El objetivo del estudio fue determinar el valor predictivo de la PCT en el diagnóstico de bacteriemia y su capacidad de diferenciar gérmenes gramnegativos y grampositivos  Métodos: Estudio de cohorte retrospectivo realizado en el Hospital Oncológico Solón Espinosa Ayala, Quito-Ecuador, que incluyó todos los pacientes adultos que ingresaron con neutropenia febril a la terapia intensiva. Se evaluaron los valores de PCT al ingreso junto con muestras de hemocultivos, desde el año 2010 al 2016  Resultados: En 117 pacientes se investigó la asociación entre niveles de PCT y el diagnóstico de bacteriemia, la PCT demostró discriminación de bacteriemia, punto de corte de 15.5 ng/ml y un área bajo la curva (AUC) de 0.76 en la curva ROC (curva operador-receptor). La concentración de PCT en pacientes con bacteriemia por gérmenes gramnegativos fue mayor comparada con pacientes con bacteriemias por gérmenes grampositivos, 26 vs. 7.1 ng/ml (p = 0.001). Valores mayores a 26.8 ng/ml predicen bacteriemia por bacilos gramnegativos (AUC: 0.81)  Conclusiones: En pacientes neutropénicos febriles críticos, un valor de corte de PCT mayor a 15.5 ng/ml es un predictor de bacteriemia (AUC: 0.76) y valores mayores a 26.8 ng/ml predicen bacteriemia por bacilos gramnegativos (AUC: 0.81).]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Background: Procalcitonin (PCT) could predict the presence of bacteremia in critically ill patients with febrile neutropenia. The objective was to determine the predictive value of PCT in the diagnosis of bacteremia and its ability to differentiate between gram-negative and gram-positive organisms  Methods: Retrospective cohort study conducted at the Oncology Hospital “Solón Espinosa Ayala,&#8221; Quito-Ecuador, which included patients who were admitted with febrile neutropenia to intensive therapy. PCT values were evaluated at admission together with blood culture samples, from 2010 to 2016  Results: The association between PCT levels and the diagnosis of bacteremia was investigated in 117 patients. PCT demonstrated bacteremia discrimination with a point of cut of 15.5 ng/ml and an area under the curve (AUC) of 0.76. The concentration of PCT in patients with bacteremia by gram-negative germs was higher compared with patients with bacteremias by gram-positive germs, 26 vs. 7.1 ng/ml (p = 0.001).
