La colangiopancreatografía retrógrada endoscópica (CPRE) es considerada el estándar de oro para su diagnóstico y tratamiento. Sin embargo, el uso indiscriminado de la CPRE aumenta el riesgo de complicaciones relacionadas a su ejecución, como pancreatitis post-CPRE, sangrado post esfinterotomía, perforaciones y colangitis. En el año 2019 la Sociedad Americana de Endoscopia Gastrointestinal (ASGE) modificaron los predictores clínicos y categorías de riesgo de coledocolitiasis, con el objetivo de restringir la CPRE exclusivamente a pacientes con alta sospecha de coledocolitiasis en los que la relación riesgo-beneficio sea favorable.  Objetivo: Evaluar el rendimiento diagnóstico de la estratificación de riesgo de las guías 2019 de la ASGE para el diagnóstico de coledocolitiasis en pacientes del Hospital Dr. Gea González.  Material y Métodos: Estudio retrospectivo, transversal y observacional con una cohorte de pacientes del Hospital Dr. Gea González, de enero de 2016 hasta agosto de 2017. Se incluyeron pacientes mayores de 18 años con sospecha de coledocolitiasis y CPRE realizada en el hospital. Las variables clínicas, bioquímicas y de ultrasonido (US) de los pacientes con o sin coledocolitiasis fueron analizadas con las pruebas de Chi cuadrada o exacta de Fisher en el caso de las variables categóricas, y las variables continuas fueron comparadas con las pruebas t de Student o U de Mann-Whitney. Se calcularon la sensibilidad, especificidad, valores predictivos positivo y negativo, razones de verosimilitud de las categorías de riesgo alto o intermedio, así como de los predictores clínicos individuales. Se realizó análisis univariado y multivariado de las categorías de riesgo y de cada predictor.  Resultados: Se evaluaron inicialmente 506 estudios de CPRE, de los cuales se excluyó a 98 por ausencia de reporte de ultrasonido abdominal, estudios de laboratorio incompletos o ser pacientes referidos. El promedio de edad 48.4 años (+ 19.1); 282 mujeres (69.1%) y 126 hombres (30.8%). Un 23.5% (96 pacientes) de las CPRES se realizaron por colangitis como complicación secundaria. La precisión de los predictores de riesgo alto fue de 57.4% (IC 95% 52.2 a 62.1), con sensibilidad de 62.2% (IC 95% 56.1 a 67.9) y especificidad de 49% (IC 95% 41.1 a 56.9). Los predictores de riesgo intermedio tuvieron una precisión de 42.6% (IC 95% 37.9 a 47.5), sensibilidad de 37.8% (IC 95% 32.1 a 43.9) y especificidad de 51% (IC 95% 43.1 a 58.9). La mayor especificidad (81.9%) fue observada con la presencia de lito en el colédoco por ultrasonido, pero con una sensibilidad correspondiente baja (22.4%). El criterio que presentó mayor sensibilidad (99.2%) fue el perfil hepático alterado, pero con una especificidad casi nula (3.4%). El VPP y VPN de todos los criterios fue bajo. Entre los predictores de riesgo alto, solo la bilirrubina total &gt;4 mg/dl junto con colédoco dilatado se asoció con coledocolitiasis (p=0.001, OR 2.02 [IC 95% 1.32 a 3.11]). De los predictores de riesgo intermedio, el único que se asoció con coledocolitiasis fue la presencia de colédoco dilatado (p=0.000, OR 2.69 [IC 95% 1.68 a 4.30]). La regresión logística multivariada mostró que la dilatación del colédoco (OR 2.24; IC 95% 1.31 a 3.82; p=0.003) se asoció de forma independiente con un mayor riesgo de coledocolitiasis en la CPRE.  Conclusiones: En nuestra población de estudio los criterios actuales de la ASGE para la predicción de coledocolitiasis mostraron una precisión predictiva subóptima, con la consecuente realización de CPRE innecesarias.]]></p></abstract>
