Objetivo: Profundizar en la relación existente entre gonartrosis primaria y deformidad angular de la articulación de la rodilla. Método: Se realizó una revisión bibliográfica de un total de 300 artículos publicados en PubMed, Hinari y Medline mediante el localizador de información EndNote, de ellos se utilizaron 52 citas seleccionadas para realizar la revisión, 47 de ellas de los últimos cinco años, donde se incluyeron cinco libros. Desarrollo: Se realizó una revisión de los aspectos bioquímicos y biomecánicos más importantes relacionados con las deformidades angulares. Con respecto a la asociación entre deformidad angular y gonartrosis primaria, se plasmaron los elementos relacionados con cada tipo de deformidad tanto en el eje axial, coronal como rotacional. Los factores relacionados con la deformidad pueden ser de tipo óseo y de partes blandas, en especial del aparato capsuloligamentoso de la articulación. Conclusiones: Las deformidades angulares en pacientes con gonartrosis primaria son frecuentes y en su presencia influyen factores relacionados con la arquitectura ósea y de partes blandas.]]></p></abstract>

El s&iacute;ntoma fundamental de esta entidad es el dolor de tipo mec&aacute;nico acompa&ntilde;ado de crepitaci&oacute;n, sensaci&oacute;n de inestabilidad y limitaci&oacute;n articular.<sup>4,5,6</sup></font></p>     <p align="justify"><font face="verdana" size="2">El m&eacute;todo cl&iacute;nico constituye la herramienta fundamental para el diagn&oacute;stico de esta enfermedad, que afecta a un gran n&uacute;mero de personas en el mundo.</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Las causas de la artrosis de la rodilla, tambi&eacute;n conocida como gonartrosis, pueden ser de tipo primarias o idiop&aacute;ticas y de tipo secundarias.<sup>7</sup> Dentro de las causas secundarias, destacan las postquir&uacute;rgicas, postinfecciosas y postraum&aacute;ticas, donde desempe&ntilde;an un papel fundamental las deformidades angulares residuales, que provocan distribuci&oacute;n anormal de las cargas de peso.<sup>8,9,10</sup></font></p>     <p align="justify"><font face="verdana" size="2">Las deformidades angulares de la rodilla aceleran los procesos degenerativos de la articulaci&oacute;n y constituyen factores de mal pron&oacute;stico demostrados en la literatura cient&iacute;fica.<sup>11,12,13</sup></font></p>     <p align="justify"><font face="verdana" size="2">A pesar de que las deformidades angulares de causa secundaria son bien conocidas, no existe documentaci&oacute;n suficiente que explique y describa estas desviaciones en la gonartrosis primaria.</font></p>     <p align="justify"><font face="verdana" size="2">Debido a esta problem&aacute;tica sobre la relaci&oacute;n entre deformidad angular y gonartrosis primaria, los autores de este trabajo tienen como objetivo profundizar en este aspecto.</font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>M&eacute;todo</b></font></p>     <p align="justify"><font face="verdana" size="2">Se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un total de 300 art&iacute;culos publicados en PubMed, HINARI y Medline mediante el localizador de informaci&oacute;n EndNote; de ellos, se utilizaron 52 citas seleccionadas para realizar la revisi&oacute;n, 47 de ellas de los &uacute;ltimos cinco a&ntilde;os, donde se incluyeron cinco libros.</font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">  <b>Desarrollo</b></font></p>     <p align="justify"><font face="verdana" size="2">La artrosis comienza por una etapa precl&iacute;nica o bioqu&iacute;mica en la que existe un desbalance entre el anabolismo y catabolismo del cart&iacute;lago articular; al predominar el catabolismo, el deterioro del tejido es progresivo hasta llegar a la etapa cl&iacute;nica de la enfermedad. Para entender la relaci&oacute;n entre deformidad y gonartrosis primaria es importante comprender los aspectos bioqu&iacute;micos y biomec&aacute;nicos esenciales.<sup>11,12</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>Aspectos bioqu&iacute;micos esenciales</b></font></p>     <p align="justify"><font face="verdana" size="2">Debido al proceso degenerativo de la artrosis, se afecta la integridad y funci&oacute;n del cart&iacute;lago articular, lo que provoca una redistribuci&oacute;n de las cargas mec&aacute;nicas.<sup>11,13</sup></font></p>     <p align="justify"><font face="verdana" size="2">Las fuerzas mec&aacute;nicas causan, a la vez, cambios en la forma y estructura de los tejidos, entre los que se incluye el cart&iacute;lago articular. Es bien conocido que cuando existen cambios en las cargas de peso de la articulaci&oacute;n, se modifican las propiedades mec&aacute;nicas del cart&iacute;lago articular, su composici&oacute;n y estructura molecular; un ejemplo muy com&uacute;n es cuando un paciente necesita de inmovilizaci&oacute;n de una de sus extremidades.<sup>11,12</sup></font></p>     <p align="justify"><font face="verdana" size="2">Debido a estos cambios mec&aacute;nicos se producen da&ntilde;os de la microestructura del cart&iacute;lago, alteraciones en el metabolismo de los condrocitos y su composici&oacute;n qu&iacute;mica.<sup>11</sup></font></p>     <p align="justify"><font face="verdana" size="2">El cart&iacute;lago articular absorbe gran cantidad de cargas mec&aacute;nicas, minimiza las fuerzas de estr&eacute;s entre las superficies articulares y ayuda a la lubricaci&oacute;n. Al aplicarse una carga externa de peso, el tejido es afectado por fuerzas de tensi&oacute;n, cizallamiento y compresi&oacute;n que afectan su composici&oacute;n e integridad.<sup>11,13</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>Aspectos biomec&aacute;nicos esenciales</b></font></p>     <p align="justify"><font face="verdana" size="2">Seg&uacute;n Sharma y colaboradores,<sup>14</sup> la cinem&aacute;tica de la rodilla no es simple, ya que est&aacute; representada por seis tipos de movimientos; tres de ellos son dominantes, como flexi&oacute;n, extensi&oacute;n, rotaci&oacute;n interna y externa; y traslaci&oacute;n anterior y posterior. El resto de los tres movimientos, de menor magnitud, son abducci&oacute;n y aducci&oacute;n, traslaci&oacute;n medial y lateral; y superior e inferior. A la extensi&oacute;n completa de la rodilla y a menos de 10 grados de flexi&oacute;n, el f&eacute;mur rota internamente en relaci&oacute;n con la tibia. Con el movimiento de flexi&oacute;n y la carga de peso, el f&eacute;mur se desplaza en direcci&oacute;n posterior en relaci&oacute;n con la tibia y rota externamente. Esta rotaci&oacute;n externa del f&eacute;mur en la flexi&oacute;n provoca que el c&oacute;ndilo lateral se desplace m&aacute;s posterior que el medial.</font></p>     <p align="justify"><font face="verdana" size="2">En relaci&oacute;n con la carga de peso, el compartimento tibiofemoral medial absorbe de 60 al 70% del peso corporal.<sup>15,16</sup></font></p>     <p align="justify"><font face="verdana" size="2">La alineaci&oacute;n, seg&uacute;n Cooke TDV y su grupo,<sup>17</sup> se describe como la orientaci&oacute;n del muslo (f&eacute;mur) y la pierna (tibia y peron&eacute;) en sentido frontal, lateral y rotacional.</font></p>     <p align="justify"><font face="verdana" size="2">Al observar al paciente en sentido frontal, es importante trazar la l&iacute;nea desde el centro de la cabeza femoral, rodilla y tobillo, lo que es conocido como "l&iacute;nea cadera, rodilla y tobillo" y en la literatura anglosajona como HKA (<i>Hip Knee Ankle</i>). Se considera desviaci&oacute;n o mala alineaci&oacute;n cuando el centro de la rodilla se desplaza hacia adentro (valgo) o hacia afuera (varo), lo que aumenta la carga de peso en el compartimento lateral o medial, respectivamente.<sup>18,19,20</sup></font></p>     <p align="justify"><font face="verdana" size="2">En la vista lateral, se emplea la misma l&iacute;nea; si el centro de la rodilla se desplaza hacia atr&aacute;s, existe deformidad en hiperextensi&oacute;n y si es hacia delante, es en flexi&oacute;n.<sup>21,22</sup></font></p>     <p align="justify"><font face="verdana" size="2">Las deformidades angulares secundarias pueden ser debidas a causas bien conocidas, como fracturas previas (intra- o extraarticulares), postquir&uacute;rgicas: postmeniscectom&iacute;a, artritis infecciosa, deformidades angulares desde la ni&ntilde;ez, por ejemplo: tibia vara. Estas causas est&aacute;n presentes antes del desarrollo de la gonartrosis.<sup>23,24,25</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">  <b>&iquest;Por qu&eacute; forma parte la deformidad angular de la gonartrosis primaria?</b></font></p>     <p align="justify"><font face="verdana" size="2">Aunque cuando existe gonartrosis se afecta toda la articulaci&oacute;n, el cart&iacute;lago articular es evidentemente el m&aacute;s afectado.<sup>26,27</sup> Por lo general, la enfermedad comienza por el compartimento medial, pero puede ser cualquier otro. Una vez que se afecta este tejido, ocurre un desbalance articular, el cual comienza a incrementarse con el paso de los a&ntilde;os y otros factores, como actividad f&iacute;sica y obesidad. Se afecta el aparato capsuloligamentoso de la articulaci&oacute;n, lo que permite as&iacute; su desviaci&oacute;n angular; es t&iacute;pico encontrar en pacientes con gonartrosis inestabilidad de tipo subjetiva referida por el enfermo, pero adem&aacute;s, existe elongaci&oacute;n del ligamento cruzado anterior y colaterales de la rodilla.<sup>28,29,30</sup></font></p>     <p align="justify"><font face="verdana" size="2">Las desviaciones angulares de la rodilla en pacientes con gonartrosis primaria pueden ser en relaci&oacute;n con el eje axial, coronal y rotacional (<a href="#a7f1" target="_self">Figura 1</a>).<sup>26,31</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p><a name="a7f1"></a></p>    <p>&nbsp;</p>    <p align="center"><img src="../img/revistas/aom/v29n4/a7f1.jpg"></p>    <p>&nbsp;</p>    <p><font size="2" face="Verdana"></font></p>     <p align="justify"><font face="verdana" size="2">Una vez que aparece la deformidad angular en la enfermedad, ya es irreversible y su magnitud depender&aacute; de factores tanto internos (edad, sexo, da&ntilde;o o magnitud del cart&iacute;lago articular) como externos (actividades que realiza el enfermo, microtraumas, sobrepeso y carga de objetos pesados).<sup>32,33,34</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">La deformidad en varo es debido a colapso del <i>stock</i> &oacute;seo medial, elongaci&oacute;n de los ligamentos colaterales laterales asociados con crecimiento del c&oacute;ndilo lateral y contractura de los ligamentos colaterales mediales.<sup>35,36,37</sup> Estos factores de deformidad producen los mismos s&iacute;ntomas y signos cl&iacute;nicos al desplazar la rodilla desde la posici&oacute;n neutral al varo. Por otra parte, durante la marcha en un paciente con rodilla normal, sin p&eacute;rdida de la estructura &oacute;sea del c&oacute;ndilo medial, las cargas de peso se distribuyen de forma m&aacute;s sim&eacute;trica, pero al existir p&eacute;rdida &oacute;sea en esta zona en el c&oacute;ndilo femoral medial y elongaci&oacute;n del c&oacute;ndilo femoral lateral, las propias cargas de peso desv&iacute;an la rodilla en varo.<sup>38,39,40</sup> La elongaci&oacute;n del ligamento colateral lateral es otro factor que contribuye a la deformidad en varo, pero su mecanismo resulta dif&iacute;cil de explicar; una posible respuesta a esta hip&oacute;tesis puede ser que las l&iacute;neas de carga de peso son m&aacute;s mediales, la p&eacute;rdida de sustancia &oacute;sea de c&oacute;ndilo femoral medial obliga al varo y las estructuras capsuloligamentosas de la rodilla se estresan y tienden a elongarse del lado lateral. Una vez presente la p&eacute;rdida &oacute;sea del lado medial, la espina tibial se desplaza hacia arriba y su &aacute;pex hacia la regi&oacute;n lateral de la articulaci&oacute;n, lo que causa dolor intenso y la formaci&oacute;n de una l&iacute;nea articular falsa.<sup>41,42,43</sup></font></p>     <p align="justify"><font face="verdana" size="2">Por su parte, la deformidad en valgo es causada por la elongaci&oacute;n o ruptura de los ligamentos colaterales mediales e hipercrecimiento del c&oacute;ndilo medial y colapso del <i>stock</i> &oacute;seo lateral asociado con contractura de los ligamentos colaterales laterales.<sup>44,45,46</sup> Los elementos planteados en la deformidad en varo son tambi&eacute;n v&aacute;lidos para esta deformidad.<sup>47,48</sup></font></p>     <p align="justify"><font face="verdana" size="2">En relaci&oacute;n con las deformidades en flexi&oacute;n, son provocadas porque la rodilla tiene mayor confort en la posici&oacute;n de 15 grados; cuando existe inflamaci&oacute;n y dolor, el paciente no puede extender la rodilla; esto, a la vez, resulta en contractura de los m&uacute;sculos de la corva, c&aacute;psula posterior, adherencias entre los ligamentos colaterales y los c&oacute;ndilos femorales.<sup>26,49,50</sup></font></p>     <p align="justify"><font face="verdana" size="2">La subluxaci&oacute;n lateral de la r&oacute;tula es observada con frecuencia en pacientes con deformidad en valgo y es causada por la acci&oacute;n del cu&aacute;driceps, que desplaza la r&oacute;tula lateralmente.<sup>4,51</sup></font></p>     <p align="justify"><font face="verdana" size="2">En caso de la rotaci&oacute;n externa de la tibia, esta deformidad responde a la acci&oacute;n del mecanismo extensor lateral, tensor de la fascia lata y gl&uacute;teo m&aacute;ximo a trav&eacute;s de la cintilla iliotibial, m&uacute;sculo b&iacute;ceps y p&eacute;rdida de la fuerza de los m&uacute;sculos rotadores internos.<sup>26,52</sup></font></p>     <p align="justify"><font face="verdana" size="2">La hiperextensi&oacute;n se observa con deformidad fija en valgo; existe contractura de los m&uacute;sculos anteriores y ruptura del ligamento cruzado anterior.<sup>26,39</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>Conclusiones</b></font></p>     <p align="justify"><font face="verdana" size="2">Las deformidades angulares en la gonartrosis primaria est&aacute;n presentes en un alto porcentaje de los pacientes. Se realiz&oacute; un breve recuento de los elementos bioqu&iacute;micos y biomec&aacute;nicos m&aacute;s esenciales de la articulaci&oacute;n de la rodilla, lo que permiti&oacute; explicar las causas de las diferentes deformidades en cuanto a los factores que favorecen su aparici&oacute;n y tipo de angulaci&oacute;n, que pueden ser en varo, valgo, flexi&oacute;n, subluxaci&oacute;n lateral e hiperextensi&oacute;n.</font></p>     ]]></body>
