Se llevó a cabo una revisión exhaustiva de la bibliografía en PubMed y Google Scholar, utilizando términos MeSH y no MeSH vinculados con urticaria, histamina y dieta. La revisión incluyó 78 artículos publicados entre 2014 y 2024. La urticaria crónica espontánea es una enfermedad persistente, en la que una cantidad considerable de pacientes no responde al tratamiento de manera óptima. Incluso un tercio de ellos podría beneficiarse al reducir el consumo de pseudoalérgenos y alimentos con alta cantidad de histamina. Las aminas bioactivas, presentes en varios alimentos, aumentan los síntomas de la urticaria crónica espontánea, al promover la liberación de histamina y otros mediadores inflamatorios. La restricción de histamina en la dieta y el fortalecimiento de la función de la enzima diamino-oxidasa parecen aliviar los síntomas, aunque la variabilidad en las concentraciones de histamina reportados en diferentes estudios complica la formulación de guías estandarizadas. Se concluye que la dieta baja en histamina y pseudoalérgenos es una opción prometedora para reducir los síntomas en pacientes con urticaria crónica espontánea, particularmente en quienes no logran un control adecuado con antihistamínicos. Se recomienda evaluar la eficacia de esta dieta en un periodo de tres a cuatro semanas para observar posibles beneficios. Aun así, se requieren estudios adicionales y mayor estandarización para definir las concentraciones seguras de histamina en los alimentos y mejorar las recomendaciones terapéuticas.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract This article analyzes the effects of a histamine- and pseudoallergen-restricted diet as an additional strategy in the treatment of chronic spontaneous urticaria (CSU). A comprehensive review of the literature was conducted using PubMed and Google Scholar, employing both MeSH and non-MeSH terms related to urticaria, histamine, and diet. The review included 78 selected articles published between 2014 and 2024. CSU is a persistent disease in which a considerable number of patients do not respond optimally to standard treatments. Up to one-third of these patients may benefit from reducing the intake of pseudoallergens and histamine-rich foods. Bioactive amines present in various foods tend to exacerbate CSU symptoms by promoting the release of histamine and other inflammatory mediators. The restriction of dietary histamine and the enhancement of diamine oxidase (DAO) enzyme function appear to alleviate symptoms, although variability in reported histamine levels across studies complicates the development of standardized guidelines. It is concluded that a low-histamine and pseudoallergen diet is a promising option for reducing symptoms in patients with chronic spontaneous urticaria (CSU), particularly those who do not achieve adequate control with antihistamines. Evaluating the effectiveness of this diet over a period of three to four weeks is recommended to observe potential benefits. Nevertheless, additional studies and further standardization are necessary to define safe histamine levels in foods and improve therapeutic recommendations.]]></p></abstract>
