La nutrición enteral temprana ha demostrado mejorar la evolución clínica de los pacientes y se ha demostrado seguridad con el uso de vasopresores; sin embargo, existen diferentes modalidades de la nutrición enteral (aporte continuo y aporte intermitente), al momento no se conoce cuál de las modalidades de aporte enteral tenga mejor tolerancia en pacientes con vasopresores.  Objetivo:  establecer la diferencia en la tolerancia de la dieta enteral continua en comparación con la dieta enteral intermitente en pacientes con vasopresores.  Material y métodos:  se incluyeron a todos los pacientes que ingresaban a terapia intensiva con requerimientos de norepinefrina a dosis de 0.1-0.3 &#956;g/kg/min y dieta enteral en las primeras 24 horas, los dos grupos del estudio fueron aquellos con nutrición continua y nutrición intermitente; a ambos grupos se les dio seguimiento por cinco días, observando datos de intolerancia a la dieta enteral. El análisis estadístico se realizó con la prueba &#967;2, análisis univariado con riesgo relativo (RR) e intervalo de confianza de 95% (IC95%)y un análisis de regresión logística múltiple.  Resultados:  total de 58 pacientes analizados. No se observó diferencia en la tolerancia a la dieta enteral entre los dos grupos (24.1% vs 17.2%, p = 0.520, RR 1.4, IC95% 0.50-3.9). El principal síntoma de intolerancia fue el residuo gástrico (75%), y se reportó incremento en la mortalidad en los pacientes con intolerancia a la dieta enteral (p = 0.019, RR 3.84, IC95% 1.24-11.8). El uso de doble vasopresor fue un factor de riesgo asociado a la intolerancia de la dieta enteral (OR 8.9, IC95% 2.20-12.14).  Conclusiones:  no existe diferencia en la tolerancia de la dieta enteral intermitente en comparación con la dieta enteral continua en pacientes con choque y uso de norepinefrina.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  state, norepinephrine is the first-choice vasopressor. Early enteral nutrition has been shown to improve the clinical course of patients and has been proven safe when used with vasopressors. However, there are different modalities of enteral nutrition (continuous and intermittent feeding), and it is currently unknown which modality of enteral feeding has better tolerance in patients on vasopressors.  Objective:  to establish the difference in tolerance between continuous enteral feeding and intermittent enteral feeding in patients on vasopressors.  Material and methods:  all patients admitted to the intensive care unit with norepinephrine requirements at doses of 0.1-0.3 &#956;g/kg/min and enteral feeding within the first 24 hours were included. The two study groups consisted of those receiving continuous nutrition and intermittent nutrition. Both groups were followed for five days, with observations of enteral feeding intolerance. Statistical analysis was performed using the &#967;2 test, univariate analysis with RR and 95% CI, and multiple logistic regression analysis.  Results:  a total of 58 patients were analyzed. There was no difference in enteral feeding tolerance between the two groups (24.1% vs 17.2%, p = 0.520, RR 1.4, 95% CI 0.50-3.9). The main symptom of intolerance was gastric residue (75%), and an increase in mortality was reported in patients with enteral feeding intolerance (p = 0.019, RR 3.84, 95% CI 1.24-11.8). Foi demonstrado que a nutrição enteral precoce melhora o curso clínico dos pacientes e é comprovadamente segura quando usada com vasopressores; porém, existem diferentes modalidades de nutrição enteral (alimentação contínua e alimentação intermitente), no momento não se sabe qual das modalidades de oferta enteral apresenta melhor tolerância em pacientes com vasopressores.  Objetivo:  estabelecer a diferença na tolerância da dieta enteral contínua em comparação à dieta enteral intermitente em pacientes com vasopressores.  Material e métodos:  foram incluídos todos os pacientes internados em terapia intensiva com necessidade de norepinefrina nas doses de 0.1-0.3 &#956;g/kg/min e dieta enteral nas primeiras 24 horas, os dois grupos de estudo foram aqueles com nutrição contínua e nutrição intermitente, ambos os grupos foram acompanhados por 5 dias, com observações de intolerância à alimentação enteral. A análise estatística foi realizada com teste &#967;2, análise univariada com RR e IC ao 95% e análise de regressão logística múltipla.  Resultados:  analisaram-se um total de 58 pacientes, entre os dois grupos não observou-se diferença na tolerância à dieta enteral (24.1% vs 17.2%, p = 0.520, RR 1.4, IC 95% 0.50-3.9), o principal sintoma de intolerância foi o resíduo gástrico (75%), relatou-se aumento na mortalidade em pacientes com intolerância à dieta enteral (p = 0.019, RR 3.84 IC 95% 1.24-11.8), e o uso de vasopressor duplo foi um fator de risco associado à intolerância à dieta enteral (OR 8.9, IC 95% 2.20-12.14).
