La prevalencia y mortalidad sigue siendo alta en Colombia, con limitación para acceso a la terapia antirretroviral, que se ha traducido en altos costos y malos resultados de supervivencia.  Objetivo:  caracterizar de forma sociodemográfica y clínica pacientes críticamente enfermos con diagnóstico de VIH admitidos en UCI en la ciudad de Medellín.  Material y métodos:  se realizó un estudio observacional, descriptivo, de corte transversal por medio de revisión de historias clínicas de pacientes atendidos en una UCI durante los años 2008-2011.  Resultados:  se incluyeron 154 pacientes. La causa más frecuente de ingreso a UCI fue la insuficiencia respiratoria aguda (46.7%), con una mortalidad en UCI reportada de 41.6%. Las tres infecciones más frecuentes encontradas y que generaron insuficiencia respiratoria aguda como causa de ingreso a UCI fueron: neumonía por Pneumocystis jirovecii (21.4%), tuberculosis pulmonar (18.2%), y neumonía nosocomial adquirida durante la estadia en el hospital (15.6%). Los factores de riesgo asociados con mortalidad fueron: la utilización de ventilación mecánica, el requerimiento de terapia de reemplazo renal continua (TRRC), la presencia de desnutrición, y la aparición de complicaciones en la UCI.  Conclusiones:  las características de nuestra muestra son similares a las reportadas en otras series; difieren en el tipo de infecciones oportunistas y la mortalidad atribuible a la infección VIH/SIDA. La principal causa de ingreso a cuidado intensivo fue la insuficiencia respiratoria aguda, seguida por el compromiso neurológico y la sepsis. Pneumocystis jirovecii fue la infección oportunista más frecuente.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  the reasons for admission to the intensive care unit (ICU) have been studied in patients with the human immunodeficiency virus (HIV). Prevalence and mortality remain high in Colombia, with limited access to antiretroviral therapy, which has translated into high costs and poor survival outcomes.  Objective:  to characterize social demographically and clinically critically ill patients diagnosed with HIV admitted to the ICU in the city of Medellín.  Material and methods:  an observational, descriptive, cross-sectional study was carried out by reviewing the medical records of patients treated in an ICU during the years 2008-2011.  Results:  154 patients were included. The most frequent cause of ICU admission was acute respiratory failure (46.7%), with a reported ICU mortality of 41.6%. The three most frequent infections found that generated acute respiratory failure as a cause of admission to the ICU were Pneumocystis jirovecii pneumonia 21.4%, pulmonary tuberculosis 18.2%, and nosocomial pneumonia acquired during the hospital stay (15.6%). The risk factors associated with mortality were: the use of mechanical ventilation, the requirement for CRRT, the presence of malnutrition, and the appearance of complications in the ICU.  Conclusions:  the characteristics of our sample are like those reported in other series; they differ in the type of opportunistic infections and the mortality attributable to HIV/AIDS infection. The main cause of admission to intensive care was acute respiratory failure, followed by neurological compromise and sepsis. <abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo:  Introdução:  os motivos de internação na Unidade de Terapia Intensiva (UTI) foram estudados em pacientes com o vírus da imunodeficiência humana (HIV). A prevalência e a mortalidade continuam altas na Colômbia, com acesso limitado à terapia antirretroviral, o que resultou em altos custos e maus resultados de sobrevivência.  Objetivo:  caracterizar sociodemograficamente e clinicamente os pacientes em estado crítico com diagnóstico de HIV internados na UTI na cidade de Medellín.  Material e métodos:  foi realizado um estudo observacional, descritivo, transversal e descritivo por meio da revisão dos registros médicos de pacientes atendidos em uma UTI entre 2008 e 2011.  Resultados:  foram incluídos 154 pacientes. A causa mais frequente de admissão na UTI foi a insuficiência respiratória aguda (46.7%), com uma mortalidade relatada na UTI de 41.6%. As três infecções mais frequentes encontradas que causaram insuficiência respiratória aguda como causa de admissão na UTI foram: pneumonia por Pneumocystis jirovecii (21.4%), tuberculose pulmonar (18.2%) e pneumonia nosocomial adquirida durante a internação hospitalar (15.6%). Os fatores de risco associados à mortalidade foram: o uso de ventilação mecânica, a necessidade de TRRC, a presença de desnutrição e a ocorrência de complicações na UTI.  Conclusões:  as características da nossa amostra são semelhantes às relatadas em outras séries; elas diferem no tipo de infecções oportunistas e na mortalidade atribuível à infecção por HIV/AIDS.
