MATERIALES Y MÉTODOS:  Serie de casos de pacientes atendidas entre abril del 2023 y diciembre del 2024 con alguna de las siguientes alteraciones: escroto no palmeado, prepucio menor de 6 cm o antecedentes de orquiectomía que impidieron el uso del escroto como aloinjerto y, en su lugar, se optó por piel de tilapia como injerto.  RESULTADOS:  Las principales complicaciones intraoperatorias durante la vaginoplastia con inversión de prepucio fueron: falta de piel en el pene, lo que hizo inviable recurrir a la piel escrotal como injerto. Como alternativa se utilizó piel de tilapia, un material biocompatible y solución para estos casos.  CONCLUSIONES:  La vaginoplastia con xenoinjerto de piel de tilapia es una alternativa quirúrgica viable para la creación de una neovagina con anatomía funcional, con tejido similar a la mucosa vaginal, de bajo costo y menor morbilidad.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  OBJETIVE:  To present a surgical approach to gender reassignment (vaginoplasty) involving the use of tilapia skin grafts.  MATERIALS ANS METHODS: A series of cases of patients treated between April 2023 and December 2024 with one of the following conditions: a non-webbed scrotum; a foreskin measuring less than 6 cm; or a history of orchiectomy that prevented the use of the scrotum as an allograft. Tilapia skin was instead chosen as the graft.  RESULTS:  The main intraoperative complication during vaginoplasty with foreskin inversion was lack of skin on the penis, making it unfeasible to use scrotal skin as a graft. Tilapia skin was used as an alternative, as it is a biocompatible material and an effective solution for these cases.  CONCLUSIONS:  Vaginoplasty using a tilapia skin xenograft is a viable surgical alternative for creating a functional neovagina with tissue resembling vaginal mucosa at a low cost with reduced morbidity.]]></p></abstract>
