraci&oacute;n de suplementos alimentarios y la alimentaci&oacute;n proporcionada a trav&eacute;s de sondas nasog&aacute;stricas o tubos percut&aacute;neos.</font></p>  	    <p align="justify"><font face="verdana" size="2">En algunos pa&iacute;ses los especialistas en gastroenterolog&iacute;a son los encargados de evaluar las condiciones del aparato digestivo infantil as&iacute; como la exploraci&oacute;n del proceso de degluci&oacute;n en el ni&ntilde;o para confirmar la ausencia o presencia de complicaciones m&eacute;dico&#45;fisiol&oacute;gicas o estructurales. Los terapeutas del lenguaje y/o los logopedas eval&uacute;an las habilidades oromotoras y los psic&oacute;logos tienen la tarea de evaluar el desarrollo cognitivo y emocional del ni&ntilde;o, su comportamiento a la hora de las comidas, la interacci&oacute;n entre &eacute;ste y sus padres o los adultos responsables de su crianza as&iacute; como la labor de identificar la presencia de posibles problemas de salud mental en la familia. Algunos ni&ntilde;os y/o sus familias pueden tambi&eacute;n presentar trastornos graves de salud mental asociados que requerir&aacute;n la intervenci&oacute;n del especialista en psiquiatr&iacute;a infantil.</font></p>          <p align="justify"><font face="verdana" size="2"><i>Evaluaci&oacute;n y tratamiento</i></font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">La etiolog&iacute;a de los TIAI es multi&#45;compleja por lo que necesita un modelo bio&#45;psicosocial para su evaluaci&oacute;n y tratamiento (Kerwin, 1999; Silverman et al, 2010). La evaluaci&oacute;n de estos ni&ntilde;os se puede llevar a cabo tanto en centros de d&iacute;a (Douglas, 2001) como en plantas de pediatr&iacute;a en hospitales generales o en unidades hospitalarias especializadas en el tratamiento de los TIAI (Byars et al 2003; Martin et al, 2008; Patel et al, 2006). La valoraci&oacute;n multifactoria&#45;les requiere la evaluaci&oacute;n de aspectos fisiol&oacute;gicos, nutricionales, comportamentales y de interacci&oacute;n paterno/materno&#45;filial. El tratamiento de estos ni&ntilde;os incluye el entrenamiento de padres en el manejo de comportamientos problem&aacute;ticos infantiles exhibidos a la hora de las comidas. Los tratamientos pueden ser realizados a nivel hospitalario (Ahearn, Kerwin, Eicher, Shantz, &amp; Swearingin, 1996; Hoch, Babbitt, Coe, Krell, &amp; Hackbert, 1994; Martin, Southall, Shea &amp; Marr, 2008; Piazza, Patel, Gulotta, Sevin, &amp; Layer, 2003) o en sus propios hogares familiares (Brown, Spencer &amp; Swift, 2002; Werler, Murphy &amp; Budd, 1998).</font></p>      <p align="justify"><font face="verdana" size="2"><i>Estrategias empleadas por los padres</i></font></p>  	    <p align="justify"><font face="verdana" size="2">Crist, Nappier &amp; Phillips (2001) encontraron que padres con hijos con TIAI y conductas problem&aacute;ticas a la hora de las comidas describ&iacute;an el uso de estrategias como 'hacer diferentes comidas para cada miembro de la familia' (16.6%), 'engatusar y halagar a sus hijos para que coman' (11.5%).</font></p>  	    <p align="justify"><font face="verdana" size="2">Cuando los ni&ntilde;os empiezan a imitar a los padres y quieren comer de forma m&aacute;s independiente en la mesa familiar, el conflicto, entre lo que el ni&ntilde;o quiere comer y lo que los padres consideran que debe consumir, emerge. Este conflicto 'natural' unido a las estrategias que los padres empleen para solucionarlo va a tener un impacto en c&oacute;mo el ni&ntilde;o acepta los alimentos y en el desarrollo de &eacute;ste aspecto en el futuro. El tratamiento inadecuado de los problemas del comportamiento a la hora de las comidas unido a problemas menores que el ni&ntilde;o est&eacute; ya presentando, por ejemplo, el tener cierta sensibilidad a la temperatura de los alimentos etc., puede conducir a situaciones de alto nivel de estr&eacute;s respecto a la ingesti&oacute;n de alimentos (Harris &amp; Booth, 1992).</font></p>          <p align="justify"><font face="verdana" size="2">Muchos autores coinciden en que las estrategias usadas por algunos padres en su intento desesperado para hacer que sus hijos coman, puede exacerbar los peque&ntilde;os problemas de alimentaci&oacute;n ya existentes, hasta convertir la hora de las comidas en una experiencia negativa (Harris &amp; Booth, 1992; Blissett, Harris &amp; Kirk, 2000). Fisher &amp; Birch (1999) encontraron que las t&eacute;cnicas 'excesivamente controladoras' tales como presionar al ni&ntilde;o para que coma o restringirle la ingesti&oacute;n de alimentos, estaban asociados con la presentaci&oacute;n de mayores problemas en el comportamiento alimentario. La presi&oacute;n para comer estaba asociada a la reducci&oacute;n de la preferencia por los alimentos mientras que la restricci&oacute;n aparec&iacute;a conectada con una mayor consumici&oacute;n de productos restringidos.</font></p>      <p align="justify"><font face="verdana" size="2"><i>Intervenci&oacute;n de car&aacute;cter conductual</i></font></p>  	    <p align="justify"><font face="verdana" size="2">Varios estudios (e.g., Ahearn et al., 1996; Kelley, Piazza, Fisher &amp; Oberdroff, 2003; Kerwin, 1999, Patel, Piazza, Martinez, Volkert, &amp; Santana, 2002; Riordan, Iwata, Finney, Wohl, &amp; Stanley, 1984) han demostrado que los tratamiento que est&aacute;n basados en los principios del condicionamiento operante son efectivos para conseguir que los ni&ntilde;os con TIAI aumenten el consumo de alimentos. Por ejemplo, las modificaciones de la conducta supusieron la proporci&oacute;n m&aacute;s elevada (76%) de intervenci&oacute;n entre los estudios revisados por Kerwin (1999). En estos estudios, la intervenci&oacute;n conductual incluye el disminuir los comportamientos negativos que mantienen el problema alimentario; incrementar la variedad y la cantidad de comida que consum&iacute;an; disminuir su aversi&oacute;n a las comidas; progresar de sus dietas de car&aacute;cter limitado; ingerir diferentes texturas y superar el rechazo generalizado a los alimentos (Babbit et al., 1994; Byars et al., 2003; Piazza, Patel et al., 2003). Estas intervenciones psicol&oacute;gicas que tienen como objetivo el modificar el comportamiento se pueden aplicar de forma aislada o unidas a otros componentes de tratamiento (Douglas, 2000; Kerwin, 1999; Linscheid, Oliver, Blyer &amp; Palmer, 1978).</font></p>          <p align="justify"><font face="verdana" size="2">Varios investigadores (e.g., Ahearn et al., 1996; Piazza, Fisher et al., 2003) han abogado por el estudio de las caracter&iacute;sticas funcionales de los TCAI con el &aacute;nimo de desarrollar intervenciones de car&aacute;cter 'funcional&#45;conductual'. Desde esta perspectiva, estudios recientes han aplicado la metodolog&iacute;a del an&aacute;lisis funcional (AF) de la conducta al &aacute;rea de los TIAI (Galensky, Miltenberger, Stricker, &amp; Garling&#45;house, 2001; Girolami &amp; Scotti, 2001). El AF es una evaluaci&oacute;n cl&iacute;nica usada para identificar aquellos reforzadores ambientales que est&eacute;n manteniendo el comportamiento inadecuado infantil a la hora de las comidas (Piazza &amp; Vaz, 2010). Un ejemplo lo encontramos en los trabajos de Piazza, Fisher y colegas (2003) que usaron AF para la evaluaci&oacute;n de 15 ni&ntilde;os con TIAI de car&aacute;cter grave y que hab&iacute;an sido ingresados en una unidad m&eacute;dica especializada en el tratamiento de estos trastornos. Los resultados de su estudio contribuyeron a la evidencia de que el refuerzo negativo, en forma de escape de la situaci&oacute;n de 'comer', actuaba como un factor de mantenimiento de la conducta inapropiada exhibida por estos ni&ntilde;os a la hora de las comidas. Otros estudios que han investigado estos aspectos han demostrado que esta funci&oacute;n podr&iacute;a ser alterada a trav&eacute;s de la manipulaci&oacute;n de factores contextuales o de las caracter&iacute;sticas de la comida (Pace et al., 1993; Patel et al., 2006).</font></p>  	    <p align="justify"><font face="verdana" size="2">Tambi&eacute;n hay evidencia cient&iacute;fica respecto a la eficacia del empleo de t&eacute;cnicas de modificaci&oacute;n del comportamiento, tales como el uso de reforzadores sociales y modelaje (modelling), para ense&ntilde;ar por ejemplo a usar la cuberter&iacute;a para comer (e.g., Berkowitz, Sherry &amp; Davis, 1971; O'Brien &amp; Azrin, 1972). Otras t&eacute;cnicas tales como el uso de mini&#45;comidas e instrucciones grupales tambi&eacute;n se han empleado para apoyar el aprendizaje de comer de forma independiente.</font></p>          <p align="justify"><font face="verdana" size="2">Sin embargo hay ni&ntilde;os que no responden a estas intervenciones, por ejemplo Luiselli y Gleason (1987) describieron el caso de un ni&ntilde;o discapacitado que rechazaba sistem&aacute;ticamente la comida y que carec&iacute;a de comunicaci&oacute;n verbal y habilidades para auto&#45;alimentarse. Estos autores aplicaron una intervenci&oacute;n conductual en las que incluyeron est&iacute;mulos sensoriales, como luces, movimientos de mecer y reforzadores. Los resultados de su estudio sugieren que reforzadores de car&aacute;cter sensorial pueden ser empleados eficazmente en ni&ntilde;os con TIAI y discapacidades asociadas.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">&nbsp;</font></p> 	         <p align="justify"><font face="verdana" size="2"><b>A modo de conclusi&oacute;n</b></font></p> 	         <p align="justify"><font face="verdana" size="2">Los TIAI, a pesar de ser de presentaci&oacute;n com&uacute;n y de estar clasificados separadamente de los TCA, no han sido investigados tan frecuentemente como estos &uacute;ltimos. Esto se puede explicar por la terminolog&iacute;a que se utilizar para definir estos trastornos, las conflictivas figuras de prevalencia y los sistemas de clasificaci&oacute;n utilizados.</font></p>      <p align="justify"><font face="verdana" size="2">La etiolog&iacute;a de estos trastornos as&iacute; como el tratamiento debe de ser multi&#45;profesional donde se incluya la teor&iacute;a del aprendizaje social que podr&iacute;a explicar el mantenimiento de algunos los TIAI. Seg&uacute;n esta perspectiva, las condiciones de las relaciones interpersonales, a la hora de las comidas actuar&iacute;an como factores contribuyentes o de mantenimiento de los TIAI. Intervenciones y tratamientos aplicados desde esta perspectiva est&aacute;n aportando evidencia emp&iacute;rica de su eficacia para combatir estos trastornos. Los estudios de investigaci&oacute;n que siguen este enfoque han contribuido a esclarecer el papel que los factores ambientales tienen en el mantenimiento de estos trastornos.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p> 	         <p align="justify"><font face="verdana" size="2"><b>Referencias</b></font></p> 	         <!-- ref --><p align="justify"><font face="verdana" size="2">Ahearn, W.
