OBJETIVO: revisar los criterios diagnósticos internacionales y hacer una propuesta que pueda aplicarse en todos los niveles de atención.  MATERIALES Y MÉTODOS: estudio restrospectivo efectuado con base en la búsqueda en MedLine, OVID, Science Direct, Practice Guidelines Internacional Networks y scholar.google.com; de los años 2010 a 2016 con las palabras clave: diagnóstico de diabetes gestacional, factores de riesgo, criterios diagnósticos y hemoglobina glucosilada. Se seleccionaron siete guías de práctica clínica y 25 artículos y se plantearon cinco puntos de controversia.  RESULTADOS: en relación con el interrogante ¿deben realizarse pruebas diagnósticas a todas las mexicanas embarazadas, a partir del primer trimestre? Un estudio indicó que las mexicanas tienen mayor prevalencia de diabetes mellitus gestacional que las de raza blanca (6.1 vs 5.4%). Sin embargo, otros estudios no reprodujeron los mismos resultados. El diagnóstico temprano de diabetes gestacional se establece a partir del primer trimestre con una glucemia en ayuno &#8805;92 mg/dL. Por lo que corresponde a la pregunta ¿a quiénes deben realizárseles pruebas diagnósticas en el primer trimestre? se encontró que a las pacientes con antecedente de diabetes mellitus gestacional, macrosomía, glucosuria y obesidad: curva de tolerancia a la glucosa antes de la vigésima semana de la gestación. En cuanto al punto de corte utilizar los criterios de la IADPSG, que han demostrado disminuir la morbilidad y mortalidad perinatal. La utilidad de la hemoglobina glucosilada tiene un valor de 4.5% o menor, con 100% de especificidad y sensibilidad muy pobre. No es útil como prueba diagnóstica.  CONCLUSIONES: la glucemia en ayuno &#8805;92 mg/dL en el primer trimestre diagnostica diabetes mellitus gestacional. A todas las mujeres con factores de riesgo de diabetes debe tomarse una curva de tolerancia a la glucosa desde la primera consulta, que será con 75 gramos de glucosa y se utilizarán los criterios del estudio HAPO. La hemoglobina glucosilada no es útil para diagnosticar diabetes mellitus gestacional. Se propone un algoritmo para la aplicación en todos los niveles de atención.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  BACKGROUND: For more than 50 years, multiple international guidelines have been developed for the diagnosis of gestational diabetes mellitus GDM with different criteria and cut-off points; However, there is no consensus in Mexico.  OBJECTIVE: To review international diagnostic criteria and make a proposal applicable in Mexico.  METHODS: We searched in MedLine, OVID, Science Direct, Practice Guidelines International Networks and scholar.google.com; from 2010 to 2016 with the following key words: diagnosis of gestational diabetes, risk factors, diagnostic criteria and glycosylated hemoglobin. Seven clinical practice guidelines and 25 articles were selected.  RESULTS: Five questions were elaborated. Should diagnostic tests be performed on all Mexicans from the first trimester? One study indicated that Mexican women have a higher prevalence of GDM than white women, 6.1 vs. 5.4%. However, other studies did not replicate the same results. Early diagnosis of gestational diabetes. Fasting blood glucose &gt;92 mg/dL from the first trimester diagnoses GDM. Which women should be test in the first trimester? Patients with a history of GDM, macrosomia, glucosuria and obesity: should be tested with a glucose tolerance test before the 20th week of gestation. Which criteria and cutoffs should be used? The criteria of IADPSG have been shown lower perinatal morbidity and mortality. Utility of the glycosylated hemoglobin. A value of 4.5% or less, has a specificity of 100% but very poor sensitivity. It is not useful as a diagnostic test.  CONCLUSIONS: Fasting glycaemia &#8805;92 mg/dL in the first trimester diagnoses GDM. To performed glucose tolerance test in women with risk factors from the first consultation. This will be with 75 grams of glucose and the HAPO criteria will be used. Glycosylated hemoglobin is not useful for diagnosing GDM. An algorithm is proposed for applicability at all levels of care.]]></p></abstract>
