El costo económico del dolor es brutal. Tan solo en Estados Unidos, en el 2010, se estiman costos totales entre $560 y $635 billones de dólares. Existen cuatro procesos básicos de la nocicepción: Transducción, conducción, modulación y percepción, en cada una de ellas se puede tratar el dolor. Ante un estímulo nocivo o daño tisular, se libera una serie de neurotransmisores que se conocen en conjunto como &#8220;sopa inflamatoria&#8221; que son sustancias responsables de producir dolor. Las señales nociceptivas después de llegar a la medula espinal, viajan por los tractos espinotalámicos hasta llegar a tálamo y otros núcleos del sistema límbico, donde se ven implicadas las respuestas emocionales y se pueden modular por una serie de mecanismos emocionales, para finalmente llegar a la corteza somatosensorial, donde finalmente se integra el fenómeno como dolor. El tratamiento del dolor incluye los AINES COX1 y COX2. Estos últimos han demostrado ser aproximadamente diez veces más seguros a nivel gastrointestinal que los AINE no selectivos. Los AINES, los opioides débiles y los opioides fuertes son ampliamente utilizados en dolor agudo postoperatorio. En dolor por cáncer su uso ésta bien fundamentados. Para el dolor neuropático existen medicamentos como los neuromoduladores, que son medicamentos antidepresivos y anticonvulsivantes, que han demostrado ser muy útiles en este tipo de dolor. Se estima que el 10% de los pacientes con dolor crónico se ven beneficiado por algún tipo de intervencionismo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Pain is the leading cause of medical consultation. The economic cost of the pain is &#8220;brutal&#8221;. Only in the United States, in 2010, the total cost is estimated between $560 and $635 billion. There are four basic processes of nociception: transduction, transmission, modulation and perception, in each of which the pain can be treated. Faced with a noxious stimulus or tissue damage, a number of neurotransmitters is released that is known collectively as "inflammatory soup&#8221; and it is responsible for producing pain. Nociceptive signals after reaching the spinal cord, will travel by espinotalamicos tracts to thalamus and other nuclei of the limbic system, where emotional responses will be involved and be modulated by a series of emotions, finally arriving to the somatosensory cortex, where they finally join as pain. Treatment of pain includes the NSAIDs COX1 and COX2. The latter proved to be approximately ten times safer at gastrointestinal level than the NSAID nonselective. Strong and weak opioids and NSAIDs are widely used in acute postoperative pain. Cancer pain is well founded. For neuropathic pain the neuromodulators such as some antidepressants and anticonvulsants, have proven to be very useful. It is estimated that 10% of patients with chronic pain are benefited by some kind of intervention.]]></p></abstract>
