MATERIAL Y MÉTODOS. Estudio transversal en 575 adolescentes de 14 a 16 años. Se utilizaron pruebas t de Student, ji cuadrada y correlación de Spearman. RESULTADOS. La prevalencia de SM fue mayor por ATPIII (18.6%) versus IDF (8.2%) (p<0.001), con 41.1% de concordancia. CONCLUSIONES. El principal componente asociado al SM es la obesidad    abdominal, seguido de hipertensi&#243;n arterial sistémica, hipertrigliceridemia,    DM y disminuci&#243;n de HDL-colesterol.<sup>1,2</sup>Para diagnosticar SM en ni&#241;os    y adolescentes existen dos definiciones, la del National Cholesterol Education    Program Adult Treatment Panel &#40;ATPIII&#41;<sup>3 </sup>y la de la International    Diabetes Federation &#40;IDF&#41;.<sup>4</sup> La prevalencia de SM en adolescentes acorde    a ATPIII e IDF var&#237;a de 1.3 a 19.6&#37; y de 1.2 a 3.9&#37;, respectivamente,    dependiendo de la poblaci&#243;n estudiada.<sup>2,5-10</sup> Comparaciones entre estas    definiciones informan diferencias significativas entre ellas.<sup>11,12</sup> </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> El incremento    paralelo de las frecuencias de obesidad y SM es mundial y el caso de México    no es la excepci&#243;n.<sup>13-16</sup> El diagn&#243;stico de SM es un reto por la falta    de criterios espec&#237;ficos para la poblaci&#243;n mexicana. El objetivo de    este estudio es establecer si existen diferencias entre ambas definiciones en    una poblaci&#243;n de adolescentes mexicanos.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Material y métodos</b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Estudio transversal    descriptivo realizado en agosto de 2008. Participaron 575 adolescentes de 14    a 16 a&#241;os de edad, 207 hombres y 368 mujeres Previa entrevista con las autoridades    escolares, se convoc&#243; a todos los estudiantes de nuevo ingreso del turno matutino    de tres preparatorias, 1 200 adolescentes de los cuales aceptaron participar    575 &#40;tasa de no respuesta: 52.1&#37;&#41;. Se obtuvo un consentimiento informado del    padre o tutor y un asentimiento informado del alumno. El protocolo fue aprobado    por el Comité de Ética e Investigaci&#243;n del Centro de Investigaci&#243;n en Ciencias    Médicas de la UAEMex. Para el diagn&#243;stico de SM se utilizaron las definiciones    del ATPIII y de la IDF para adolescentes.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Somatometr&#237;a</i>.    La circunferencia de la cintura &#40;CC&#41; se midi&#243; de acuerdo con los    criterios de la OMS.<sup>17</sup> También se midi&#243; la presi&#243;n arterial    &#40;PA&#41; con un esfigoman&#243;metro de mercurio, en posici&#243;n sentada    y en reposo, por personal capacitado y por duplicado. Se calcul&#243; el &#237;ndice    de masa corporal &#40;IMC&#41; para establecer el sobrepeso &#40;&#62;percentil    85&#41; y obesidad &#40;&#62; percentil 95&#41; utilizando tablas para adolescentes.18</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Perfil bioqu&#237;mico</i>.    En ayuno de 12 horas se tomaron las muestras sangu&#237;neas por venopunci&#243;n    con sistema al vac&#237;o. Los tubos se centrifugaron por 10 min a 500g para    obtener el suero. Cuantificamos glucosa, HDL-colesterol y triglicéridos por    procedimientos enzim&#225;ticos &#40;Randox&#41;. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>An&#225;lisis    estad&#237;stico</i>. Realizamos un an&#225;lisis descriptivo; la diferencia    entre los valores encontrados por ATPIII e IDF se estableci&#243; mediante las    pruebas t de Student y ji cuadrada, y la asociaci&#243;n entre ambas mediante    correlaci&#243;n de Spearman. Consideramos las diferencias significativas cuando    p &#8804; 0.05. &#40;SPSS versi&#243;n 17&#41;. </font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Resultados</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">La prevalencia    de SM por ATPIII fue 18.6&#37; y 8.2&#37; por IDF &#40;p&#60;0.001&#41;, y se encontr&#243; que 41.1&#37;    cumple con ambas definiciones &#40;kappa = 0.517&#41;.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> El an&#225;lisis    de cada par&#225;metro mostr&#243; diferencias estad&#237;sticas por género    &#40;p&#60; 0.005&#41;, excepto en los triglicéridos. Comparando los porcentajes    que rebasaron el punto de corte de los criterios establecidos, en cada par&#225;metro    encontramos una diferencia significativa &#40;p&#60; 0.001&#41;, excepto para    la PA diast&#243;lica &#40;<a href="/img/revistas/spm/v52n6/a07qua1.jpg">cuadro I</a>&#41;. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> El an&#225;lisis    de correlaci&#243;n de Spearman identific&#243; que la variable de mayor impacto    fue la CC para ambas definiciones, seguida de la disminuci&#243;n del HDL-colesterol    y la PA sist&#243;lica para IDF y del aumento de triglicéridos y la PA sist&#243;lica    para ATPIII &#40;<a href="/img/revistas/spm/v52n6/a07qua2.jpg">cuadro II</a>&#41;.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> La prevalencia    de sobrepeso fue 26.6&#37; y 5.7&#37; para obesidad; la prevalencia de SM por    ATPIII en adolescentes con sobrepeso fue 35.1&#37; y 71.3&#37; con obesidad.    Con IDF la prevalencia de SM fue menor con sobrepeso y obesidad &#40;13.2 y    59.4&#37;, respectivamente&#41;, pero la tendencia fue similar &#40;mayor riesgo    de SM con obesidad&#41; &#40;<a href="/img/revistas/spm/v52n6/a07qua1.jpg">cuadro I</a>&#41;.</font></p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Discusi&#243;n    </b> </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Los criterios diagn&#243;sticos    de SM en adultos est&#225;n bien establecidos y su prevalencia es mayor por IDF    versus ATPIII;<sup>19-24</sup> al contrastar con la reportada en ni&#241;os y adolescentes    es mayor por ATPIII,<sup>25-28</sup> resultados que son similares a los datos obtenidos    en este estudio. Sin embargo, existen diferencias entre las definiciones. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Por esta raz&#243;n    decidimos analizar las variables que pudieran influenciar la prevalencia por    ambas definiciones. La variable de mayor impacto fue la CC, lo que concuerda    con estudios realizados en EUA, China, Italia, Argentina, Brasil y Europa.<sup><sup>5,8,11,13,29,30</sup></sup>    La PA sist&#243;lica también fue significativa como ya se hab&#237;a descrito en México    y Brasil.<sup><sup>5,9</sup></sup> Con ATPIII, el aumento de triglicéridos fue el segundo factor    de importancia, acorde con hallazgos descritos en México, EUA, Ir&#225;n y Brasil.<sup>5,7-9</sup>    Para IDF el segundo criterio m&#225;s importante fue la disminuci&#243;n de HDL-colesterol,    similar a lo encontrado en Italia y Per&#250;.<sup>1,29</sup></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> La prevalencia    de SM result&#243; mayor con ATPIII de acuerdo a lo descrito anteriormente.<sup>11,12</sup>    Esto puede deberse a los cambios fisiol&#243;gicos propios del crecimiento del adolescente.    Independientemente de la definici&#243;n empleada, la prevalencia en edad pedi&#225;trica    aument&#243; en todo el mundo<sup>31</sup> como reflejo probable del estilo de vida moderno:    mayor sedentarismo y consumo de alimentos procesados.<sup><sup>13,32</sup></sup> Por supuesto, también    pueden influir los par&#225;metros elegidos para la evaluaci&#243;n del SM y sus puntos    de corte, por lo que es pertinente hacerlos espec&#237;ficos para cada poblaci&#243;n,    considerando género y edad.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> La prevalencia    de SM por ATPIII para los 575 adolescentes estudiantes de 15 a 16 a&#241;os &#40;18.6&#37;&#41;    fue menor a la descrita para 484 adolescentes de 15 a 19 a&#241;os, hijos de trabajadores    urbanos universitarios &#40;35.4&#37;&#41;,<sup>9</sup> tal vez por el rango de edad estudiado, aunque    fue mayor al descrito en EUA &#40;8.6&#37;&#41;<sup>8</sup> y en Brasil &#40;1.3&#37;&#41;.<sup>5</sup> Asimismo, la prevalencia    de SM por IDF observada en este estudio &#40;8.2&#37;&#41; fue mayor que la descrita para    adolescentes de otros pa&#237;ses, EUA &#40;3.9&#37;&#41;,<sup>2</sup> China &#40;1.2&#37;&#41;<sup>10</sup> e Italia &#40;0.02&#37;&#41;.<sup>29</sup></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> El sobrepeso y    la obesidad fueron factores importantes para la detecci&#243;n del SM, siendo    m&#225;s sensible la definici&#243;n de ATPIII &#40;<a href="/img/revistas/spm/v52n6/a07qua1.jpg">cuadro    I</a>&#41;. Esto sugiere que IDF subestima la CC al tomar el percentil 90 en    lugar del percentil 75 del ATPIII, par&#225;metro obtenido como de mayor impacto    para ambas definiciones. Por lo tanto, se recomienda tomar como valor de corte    para ambas definiciones el percentil 75. La CC es un indicador de grasa intraabdominal    que se correlaciona directamente con el riesgo de ECV y con el perfil lip&#237;dico    aterogénico,<sup>33</sup> lo que implica un mayor riesgo de tener obesidad y    sus consecuencias en la edad adulta.<sup>34</sup> </font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Agradecimientos</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Este trabajo es    parte del Proyecto de Tesis Doctoral de la UAEMex y del Programa Nacional de    Posgrado CONACYT y recibi&#243; financiamiento de la UAEMex &#40;proyecto 2571/2007U&#41;.
