<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  Sepsis and septic shock remain one of the main causes of mortality and morbidity in the Intensive Care Unit. Myocardial dysfunction related to sepsis and septic shock is expressed as systolic or diastolic dysfunction of the left and/or right ventricle, isolated or combined, associated with an unavoidable short- and long-term outcome, compared to patients with preserved cardiac function; during its approach we will face a complex and difficult decision-making process, this is where transthoracic echocardiography becomes relevant as a diagnostic tool. Left atrial strain (LAS) allows early detection of left ventricular (LV) dysfunction before LVEF decline, assessing atrial function throughout the cardiac cycle by measuring reservoir, conduction, and pump functions. Reservoir phase left atrial strain (RSLS) could provide early recognition of clinical implications that facilitate risk stratification and prognosis of adverse events.  Objective:  To analyze whether reservoir phase left atrial strain could be an early predictor of diastolic dysfunction in refractory septic shock in the Intensive Care Unit of the ISSSTE Veracruz High Specialty Regional Hospital B.  Material and methods:  Observational, descriptive, retrospective, unidirectional, single-center, homodemic study. It was carried out in the Intensive Care Unit of the ISSSTE Veracruz High Specialty Regional Hospital B, between May 2023 and July 2024. In a finite, non-probabilistic convenience sample with a record of 35 patients.  Results:  In a population of 35 patients, a binary logistic regression model was used to evaluate that RSLS &lt; 19% is associated as a risk factor for death with an OR 1.62 (95% CI 1.215-2.113), which generates an estimated score of 68%, being a strong relationship of variability in the occurrence of complications through the Nagelkerke R² coefficient of determination and representing with a p-value of 0.001, the survival rate was determined using the Kaplan-Meier curve where we can see that the survival rate is higher in patients with a RSLS &gt; 20% compared to those with a RSLS &lt; 19%. Regarding the RSLS mortality probability analysis, it turned out to be non-inferior to PSAP and fluid balance, however, with PVC it &#8203;&#8203;does not show statistical significance in the mortality outcome, presenting a p-value of 0.69.  Conclusions:  RSLS could be feasible to predict the prognosis of adverse events associated with morbidity and mortality, therefore, justifying its gradual implementation in routine clinical practice, which makes it important as part of the evaluation in patients with refractory septic shock. However, it is important to consider that more studies should be carried out and thus establish a new line of research.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo:  Introdução:  Sepse e choque séptico continuam sendo uma das principais causas de mortalidade e morbidade na Unidade de Terapia Intensiva. A disfunção miocárdica relacionada à sepse e ao choque séptico se expressa como disfunção sistólica ou diastólica do ventrículo esquerdo e/ou direito, isolada ou combinada, associada a um desfecho inevitável a curto e longo prazo, em comparação com pacientes com função cardíaca preservada; Durante a abordagem enfrentaremos um processo complexo e difícil de tomada de decisão, sendo aí que a ecocardiografia transtorácica se torna relevante como ferramenta diagnóstica. A deformação atrial esquerda (LAS) permite a detecção precoce da disfunção ventricular esquerda (VE) antes do declínio da FEVE, avaliando a função atrial ao longo do ciclo cardíaco medindo as funções de reservatório, condução e bomba. A deformação atrial esquerda (LASr) na fase de reservatório pode nos oferecer o reconhecimento precoce de implicações clínicas que facilitam a estratificação de risco e o prognóstico de eventos adversos.  Objetivo:  Analisar se a deformação atrial esquerda em sua fase de reservatório pode ser um preditor precoce de disfunção diastólica no choque séptico refratário na Unidade de Terapia Intensiva do Hospital Regional de Alta Especialidade B do ISSSTE Veracruz.  Material e métodos:  Estudo observacional, descritivo, retrospectivo, unidirecional, unicêntrico, homodêmico. Foi realizado na Unidade de Terapia Intensiva do Hospital Regional de Alta Especialidade B do ISSSTE Veracruz, entre maio de 2023 e julho de 2024. Em uma amostra finita, não probabilística, de conveniência, com registro de 35 pacientes.  Resultados:  Em uma população de 35 pacientes, foi utilizado um modelo de regressão logística binária para avaliar que SAIr &lt; 19% está associado como fator de risco para óbito com um OR 1,62 (IC 95% 1,215-2,113), o que gera um escore estimado de 68%, sendo uma forte relação de variabilidade na ocorrência de complicações através do coeficiente de determinação R² de Nagelkerke e representando um p-valor de 0,001, a taxa de sobrevida foi determinada utilizando a curva de Kaplan-Meier onde podemos observar que a taxa de sobrevida é maior em pacientes com SAIr &gt; 20% em comparação àqueles com SAIr &lt; 19%. Em relação à análise da probabilidade de mortalidade do SAIr, esta se mostrou não inferior com o PSAP e com o balanço hídrico, porém, com o PVC não apresenta significância estatística no desfecho mortalidade, apresentando um p-valor de 0,69.  Conclusões:  O SAIr pode ser viável para predizer o prognóstico de eventos adversos associados à morbidade e mortalidade, justificando, portanto, sua implementação gradual na prática clínica de rotina, o que o torna importante como parte da avaliação em pacientes com choque séptico refratário. No entanto, é importante considerar que mais estudos devem ser realizados e, assim, estabelecer uma nova linha de pesquisa.]]></p></abstract>
