Material y métodos: Diseño: Cohorte prospectiva. Sitio: Unidad de Cuidados Intensivos Respiratorios (UCIR) del Hospital General de México, O.D. Pacientes: De 453 pacientes que ingresaron a la UTI, se incluyeron 446 casos, 235 fueron del sexo masculino (52.7%). La edad media fue de 47.9 ± 16.7 años. Intervenciones: Ninguna.  Resultados:  Hubo 151 aislamientos de A. baumannii/haemolyticus en 85 pacientes (19.1%). La edad media fue de 50.6 ± 17.0 años (20-85 años). Las principales comorbilidades previas a la hospitalización fueron diabetes 25 casos (29.4%) e hipertensión arterial 22 (25.9%). Los principales motivos de ingreso fueron choque séptico en 39 (45.9%), insuficiencia respiratoria aguda 16 (18.8%) y coma 10 (11.8%). El SAPS-III promedio fue de 60.8 ± 14.6, con una mortalidad predicha del 48.3%. La mortalidad observada al alta de la UTI y el hospital fue de 51.8% y 61.2% respectivamente. La razón de mortalidad predicha fue de 1.27. El tiempo de estancia en la UTI y el hospital fue de 18.4 ± 14.9 días y 30.2 ± 22.4 días respectivamente. La incidencia de infección por A. Baumannii fue del 19.1%., siendo los sitios de aislamiento más frecuentes vía aérea, sangre y herida quirúrgica. Las cepas aisladas fueron por definición multidrogo resistentes. La sensibilidad osciló entre el 19 y 26.5% para aztreonam, piperacilina-tazobactam, moxifloxacino, tobramicina, imipenem y gentamicina. El único antibiótico con 100% de susceptibilidad fue la tigeciclina.  Conclusiones:  El aislamiento de A. Baumannii impacta la morbilidad y la mortalidad y se relaciona directamente con la gravedad del paciente, reflejada en mayores puntajes en la escala de SAPS III de mortalidad predicha e incidencia de Disfunción Orgánica Múltiple en comparación con el resto de pacientes críticos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Objective:  Describe the incidency, demographics, associated factors, SAPS-III, mortality associated with A. baumannii infection and frequency of sites culture isolation and drug resistance profile.  Methods: Design: Prospective cohort. Setting: Hospital General de Mexico&#8217;s Respiratory Intensive Care Unit (UCIR). Patients: Of 453 patients admitted to the ICU, 446 cases were included, 235 were male (52.7 %). Mean age was 47.9 ± 16.7 years. Interventions: None.  Results:  There were 151 isolates of A. baumannii/haemolyticus in 85 patients (19.1%). Mean age was 50.6 ± 17.0 years (20-85 years). Main previous comorbidities were diabetes mellitus, 25 cases (29.4%) and systemic hypertension, 22 cases (25.9%). Main reasons for admission were septic shock in 39 patients (45.9%), acute respiratory failure, 16 patients (18.8%) and coma, 10 patients (11.8%). Mean SAPS-III was 60.8 ± 14.6, predicted mortality 48.3%. Observed mortality at discharge from the ICU and hospital were 51.8% and 61.2%, respectively. Predicted mortality ratio was 1.27. Average ICU and hospital length of stay were 18.4 ± 14.9 and 30.2 ± 22.4 days, respectively. Isolated strains were MDR by definition. Sensitivity was 19% to 26.5% for aztreonam, piperacillin-tazobactam, moxifloxacin, tobramycin, imipenem and gentamicin, in that order, without being greater for any other antimicrobial drug, except tigecycline with 100% of susceptibility.  Conclusions:  The isolation of A. baumannii yields a high impact in morbidity and mortality and is directly related to patient severity reflected in higher scores on scales SAPS III predicted mortality and incidence of MODS and, as expected, greater than other patients in the ICU.]]></p></abstract>

Pacientes: De 453 pacientes internados na UTI, 446 casos foram incluídos, 235 eram do sexo masculino (52.7%). A idade média foi de 47.9 + 16.7 anos. Intervenções: Nenhuma.  Resultados:  Foram encontrados 151 cepas isoladas de A. baumannii/haemolyticus em 85 pacientes (19.1%). A idade média foi de 50.6 ± 17.0 anos (20-85 anos). As principais comorbidades prévias a hospitalização foram diabetes 25 casos (29.4%) e hipertensão arterial 22 (25.9%). Os principais motivos para admissão foram choque séptico em 39 (45.9%), insuficiência respiratória aguda 16 (18.8%) e coma 10 (11.8%). A incidência de infecção por A. baumannii foi de 19.1%, sendo os locais mais comuns de isolamento a via aérea, sangue e ferida cirúrgica. As cepas isoladas foram por definição multidroga resistente. A sensibilidade variou entre 19 e 26.5% para o aztreonam, piperacilina-tazobactam, moxifloxacina, tobramicina, imipenem e gentamicina. O único antibiótico com 100% de sensibilidade foi a tigeciclina.  Conclusões:  O isolamento de A. baumannii impacta a morbilidade e a mortalidade e está diretamente relacionado com a gravidade do paciente, refletida na maior pontuação na escala de SAPS 3 de mortalidade prevista e incidência de Insuficiência de Multiplos Órgãos em comparação com outros pacientes críticos.]]></p></abstract>
