<self-uri xlink:href="http://www.scielo.org.mx/scielo.php?script=sci_arttext&amp;pid=S2444-64832020000600386&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://www.scielo.org.mx/scielo.php?script=sci_abstract&amp;pid=S2444-64832020000600386&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://www.scielo.org.mx/scielo.php?script=sci_pdf&amp;pid=S2444-64832020000600386&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="es"><p><![CDATA[Resumen  Introducción: Las lesiones subepiteliales (LSE) comprenden un conjunto de hallazgos endoscópicos que semejan un abultamiento de la pared intestinal a menudo cubierto por mucosa intacta, su diagnóstico diferencial incluye lesiones benignas, potencialmente malignas y malignas no discernibles mediante estudios endoscópicos convencionales. Durante más de 20 años su abordaje ha evolucionado significativamente tras la introducción del ultrasonido endoscópico (USE) y recientemente el disponer de tejido ha permitido mejorar el cuidado de los pacientes.  Objetivo: Describir la utilidad diagnóstica del USE en la evaluación de LSE del tracto digestivo alto.  Materiales y métodos: Estudio de prueba diagnóstica que incluyó pacientes que fueron a endoscopia por lesiones subepiteliales del tracto digestivo alto durante el periodo de tiempo de 01/2016 a 03/2020 en el Centro Médico Nacional 20 de Noviembre; quienes fueron evaluados con videoendoscopia y USE. Se consideraron características asociadas a malignidad: tamaño mayor a 30 mm, adenopatías regionales, ecogenicidad heterogénea y bordes irregulares. La presencia de malignidad se confirmó por histopatología, a partir de muestras por punción con aguja fina.  Resultados: De un total de 65 pacientes referidos por LSE se incluyeron 41 pacientes con LSE confirmadas, 83 % fueron mujeres, la mediana de edad fue 61 (RIC 49-69) años. Se realizó USE en 25/41 pacientes (60.9%); de estos, fueron sometidos a biopsia 14 pacientes. La localización de la LSE fue: 9.75% en esófago, 73.1% en estómago y 17% en duodeno; con un tamaño de 17.5 (RIC 10.6-35.2) mm. Se determino malignidad por criterios de USE en 8/25 (32 %), de los cuales en 6/8 pacientes (75 %) fueron confirmados por histopatología. Para diagnosticar malignidad con el USE se obtuvo: sensibilidad 100% (IC95%, 51-100), especificidad 62.5% (IC95%, 30.5-86.3), VPP 57.1% (IC95%, 25-84), VPN 100% (IC95%, 56.6-100) y exactitud diagnóstica 75% (IC95%, 46.8-91.14). Todas las lesiones malignas fueron tratadas.  Conclusiones: El USE es presenta alta sensibilidad y especificidad en la evaluación diagnóstica de LSE del tracto digestivo alto; las cuales se incrementan al realizar biopsia por punción con aguja fina, especialmente cuando se realizan 2 pases.]]></p></abstract>
