Entre las alternativas terapéuticas más estudiadas se encuentra la ventilación en posición prona.  Objetivo:  Demostrar que la ventilación en posición prona aplicada de manera temprana y de forma prolongada en pacientes con SDRA severo mejora los parámetros de oxigenación.  Material y métodos:  Estudio retrospectivo realizado en el Departamento de Medicina Crítica del Centro Médico ABC Observatorio, del 1 de marzo de 2013 al 31 de julio de 2015.  Resultados:  Se estudiaron 34 individuos con SDRA severo. La respuesta a la colocación en posición prona fue valorada de acuerdo a su respuesta en la PaO2/FiO2. La permanencia de la PaO2/FiO2 en posición supina fue mayor en el grupo de origen pulmonar, en 227 mmHg, comparado con el extrapulmonar, 186 mmHg. La mortalidad de la población a las 48 horas fue de 11.7% (cuatro) y a los 28 días, 47% (16).  Discusión:  En nuestro trabajo se incluyeron 34 sujetos con diagnóstico de SDRA severo. En todos los sujetos que fueron llevados a posición prona y luego regresados a posición supina se observó mejoría al cambio de posición supina. Lamentablemente, el efecto de la pronación sobre la supervivencia es aún incierto, ya que le mejoría en la oxigenación no implica necesariamente disminución en la mortalidad.  Conclusiones:  Se demostró que la ventilación prona continúa siendo una medida segura en los pacientes con síndrome de distrés respiratorio agudo grave, con mejoría en los niveles de PaO2/FiO2, aunque sin ella en la mortalidad a los 28 días.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  From the first definition of acute respiratory distress syndrome (ARDS), the treatment of this condition has been based on the correction of hypoxemia. Among the most studied alternative therapies is prone ventilation.  Objective:  To demonstrate that prolonged prone ventilation applied early in patients with severe ARDS improves the oxygenation parameters.  Material and methods:  Retrospective study conducted in the Department of Critical Medicine of the ABC Medical Center from March 2013 to July 2015.  Results:  Thirty-four patients with severe ARDS were studied. Their response to the placement in the prone position was assessed according to their response in PaO2/FiO2. The permanence of the PaO2/FiO2 in supine position was greater in the group of pulmonary origin, 227 mmHg, compared to the extrapulmonary, 186 mmHg. The mortality of the population at 48 hours was 11.7% (four) and at 28 days 47% (16).  Discussion:  In our study, 34 patients with a diagnosis of severe ARDS were included. All of those who were brought into a prone position and then returned to a supine position showed improvement upon the change from supine. Unfortunately, pronation effect on survival is still uncertain as improvement in oxygenation will not necessarily imply a reduction in mortality.  Conclusions:  The prone ventilation continues to be a reliable measure in patients with severe ARDS, with improved levels of PaO2/FiO2, but no improvement in the mortality at 28 days.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo:  Introdução: Desde a primeira definição (SARA) o tratamento atual desta entidade é baseada na correção da hipoxemia. Entre as alternativas terapêuticas mais estudadas é a ventilação em posição prona.  Objetivo:  Demonstrar que a ventilação em posição prona aplicada de manera precoce e prolongada em pacientes com SARA grave melhora os parâmetros de oxigenação.  Material e metodos:  Estudo retrospectivo realizado no Departamento de Medicina Crítica do Centro Médico ABC Observatório,1 de março de 2013 a 31 de julho de 2015.  Resultados:  Foram estudados 34 pacientes com SARA grave. A resposta à colocação em posição prona foi avaliado de acordo a PaO2/FiO2. A permanência do PaO2/FiO2 em posição supina foi maior no grupo de origem pulmonar (227 mmHg) comparada com a extrapulmonar (186 mmHg). A mortalidade da população do estudo de 48 horas foi de 11.7% e em 28 dias foi de 47%.  Discussão:  Em nosso estudo foram incluídos 34 pacientes com o diagnóstico de SARA grave. As mudanças de posição em pacientes que se encontravam na posição prona e logo regressados a posição supina, em nosso estudo, se observou uma melhoria em todos os pacientes que mudaram à posição supina. Lamentavelmente o efeito da pronação sobre a sobrevivência ainda é incerto, já que a melhoría da oxigenação não necessariamente diminui a mortalidade.  Conclusões:  Demonstrou-se que a ventilação prona continua sendo uma medida segura em pacientes com SARA, existe uma melhoria dos níveis de PaO2/FiO2, sem melhoria na mortalidade aos 28 dias.]]></p></abstract>
