Sin embargo, es invasiva y confiere riesgo de nefrotoxicidad. El uso de ultrasonido dúplex vascular (USV) podría ser una alternativa no invasiva en lugar de la arteriografía. Nuestro objetivo es determinar si el USV es tan eficiente como la angiografía para el diagnóstico y plan terapéutico en la EAP.  Material y métodos: Estudio observacional, de concordancia, en pacientes con EAP que fueron sometidos a USV y a arteriografía en el Hospital Regional Licenciado Adolfo López Mateos del Instituto de Seguridad y Servicios Sociales de los Trabajadores del Estado. Se compararon las siguientes variables: segmento afectado por EAP, vía de acceso vascular y los vasos sanguíneos a intervenir. Se utilizó el índice de concordancia kappa, y como prueba inferencial la Chi-cuadrada. Una p &lt; 0.05 se consideró estadísticamente significativa.  Resultados: Se incluyeron 36 pacientes, de edad promedio 68 ± 10 años, el 61% eran masculinos. El 97.2% tenían diabetes mellitus, el 75%, hipertensión arterial, el 16.7%, dislipidemia, el 36.1% fumaban y el 30.6% tenían enfermedad renal crónica. La manifestación clínica predominante entre los pacientes fue isquemia crítica con pérdida de tejido en el 80.6% de los casos, dolor en reposo en el 11.1% de los casos y claudicación al caminar &lt; 200 m en el 8.3%. El angiosoma más afectado fue el tibial anterior (55.6% de los casos), seguido del tibial posterior (22.2%), y el tipo de revascularización realizada a los pacientes fue la directa (61.1% de los casos). Se determinó concordancia diagnóstica con un valor kappa global de 0.713 (p &lt; 0.001). Para enfermedad multinivel, en vasos tibiales, el kappa fue de 0.769. La concordancia para el sistema TASC II entre USG dúplex y la arteriografía tuvo un valor kappa de 0.621 (p &lt; 0.001). La concordancia entre ultrasonografía (USG) dúplex y angiografía por sustracción digital (ASD) para planificar el sitio de acceso vascular fue de 1. Para identificar la arteria específicamente afectada con EAP, la concordancia fue de 0.3.  Conclusiones: El USV fue tan eficiente como la arteriografía para la planeación del abordaje quirúrgico y la elección del sitio de acceso en EAP, así como identificar la circulación principal o segmento afectado, pero es una opción poco precisa para identificar la totalidad de arterias afectadas.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background: Angiography is the gold standard for diagnosis and to evaluate the extension of peripheral arterial disease (PAD). However, it is invasive and confers risk of nephrotoxicity. The use of vascular ultrasonography (VU) could be a non-invasive diagnostic alternative. The aim of this study is to compare the VU with angiography for the diagnosis and therapeutic plan in EAP.  Material and methods: Observational study of concordance in patients with PAD who underwent USG duplex and arteriography in the Hospital Regional Licenciado Adolfo López Mateos. The following variables were compared; segment affected by PAD, vascular access route and blood vessels to treat. The kappa concordance index was used, and the Chi-square test was used as an inferential test. A p &lt; 0.05 was considered statistically significant.  Results: 36 patients were included, of average age 68 ± 10 years, 61% were male. 97.2% had diabetes mellitus, 75% had high blood pressure, 16.7% had dyslipidemia, 36.1% were smokers and 30.6% had chronic kidney disease. The predominant clinical manifestation among the patients was critical ischemia with tissue loss in 80.6% of cases, pain at rest in 11.1% of cases and claudication when walking &lt; 200 m in 8.3%. The most affected angiosoma was the tibialis anterior (55.6% of the cases), followed by the posterior tibialis (22.2%) and the type of revascularization performed on the patients was direct (61.1% of the cases). Diagnostic concordance was determined with a global kappa of = 0.713; p &lt; 0.001; for multilevel disease, in tibial vessels the kappa was 0.769. The concordance for the TASC II system between duplex USG and arteriography had a kappa value of 0.621 (p &lt; 0.001). The agreement between duplex USG and ASD to plan the vascular access site was 1. To identify the artery specifically affected with PAD, the agreement was 0.3.  Conclusions: The VU was as efficient as the Arteriography for the planning of the surgical approach and the choice of the access site in PAD, as well as to identify the main circulation or affected segment, but it is an inaccurate option to identify the totality of affected arteries.]]></p></abstract>
