Se utilizaron las bases de datos Medline/Pub-Med, Scopus y metabuscador Google Scholar para recoger artículos publicados entre enero de 1990 y mayo de 2013. Se analizó un total de 104 publicaciones. Los parámetros de composición corporal son los más utilizados para valorar los resultados de los estudios, seguidos de aquellos que estiman la capacidad aeróbica máxima. Los artículos de intervención se presentan con unas características metodológicas muy heterogéneas, aunque existen tendencias claras en el uso de determinados aspectos. En cuanto a las revisiones, la mayoría son sistemáticas e incluyen metaanálisis; el índice de masa corporal (IMC) es el parámetro más utilizado.]]></p></abstract>

In this studies, body mass index (BMI) is the most used parameter.</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Keywords:</b> review; motor activity; school health; methodology; child.</font></p>  	    <p>&nbsp;</p>  	    <p align="justify"><font face="verdana" size="2">El ejercicio f&iacute;sico, entendido en t&eacute;rminos de gasto energ&eacute;tico realizado con el objetivo de mejorar la condici&oacute;n f&iacute;sica (CF) es una importante herramienta terap&eacute;utica para prevenir y tratar problemas de salud, entre los que destaca la obesidad infantil debido a su incremento considerable en los &uacute;ltimos a&ntilde;os.<sup>1,2</sup> La distribuci&oacute;n de grasa o la condici&oacute;n cardiovascular est&aacute; directamente relacionada con los niveles de fuerza, velocidad, resistencia, flexibilidad y coordinaci&oacute;n,<sup>2,3</sup> as&iacute; como el estado de salud de los m&aacute;s j&oacute;venes, tal y como evidencian actuales investigaciones.<sup>3,4</sup> La pr&aacute;ctica continuada de actividades que implican el movimiento de huesos y m&uacute;sculos, es decir, actividad f&iacute;sica (AF), en la infancia y adolescencia consigue disminuir la masa grasa y mejorar la composici&oacute;n corporal,<sup>3,5,6</sup> aumenta la CF y cardiovascular,<sup>7&#45;9</sup> mejora el contenido mineral oseo<sup>5,10</sup> y combate el sedentarismo entre la poblaci&oacute;n m&aacute;s joven de manera eficaz.<sup>11&#45;13</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Estos amplios efectos que tiene sobre la salud el ejercicio de manera particular, y la AF de forma general, hacen que el desarrollo de h&aacute;bitos activos en la poblaci&oacute;n infantil y juvenil tome cada vez m&aacute;s importancia en la sociedad. En este sentido, se aprecia una tendencia en esta &uacute;ltima d&eacute;cada hacia el aumento de propuestas, a trav&eacute;s de programas de AF extra&#45;curriculares, los cuales ofrece la oportunidad a ni&ntilde;os y j&oacute;venes de incrementar sus rutinas de AF habitual.<sup>5,14,15</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La investigaci&oacute;n en relaci&oacute;n con la realizaci&oacute;n de AF extracurricular ha crecido exponencialmente, aumentando en los &uacute;ltimos a&ntilde;os de manera considerable. Existe una creciente preocupaci&oacute;n en la literatura por el hallazgo de programas de intervenci&oacute;n que alcancen efectos significativos en aspectos relacionados con la salud y la CF; entre ellos se encuentran m&uacute;ltiples estudios al respecto, con metodolog&iacute;as muy diferentes entre s&iacute;,<sup>7,8,11,16,17</sup> y que se presentan como alternativas de AF extraescolar.<sup>3,6,8,18</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Esta heterogeneidad en la base de la literatura cient&iacute;fica, adem&aacute;s de la importante descompensaci&oacute;n en el n&uacute;mero de estudios seg&uacute;n el tipo de intervenci&oacute;n planteada, provoca importantes discrepancias entre estudios de semejante naturaleza. En este sentido, resulta dif&iacute;cil discernir aspectos como las caracter&iacute;sticas comunes de las intervenciones de mayores logros o los par&aacute;metros de salud y de condici&oacute;n f&iacute;sica m&aacute;s adecuados para valorar a ni&ntilde;os y j&oacute;venes.</font></p>  	    <p align="justify"><font face="verdana" size="2">El prop&oacute;sito de esta revisi&oacute;n fue realizar un an&aacute;lisis de la producci&oacute;n cient&iacute;fica que se ha realizado hasta la fecha sobre ejercicio y AF extracurricular en ni&ntilde;os occidentales de 6 a 12 a&ntilde;os. Para ello, se llev&oacute; a cabo una descripci&oacute;n pormenorizada de las caracter&iacute;sticas metodol&oacute;gicas y los par&aacute;metros de medici&oacute;n m&aacute;s recurridos en la bibliograf&iacute;a, y se realiz&oacute; un an&aacute;lisis de los objetivos de las investigaciones revisadas, as&iacute; como de sus resultados.</font></p>  	    <p>&nbsp;</p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Material y m&eacute;todos</b></font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Estrategias de b&uacute;squeda bibliogr&aacute;fica</b></font></p>  	    <p align="justify"><font face="verdana" size="2">Se revisaron art&iacute;culos originales en ingl&eacute;s y espa&ntilde;ol publicados en las bases de datos de Medline/Pub&#45;Med, Scopus, y Google Scholar, de enero de 1990 a mayo de 2013, referentes a estudios descriptivos y de intervenci&oacute;n, as&iacute; como de revisiones literarias, sistem&aacute;ticas y metaan&aacute;lisis en relaci&oacute;n con la utilizaci&oacute;n de programas de AF para lograr mejoras en cualquier par&aacute;metro observable en ni&ntilde;os de 6 a 12 a&ntilde;os.</font></p>  	    <p align="justify"><font face="verdana" size="2">Las palabras clave utilizadas para la revisi&oacute;n propuestas en la b&uacute;squeda abarcaron, por un lado, a la poblaci&oacute;n objeto de estudio: "<i>preteens, children, school&#45;age</i> y <i>childhood</i>"; y por otro, a posibles modelos de AF: "<i>aftershool physical activity, sport participation</i> y <i>health promotion model</i>". Tales palabras fueron insert&aacute;ndose en pareja en las distintas bases de datos (una de cada grupo) a trav&eacute;s del conector AND, hasta que se realizaron todas las combinaciones. La estrategia de b&uacute;squeda centr&oacute; la atenci&oacute;n en t&iacute;tulos/keywords. Se incluyeron adem&aacute;s otros art&iacute;culos referidos en estudios originales o de revisi&oacute;n.</font></p>  	    <p align="justify"><font face="verdana" size="2">La b&uacute;squeda desprendi&oacute; un total de 1 413 art&iacute;culos, de los cuales 104 fueron finalmente seleccionados. El trabajo sigui&oacute; un protocolo de revisi&oacute;n de abstract para la inclusi&oacute;n/exclusi&oacute;n de estudios, realizada por un investigador salvo controversia en la inclusi&oacute;n de trabajos. En tal caso se realiz&oacute; una reuni&oacute;n con otros dos investigadores para llegar a un consenso.</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Criterios de inclusi&oacute;n y exclusi&oacute;n</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La selecci&oacute;n de los estudios cient&iacute;ficos estuvo basada en los siguientes criterios de inclusi&oacute;n: a) estudios de revisi&oacute;n, descriptivos o de intervenci&oacute;n publicados en revistas con factor de impacto, indexadas en bases de datos de prestigio internacional; b) art&iacute;culos enfocados al an&aacute;lisis de programas de AF para ni&ntilde;os de entre 6 y 12 a&ntilde;os; y c) poblaciones occidentales. La duraci&oacute;n de la intervenci&oacute;n no fue utilizada como criterio. Por otro lado, como criterios de exclusi&oacute;n se establecieron: a) estudios cient&iacute;ficos publicados en forma de resumen y/o comunicaciones cortas; b) redactados en idioma distinto al ingl&eacute;s o espa&ntilde;ol; c) estudios anteriores a 1990; d) uso de muestras con caracter&iacute;sticas f&iacute;sicas y antropom&eacute;tricas desiguales o sin randomizar; e) estudios dirigidos a poblaciones con problemas de salud severos, excepto problemas de sobrepeso u obesidad; f) mala o nula explicaci&oacute;n de los programas de intervenci&oacute;n analizados.</font></p>  	    <p>&nbsp;</p>  	    <p align="justify"><font face="verdana" size="2"><b>Resultados</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La estrategia de b&uacute;squeda permiti&oacute; obtener un total de 104 publicaciones: 29 trabajos descriptivos, 50 art&iacute;culos de intervenci&oacute;n y 25 revisiones de la literatura cient&iacute;fica sobre la tem&aacute;tica. Del total de publicaciones revisadas, el 75.96% son art&iacute;culos de intervenci&oacute;n y descriptivos. En el <a href="/img/revistas/spm/v57n6/a15c1.jpg" target="_blank">cuadro I</a> se ofrece una clasificaci&oacute;n de estos estudios en funci&oacute;n de los par&aacute;metros de medici&oacute;n utilizados para la extracci&oacute;n de resultados.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Los par&aacute;metros de medici&oacute;n m&aacute;s recurridos en los art&iacute;culos descriptivos son los de composici&oacute;n corporal; 55.17% incluye este par&aacute;metro entre sus resultados, seguido de los par&aacute;metros que miden de una u otra manera los niveles de AF realizados por la poblaci&oacute;n (41.38% del total de art&iacute;culos). En lo referente a intervenciones, esta revisi&oacute;n hall&oacute;, de igual manera, un n&uacute;mero muy alto de estudios que incluyen la composici&oacute;n corporal como par&aacute;metro b&aacute;sico dentro de sus resultados; as&iacute;, 68% de art&iacute;culos lo utilizan, seguido de la valoraci&oacute;n de la capacidad aer&oacute;bica m&aacute;xima (48% del total de art&iacute;culos), con lo que se encuentra una importante tendencia al uso del test de 20 metros de Leger (<a href="/img/revistas/spm/v57n6/a15c2.jpg" target="_blank">cuadro II</a>).</font></p>  	    <p align="justify"><font face="verdana" size="2">Se aprecia variabilidad en la duraci&oacute;n de las sesiones requeridas, aunque destacan art&iacute;culos que necesitan sesiones de entre 30&#45;44 min/d&iacute;a (24% del total), seguidos de aqu&eacute;llos con sesiones superiores a una hora. Se observa un mayor consenso en los patrones de frecuencia semanal de la sesi&oacute;n, al ser de entre 2&#45;3 sesiones semanales la elecci&oacute;n de la gran mayor&iacute;a de los programas llevados a cabo. En cuanto a la duraci&oacute;n de las intervenciones, existe una tendencia hacia la elecci&oacute;n no m&aacute;s de tres meses de experimentaci&oacute;n.</font></p>  	    <p align="justify"><font face="verdana" size="2">Aunque existe un n&uacute;mero elevado de intervenciones que combinan la variable programa de AF con otro tipo de variable independiente, como clases de educaci&oacute;n para la salud o control de ingesta cal&oacute;rica, la gran mayor&iacute;a de los estudios opta por intervenir utilizando exclusivamente la AF como elemento para conseguir un efecto en la muestra. En este sentido, se observa una clara relaci&oacute;n entre el perfil del investigador y la elecci&oacute;n de variables, de manera que 100% de los autores que utilizan la combinaci&oacute;n de dichas variables se encuentran directamente relacionados con el campo de la medicina.<sup>3,9,12</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En cuanto a clases de programas aplicados durante las intervenciones, las metodolog&iacute;as con diferencia m&aacute;s utilizadas en las distintas investigaciones son, por un lado, aqu&eacute;llas basadas en juegos y actividades cardiovasculares, las cuales suponen 44% del total, y por otro, las actividades intermitentes de alta intensidad utilizadas en 24% del total de los estudios.</font></p>  	    <p align="justify"><font face="verdana" size="2">De los 29 art&iacute;culos descriptivos, 24.14% focaliza su atenci&oacute;n en aspectos cualitativos relacionados con la AF extraescolar, tales como conocer las motivaciones de escolares en relaci&oacute;n con la AF,<sup>19</sup> relaciones entre AF y autoconcepto<sup>20</sup> o descripci&oacute;n de h&aacute;bitos saludables.<sup>21</sup> El resto de los art&iacute;culos (75.86%) coincide en observar el efecto de la AF en la CF o en la salud, ofreciendo resultados positivos.<sup>14,22,23</sup> Destaca el trabajo de Hastie, Sinelnikov y Wadsworth,<sup>22</sup> el cual profundiza especialmente al respecto al mostrar en su investigaci&oacute;n efectos diferenciados entre entrenamiento espec&iacute;fico y AF recreativa. Los resultados de este estudio arrojan diferencias significativas entre sus grupos tras la realizaci&oacute;n del <i>PACER test</i> F<sub>1, 414</sub>= 38.05, &#951;<sup>2</sup>=0.085 <i>p</i>&lt;0.001, con medias favorables para el ejercicio f&iacute;sico espec&iacute;fico (entrenamiento espec&iacute;fico: x=70.39 vueltas; dt=29.12; AF l&uacute;dico= x=51.19 vueltas; dt=29.34).</font></p>  	    <p align="justify"><font face="verdana" size="2">Los resultados relativos a los art&iacute;culos de intervenci&oacute;n coindicen con los anteriores en la relaci&oacute;n positiva entre AF y CF/salud (<a href="/img/revistas/spm/v57n6/a15c3.jpg" target="_blank">cuadros III</a> y <a href="/img/revistas/spm/v57n6/a15c4.jpg" target="_blank">IV</a>).<sup><a href="#nota">*</a></sup>,<sup>24</sup> Los estudios que analizan otras variables, como programas de educaci&oacute;n saludable o control de ingesta cal&oacute;rica, tambi&eacute;n logran efectos positivos (98% del total) a excepci&oacute;n de la investigaci&oacute;n dirigida por Robbins<sup>55</sup> y colaboradores, cuyo programa de educaci&oacute;n saludable no mostr&oacute; resultados significativos a pesar de que el grupo intervenci&oacute;n experiment&oacute; mejoras.</font></p>  	    <p align="justify"><font face="verdana" size="2">En relaci&oacute;n con revisiones de la literatura cient&iacute;fica, se extrajeron 25 trabajos que supon&iacute;an 24.14% del total de art&iacute;culos, con lo que se encontr&oacute; un elevado n&uacute;mero de revisiones cuyos an&aacute;lisis est&aacute;n dirigidos a intervenciones de AF con criterios metodol&oacute;gicos comunes. En lo que a metaan&aacute;lisis se refiere, casi la totalidad de estos estudios centra su atenci&oacute;n en el &iacute;ndice de masa corporal; en este sentido se encuentra una alta tendencia al uso de este par&aacute;metro como medida de comparaci&oacute;n. Todos los estudios miden la eficacia de la AF sobre el IMC, y se encontraron resultados positivos en la gran mayor&iacute;a.<sup>56&#45;58</sup> S&oacute;lo los an&aacute;lisis realizados por Harris, Kuramoto, Schulzer y Retallack,<sup>59</sup> y Metcalf, Henley y Wilkin,<sup>60</sup> hallaron diferencias no significativas tras las intervenciones de AF.</font></p>  	    <p align="justify"><font face="verdana" size="2">Escalante, Saavedra, Garc&iacute;a&#45;Hermoso y Dom&iacute;nguez<sup>61</sup> tambi&eacute;n optan por la observaci&oacute;n de la composici&oacute;n corporal, pero en sus an&aacute;lisis utilizaron como referencia el perfil lip&iacute;dico en poblaci&oacute;n obesa. Las conclusiones del estudio mostraron que aunque los programas basados en el ejercicio aer&oacute;bico (60 minutos, 3 veces / semana, &le; 75% de <i>FCm&aacute;x</i>) optimizaron las concentraciones de LDL&#45;C y TG, los programas de ejercicio combinado (&#8805; 60 min, &gt; 75% de <i>FCm&aacute;x</i>) mejoraron adem&aacute;s la concentraci&oacute;n de HDL&#45;C.</font></p>  	    <p align="justify"><font face="verdana" size="2">Tan solo la revisi&oacute;n de Baquet, Van Praagh y Berthoin<sup>62</sup> plantea un metaan&aacute;lisis sin tener en cuenta los estados de composici&oacute;n corporal de las muestras. El objetivo de dicho trabajo fue analizar el impacto que las intervenciones de AF ten&iacute;an sobre el <i>VO<sub>2</sub>m&aacute;x</i> de ni&ntilde;os. Seg&uacute;n sus resultados, el <i>VO<sub>2</sub>m&aacute;x</i> s&oacute;lo alcanz&oacute; una mejora de 5 a 6 % en los dise&ntilde;os de intervenci&oacute;n con entrenamientos aer&oacute;bicos.</font></p>  	    <p>&nbsp;</p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Discusi&oacute;n</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La presente revisi&oacute;n se realiz&oacute; con el fin de analizar el panorama general de la producci&oacute;n cient&iacute;fica en torno a la AF extraescolar y conocer c&oacute;mo la literatura ha abordado su estudio hasta la fecha, para as&iacute; mostrar las necesidades de investigaci&oacute;n actuales en relaci&oacute;n con el &aacute;rea de estudio en cuesti&oacute;n. Los par&aacute;metros de composici&oacute;n corporal son los m&aacute;s utilizados para valorar resultados. Las intervenciones se presentan con unas caracter&iacute;sticas metodol&oacute;gicas bastante hetereog&eacute;neas. La mayor parte de los trabajos consultados, tanto de intervenci&oacute;n como observacionales, coinciden en el objetivo de evaluar el efecto de la AF en la condici&oacute;n f&iacute;sica o la salud, ofreciendo resultados positivos. En cuanto a las revisiones, la mayor&iacute;a son sistem&aacute;ticas e incluyen metaan&aacute;lisis, donde el IMC es el par&aacute;metro m&aacute;s utilizado con diferencia.</font></p>  	    <p align="justify"><font face="verdana" size="2">La composici&oacute;n corporal es el par&aacute;metro de medici&oacute;n m&aacute;s utilizado en los estudios de intervenci&oacute;n observados, en los que destaca el uso del IMC. El elevado n&uacute;mero de investigaciones que utilizan este &iacute;ndice explica la alta tendencia que existe en su elecci&oacute;n como medida de comparaci&oacute;n en los meta&#45;an&aacute;lisis revisados.<sup>56&#45;60</sup> Cabe destacar que aunque la medici&oacute;n del IMC en intervenciones escolares resulta acertada tanto por su idoneidad para la comparaci&oacute;n de resultados como por su facilidad de uso en estas edades, su an&aacute;lisis de manera aislada no parece aconsejable ya que, seg&uacute;n autores como Kweitel,<sup>63</sup> es una herramienta que ofrece poca fiabilidad para determinar la composici&oacute;n corporal, pues asume de manera directa que todo el peso que exceda de los valores determinados por las tablas talla&#45;peso corresponder&aacute;n a masa grasa.</font></p>  	    <p align="justify"><font face="verdana" size="2">Adem&aacute;s de la utilizaci&oacute;n de la composici&oacute;n corporal, los resultados muestran un uso reiterado de par&aacute;metros para estimar la capacidad aer&oacute;bica m&aacute;xima. El uso del <i>VO<sub>2</sub>M&aacute;x</i> resulta muy com&uacute;n entre los estudios, a pesar de que su utilizaci&oacute;n resulta igualmente controvertida en estas edades. En este sentido, se encuentra ampliamente aceptado que la capacidad aer&oacute;bica aumenta con el crecimiento de forma similar a los incrementos en la funci&oacute;n pulmonar y cardiovascular, afirmaci&oacute;n demostrada considerablemente en estudios como los mencionados con anterioridad, de Baquet y colaboradores,<sup>30</sup> en los que, tras analizar adaptaciones inducidas por el entrenamiento del trabajo aer&oacute;bico en ni&ntilde;os, se concluye que la mejora media alcanzada con respecto al <i>VO</i><sub>2</sub><i>m&aacute;x</i> es de 8 a 10% en los estudios que pretend&iacute;an efectos significativos del entrenamiento y s&oacute;lo de 5% en los dem&aacute;s. Este estudio abre la puerta a la necesidad de comparar el entrenamiento de interv&aacute;lico de alta intensidad con el trabajo aer&oacute;bico en ni&ntilde;os y preadolescentes.</font></p>  	    <p align="justify"><font face="verdana" size="2">En cuanto a las diferencias metodol&oacute;gicas halladas en los art&iacute;culos de intervenci&oacute;n, el an&aacute;lisis de resultados mostr&oacute; una serie de tendencias en las caracter&iacute;sticas del m&eacute;todo de investigaci&oacute;n utilizado en los estudios. De &eacute;stos, 46% utiliza una muestra inferior a 100 participantes, donde 24% est&aacute; por debajo de los 40 sujetos; esto provoca que las investigaciones se limiten al &aacute;mbito de aplicaci&oacute;n. La duraci&oacute;n de la sesi&oacute;n tiende a no ser de m&aacute;s de una hora (32% de los casos, la duraci&oacute;n estaba por debajo de los 45 minutos) y la frecuencia semanal es de alrededor de tres d&iacute;as. En cuanto a los programas de intervenci&oacute;n, destaca el contraste entre las dos metodolog&iacute;as m&aacute;s utilizadas (l&uacute;dica y de intensidad elevada). En coincidencia con los estudios de Baquet y colaboradores,<sup>30</sup> se considera necesario plantear estudios comparativos entre estas dos l&iacute;neas de trabajo, ya que resultar&iacute;a muy interesante conocer los efectos de ambos m&eacute;todos en distintos par&aacute;metros de salud y CF.</font></p>  	    <p align="justify"><font face="verdana" size="2">Por &uacute;ltimo, destacan las carencias en los art&iacute;culos revisados referentes a las explicaciones de los protocolos de entrenamiento, el gran n&uacute;mero de estudios que pasan por alto el estadio madurativo de los sujetos sometidos a evaluaci&oacute;n, la falta de claridad en la exposici&oacute;n de las caracter&iacute;sticas de variables independientes alternativas de la intervenci&oacute;n (concreci&oacute;n de controles de ingesta cal&oacute;rica propuestos, aclaraci&oacute;n de los contenidos llevados a cabo en las clases de educaci&oacute;n para la salud puestas en marcha, etc.) o el establecimiento exacto del n&uacute;mero de sesiones llevadas a cabo a lo largo de la intervenci&oacute;n, ya que la gran mayor&iacute;a de los art&iacute;culos revisados s&oacute;lo marcan la duraci&oacute;n de la sesi&oacute;n y el tiempo destinado a la intervenci&oacute;n sin tener en cuenta aspectos que pueden influir en la puesta en marcha del programa como periodos vacacionales o suspensi&oacute;n de sesiones por circunstancias concretas.</font></p>  	    <p align="justify"><font face="verdana" size="2">Esta revisi&oacute;n presenta una visi&oacute;n global de gran relevancia al abordar la investigaci&oacute;n en torno a la AF escolar en la actualidad, que deja la puerta abierta a la creaci&oacute;n de nuevos estudios al respecto que logren mejorar un tipo de AF cada vez m&aacute;s practicada y demandada. En este sentido, ser&iacute;a beneficioso que las futuras investigaciones tuvieran en cuenta algunos de los par&aacute;metros de medici&oacute;n m&aacute;s utilizados, as&iacute; como los m&eacute;todos de investigaci&oacute;n m&aacute;s repetidos con el fin de facilitar la comparaci&oacute;n entre los resultados de diferentes investigaciones. Es necesaria la puesta en marcha de intervenciones con muestras m&aacute;s elevadas para poder generalizar los resultados a la poblaci&oacute;n juvenil e infantil.
