Esta patología se caracteriza por una cifosis pronunciada en la columna torácica o toracolumbar, acompañada de cambios estructurales característicos como vértebras en cuña, irregularidades en los platillos vertebrales y la presencia de nódulos de Schmorl. A diferencia de otras deformidades posturales, la enfermedad de Scheuermann implica alteraciones morfológicas permanentes en las vértebras afectadas. Este trastorno musculoesquelético no sólo impacta la postura y anatomía del paciente, sino que también puede generar un cuadro de dolor crónico y limitación física significativa, especialmente cuando no se detecta y trata de manera oportuna durante la adolescencia. La progresión natural de la enfermedad puede resultar en una deformidad estructural irreversible, incluso con el mejor tratamiento disponible en casos avanzados. Las protrusiones del disco intervertebral y los cambios degenerativos secundarios frecuentemente causan síntomas severos que presentan un desafío terapéutico considerable. Los hallazgos radiológicos característicos muestran una cifosis dorsal que oscila entre 50o y 70o, frecuentemente acompañada de escoliosis de grado variable y acuñamiento vertebral anterior en al menos tres vértebras consecutivas. El manejo terapéutico debe individualizarse según la severidad de la deformidad, la edad del paciente y la presencia de síntomas. Las opciones de tratamiento incluyen desde medidas conservadoras como fisioterapia y uso de corsé, hasta intervenciones quirúrgicas en casos severos o refractarios al tratamiento conservador. Es importante destacar que la intervención quirúrgica se reserva para casos severos (cifosis &gt; 75o) con dolor intratable o progresión documentada a pesar del tratamiento conservador, con déficits neurológicos, compromiso cardiopulmonar y mala plasticidad. El seguimiento a largo plazo es fundamental para monitorizar la progresión de la deformidad y la aparición de complicaciones secundarias. Los pacientes incluidos en este estudio presentan un espectro clínico notablemente variable, que abarca desde casos asintomáticos con deformidad visible hasta pacientes que experimentan dolor crónico incapacitante y limitación funcional significativa. Se presenta el tratamiento para mantener una funcionalidad adecuada y el seguimiento recomendable para estos pacientes, particularmente durante el periodo de crecimiento con la finalidad de optimizar los resultados a largo plazo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract: Scheuermann&#8217;s disease represents a distinct form of structural kyphosis that predominantly affects adolescents during their growth period. This condition is characterized by marked kyphosis of the thoracic or thoracolumbar spine, accompanied by characteristic structural changes such as wedge-shaped vertebrae, irregularities in the vertebral endplates, and the presence of Schmorl&#8217;s nodes. Unlike other postural deformities, Scheuermann&#8217;s disease involves permanent morphological alterations in the affected vertebrae. This musculoskeletal disorder not only impacts the patient&#8217;s posture and anatomy but can also lead to chronic pain and significant physical limitation, especially when it is not detected and treated promptly during adolescence. The natural progression of the disease may result in irreversible structural deformity, even with the best available treatment in advanced cases. Protrusion of the intervertebral discs and secondary degenerative changes frequently cause severe symptoms that pose a considerable therapeutic challenge. Characteristic radiological findings show dorsal kyphosis ranging between 50° and 70°, often accompanied by variable degrees of scoliosis and anterior wedging in at least three consecutive vertebrae. Therapeutic management should be individualized based on the severity of the deformity, the patient&#8217;s age, and the presence of symptoms. Treatment options range from conservative measures such as physical therapy and bracing to surgical interventions in severe or refractory cases. It is important to note that surgical intervention is reserved for severe cases (kyphosis &gt; 75°) with intractable pain or documented progression despite conservative treatment, neurological deficits, cardiopulmonary compromise, or poor skeletal plasticity. Long-term follow-up is essential to monitor the progression of the deformity and the development of secondary complications. The patients included in this study present a notably variable clinical spectrum, ranging from asymptomatic individuals with visible deformity to those experiencing debilitating chronic pain and significant functional limitations. This report presents treatment strategies aimed at maintaining adequate functionality and provides recommendations for follow-up, particularly during the growth period, to optimize long-term outcomes.]]></p></abstract>
