Las alteraciones macrohemodinámicas pueden discordar con los trastornos de la microcirculación; se sugiere la medición de la diferencia arteriovenosa de dióxido de carbono (delta-CO2) como marcador que refleja disfunción mitocondrial por deuda de oxígeno persistente. Esta determinación es útil en pacientes adultos; esto aún no se comprueba en el paciente pediátrico.  Material y métodos:  estudio observacional efectuado en la Unidad de Terapia Intensiva Pediátrica del Centro Médico Nacional 20 de Noviembre. Incluyó pacientes con choque séptico, con toma de gasometría venosa y arterial al ingreso, a las seis, 12 y 24 horas y evaluación simultánea de escala PRISM III (Pediatric Risk of Mortality), para valorar delta-CO2 &gt; 6 mmHg como predictor de mortalidad en pacientes con choque séptico, evaluación de respuesta terapéutica.  Resultados:  se encontró correlación del riesgo de mortalidad directamente proporcional con los niveles elevados de delta-CO2 en primeras 24 horas de estancia en unidad de terapia intensiva pediátrica; presenta correlación estadística positiva (coeficiente de correlación de Pearson de 0.99 y riesgo relativo &gt; 1) con aumento de niveles de delta-CO2 en pacientes con mejoría de variables macrohemodinámicas.  Conclusiones:  delta-CO2, puede ser considerado factor pronóstico de evolución y mortalidad en pacientes pediátricos con choque séptico. Se requiere de estudios con muestras ampliadas para corroborar utilidad e implementación de medición de delta-CO2 en los pacientes pediátricos. La normalización de las alteraciones macrocirculatorias no define la mejoría a nivel microcirculatorio, evolución y pronóstico.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  monitoring of patients with septic shock seeks to evaluate parameters that reflect the microciculatory involvementand allow adjustements therapeutics. Macrohemodynamicalterations may be inconsistent with the microcirculation disorders; measurement of the arteriovenous carbon dioxide diference is suggested (delta CO2) as marker that reflects dysfunction mitochondria due to persisten oxygen debt. This determination is useful in adult patients ; this still its not verified in the patient pediatric.  Material and methods:  observational study carried out in the Therapy Unit Pediatric Intensive Care of de National Medical Center 20 de Noviembre. Included patients with shock septic, with take of gasometry venous and arterial to the in come,6,12 and 24 hours and assessment simultaneous os scale Pediatric Risk of Mortality (PRISM III) to assess delta-CO2 &gt;66 mm Hg as a predictor of mortality in patients with septic shock and response evaluation therapy.  Results:  correlation of mortality risk was foun directly proportional to elevated delta-CO2 levels in the first 24 hours of stay in pediatric intensive care unit; presents statistical correlation positive (coefficient of correlation of Pearson 0.99 and ris relative &gt; 1) in incresead delta CO2 levels with improvement in macrohemodynamic variables.  Conclusions:  delta-CO2 can be considered a prognostic factor for evolution and mortality in pediatric patients with septic shock. He require of studies with samples expanded for corroborate utility and implementation of measurement of delta-CO2 in pediatric patients. The normalization of the alteration macrocirculatory not define the improvement to level microcirculatory, evolution and forecast.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo:  Introdução:  o acompanhamento do paciente com choque séptico busca avaliar parâmetros que reflitam o envolvimento microcirculatório e permitam ajustes terapêuticos. As alterações macrohemodinâmicas podem ser discordantes com distúrbios da microcirculação; A medição da diferença arteriovenosa de dióxido de carbono (delta-CO2) é sugerida como um marcador que reflete a disfunção mitocondrial devido ao débito persistente de oxigênio. Esta determinação é útil em pacientes adultos; Isso ainda não foi comprovado em pacientes pediátricos.  Material e métodos:  estudo observacional, na Unidade de Terapia Intensiva Pediátrica do Centro Médico Nacional 20 de Noviembre, incluiu pacientes em choque séptico, com gasometria venosa e arterial colhidas na admissão, às 6, 12 e 24 horas e avaliação simultânea da escala PRISM III, para avaliar delta-CO2 &gt; 6 mmHg como preditor de mortalidade em pacientes com choque séptico, avaliação da resposta terapêutica.  Resultados:  foi encontrada correlação de risco de mortalidade diretamente proporcional aos níveis elevados de delta-CO2 nas primeiras 24 horas de permanência na Unidade de Terapia Intensiva Pediátrica. Apresenta correlação estatística positiva (coeficiente de correlação de Pearson de 0.99 e. risco relativo &gt; 1), na redução dos níveis de delta-CO2 em pacientes com melhora das variáveis macrohemodinâmicas.  Conclusões:  delta-CO2 pode ser considerado fator prognóstico de evolução e mortalidade em pacientes pediátricos com choque séptico. Estudos com amostras ampliadas são necessários para corroborar a utilidade e implementação da medição do delta-CO2 em pacientes pediátricos. A normalização das alterações macrocirculatórias não define a melhoria a nível microcirculatório, evolução e prognóstico.]]></p></abstract>
