Coronariograf&iacute;a no invasiva y perfusi&oacute;n mioc&aacute;rdica.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Keywords: </b>PET/CT, coronary tomography, Non&#150;invasive coronariography, Myocardial perfusion.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2">La tomograf&iacute;a por emisi&oacute;n de positrones unida a la tomograf&iacute;a computadorizada (PET/CT) es una nueva t&eacute;cnica h&iacute;brida que permite estudiar en forma no invasiva al coraz&oacute;n de manera integral, valorando la anatom&iacute;a y el grado de calcificaci&oacute;n coronaria, las caracter&iacute;sticas de la placa aterosclerosa, as&iacute; como la perfusi&oacute;n mioc&aacute;rdica regional, el flujo coronario cuantificado en mL/min/g de masa mioc&aacute;rdica y el metabolismo del miocito para la detecci&oacute;n de viabilidad mioc&aacute;rdica. Es decir, es un estudio anat&oacute;mico y funcional del coraz&oacute;n.</font></p>     <p align="justify"><font face="verdana" size="2">El cateterismo card&iacute;aco selectivo como est&aacute;ndar de referencia para evaluar el grado de estenosis de las arterias coronarias es un procedimiento invasivo, con bajo riesgo de complicaciones (&lt; 2% incluyendo mortalidad) y con un costo significativo que ofrece la ventaja de realizar intervenciones terap&eacute;uticas de reperfusi&oacute;n.<sup>1,2</sup> Sin embargo, en la pr&aacute;ctica se requiere de un estudio diagn&oacute;stico que permita visualizar la anatom&iacute;a coronaria de forma no invasiva y a un menor costo. Las t&eacute;cnicas para angiograf&iacute;a coronaria no invasiva constituye una alternativa con alta precisi&oacute;n diagn&oacute;stica que permite reconocer la anatom&iacute;a del &aacute;rbol coronario, evaluar la presencia de estenosis coronaria significativa antes de una posible intervenci&oacute;n de reperfusi&oacute;n mec&aacute;nica, o valorar la permeabilidad de <i>stents </i>e injertos coronarios <i><a href="/img/revistas/acm/v75n1/a14f1.jpg" target="_blank">(Fig. 1)</a>.</i><sup>3</sup></font></p>     <p align="justify"><font face="verdana" size="2">Como ya se mencion&oacute; previamente el PET/CT tiene la capacidad para valorar la perfusi&oacute;n y flujo coronario en adici&oacute;n a la anatom&iacute;a coronaria, lo que permite determinar la presencia de isquemia o necrosis mioc&aacute;rdica con una sensibilidad mayor al SPECT, as&iacute; como reconocer la presencia de tejido viable en aquellos pacientes con antecedente de infarto del miocardio y disfunci&oacute;n ventricular en los que se requiere determinar la necesidad de tratamiento m&eacute;dico <i>versus </i>la terapia de revascularizaci&oacute;n <i><a href="/img/revistas/acm/v75n1/a14f2.jpg" target="_blank">(Fig. 2)</a>.</i><sup>4&#150;6</sup> Al cuantificar en forma absoluta el flujo, el PET/CT permite estudiar la funci&oacute;n endotelial y calcular la reserva de flujo coronario.<sup>7</sup></font></p>     <p align="justify"><font face="verdana" size="2">Otra de las aplicaciones de esta t&eacute;cnica es el reconocimiento de actividad oncol&oacute;gica a nivel del coraz&oacute;n <i><a href="/img/revistas/acm/v75n1/a14f3.jpg" target="_blank">(Fig. 3)</a>. </i>La fusi&oacute;n de las t&eacute;cnicas de imagen PET/CT supone nuevos avances en imagenolog&iacute;a cardiovascular que ofrece refinar la capacidad diagn&oacute;stica y estratificar el riesgo de pacientes con enfermedad arterial coronaria en forma integral y no invasiva.<sup>7</sup></font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Referencias</b></font></p>     ]]></body>
