Del total de casos de sangrado gastrointestinal la hemorragia gastrointestinal baja (HGIB) constituye del 20 al 30%. A pesar de ser menos frecuente que la hemorragia gastrointestinal alta (HGIA) y a diferencia de esta, en la actualidad no existen escalas validadas en nuestra población, que nos permitan clasificar con precisión a los pacientes con HGIB de bajo riesgo que no requieren métodos diagnósticos-terapéuticos de urgencia y puedan ser manejados como pacientes ambulatorios, causando un impacto importante en el uso de recursos y ocupación de los servicios de salud.  Objetivo: Evaluar la utilidad clínica de la escala SHA2PE en la identificación de pacientes de bajo riesgo con hemorragia gastrointestinal baja en un Hospital de 3er nivel. Material y métodos: Estudio observacional, transversal, analítico, prueba pronostica. Se determinó la utilidad clínica de la escala mediante pruebas de sensibilidad y especificidad VPP, VPN, RVSP y RVSN, área bajo la curva en análisis ROC. La variable dependiente fue el requerimiento de tratamiento hospitalario en pacientes con HGIB. Se determinó con estadística descriptiva e inferencial, OR y análisis multivariados, en todos los casos se tomó un valor de p&#8804; 0.05 como estadísticamente significativo.  Resultados: Se incluyeron un total de 85 pacientes, la edad media fue de 61 años (±17), 51.8% fueron hombres (44 pacientes). El 81.2% fueron tratados como pacientes ambulatorios (69 pacientes), el 60% de la muestra tuvieron riesgo bajo según la escala SHA2PE. La sensibilidad de la escala fue de .812 y especificidad de .580 con un área bajo la curva de .753 (IC al 95%= .592, - .912). Prevalencia 18%. VPP=8%, VPN=64%, RVSP=0.37 y RVSN=2.35.  Conclusión: la escala SHA2PE tiene una alta sensibilidad y especificidad moderada para detectar pacientes de bajo riesgo con HGIB. El mejor punto de corte que ofrece mayor certeza diagnóstica con respecto a la detección de pacientes con bajo riesgo es de 1 punto en la escala de SHA2PE.]]></p></abstract>
