MATERIALES Y MÉTODOS: Estudio de cohortes, epidemiológico, longitudinal, prospectivo y comparativo con y sin intervención, en el que se evaluó la incidencia de hematoma de la cúpula vaginal en pacientes con histerectomía vaginal atendidas en el servicio de ginecología del Hospital General Universitario de Ciudad Real, España, con o sin otras cirugías asociadas. Se recopiló el valor de la hemoglobina pre y posoperatoria para estudiar la pérdida sanguínea; el primer día posquirúrgico se cuantificó una escala visual analógica del dolor y al alta se practicaron: urocultivo, exudado vaginal y ecografía transvaginal para detectar hematoma de la cúpula vaginal, si lo hubiera.  RESULTADOS: Se analizaron 175 pacientes repartidas en dos grupos: uno con colocación de taponamiento vaginal al término de la intervención (n = 89) y el otro sin taponamiento (n = 86). Los grupos se integraron aleatoriamente, por secuencia. El estudio ecográfico transvaginal previo al alta para valorar el hematoma no evidenció diferencias significativas entre los grupos; tampoco para la pérdida de sangre, el dolor o la morbilidad infecciosa.  CONCLUSIONES: El taponamiento vaginal no resultó eficaz para disminuir la incidencia del hematoma de cúpula, ni la pérdida de sangre, ni las complicaciones infecciosas. Si bien su aplicación no influye en la escala del dolor sí obliga al sondaje vesical permanente y a prolongar la estancia hospitalaria.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  OBJECTIVE: To analyze the effectiveness of vaginal packing in reducing the incidence of vaginal post-hysterectomy hematomas and determine its influence on blood loss, infectious morbidity, and postoperative pain.  METHODS AND MATERIALS: We conducted a prospective, comparative, longitudinal, epidemiological cohort study with and without intervention to evaluate the incidence of vaginal dome hematoma in patients undergoing vaginal hysterectomy with or without other associated surgeries at the gynecology department of the General University Hospital of Ciudad Real, Spain. Preoperative and postoperative hemoglobin values were collected to study blood loss. On the first postoperative day, pain was quantified using a visual analog scale, and urine cultures, vaginal exudate cultures, and transvaginal ultrasounds were performed at discharge to detect vaginal vault hematomas, if present.  RESULTS: A total of 175 patients were analyzed and divided into two groups: one with vaginal packing at the end of the procedure (n = 89), and one without packing (n = 86). The groups were randomly assigned in sequence. Transvaginal ultrasounds performed prior to discharge to assess hematomas showed no significant differences between the groups. There were also no differences in blood loss, pain, or infectious morbidity.  CONCLUSIONS: Vaginal packing was ineffective in reducing the incidence of dome hematoma, blood loss, or infectious complications. While its use does not affect the pain scale, it requires permanent bladder catheterization and prolongs the hospital stay.]]></p></abstract>
