Se evaluó el impacto del screening preoperatorio, la efectividad de la descolonización dirigida en portadores de SA y la prevalencia de infecciones articulares periprotésicas (IAP) agudas en pacientes sometidos a artroplastía de cadera y rodilla.  Material y métodos:  estudio observacional analítico, de casos y controles, ambispectivo y consecutivo. Se incluyeron 256 pacientes para cirugía protésica de cadera y rodilla entre Enero de 2020 y Diciembre de 2022. Aquéllos intervenidos en el período previo a la búsqueda prequirúrgica de Staphylococcus aureus, se denominó grupo preintervención (G1) entre Enero de 2020 y Junio de 2021 y aquéllos intervenidos en la búsqueda y descolonización denominado grupo intervención (G2) entre Julio de 2021 a Diciembre de 2022. En el G2 se evaluó la portación de SA y la efectividad de la descolonización bacteriana. También se compararon las prevalencias de infección periprotésica aguda por dicho germen en ambos grupos.  Resultados:  la prevalencia de colonización nasal e inguinal para SA meticilino sensible fue de 25.8% y la de SA meticilino resistente de 2.3%. La terapia de descolonización preoperatoria en portadores de SA se asoció con una reducción del índice de infecciones periprotésicas.  Conclusiones:  la incidencia de portadores de Staphylococcus aureus en pacientes programados para artroplastía de cadera y rodilla fue elevada. La detección preoperatoria de SA con una terapia de descolonización dirigida se asoció con una reducción de las infecciones periprotésicas mediadas por este microorganismo]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  the impact of preoperative screening and the effectiveness of targeted decolonization in carriers of Staphylococcus aureus (SA) on the prevalence of acute periprosthetic infections (PJI) in patients undergoing hip and knee arthroplasty was evaluated.  Material and methods:  analytical, observational, case-control, ambispective and consecutive study. 256 patients were included for hip and knee prosthetic surgery between January 2020 and December 2022. Those who underwent surgery in the period prior to the pre-surgical search for S. Aureus were called pre-intervention group (G1) between January 2020 and June of 2021, and those intervened in the search and decolonization called intervention group (G2) between July 2021 to December 2022. In G2, SA carriage and the effectiveness of bacterial decolonization were evaluated. The prevalence of periprosthetic infection due to this germ in both groups was also compared.  Results:  the prevalence of nasal and inguinal colonization for MSSA was 25.8% and that of MRSA was 2.3%. Preoperative decolonization therapy in SA carriers was associated with a reduction in the rate of periprosthetic infections.  Conclusions:  the incidence Staphylococcus aureus carriers in patients scheduled for hip and knee arthroplasty was high. Preoperative detection of S. aureus with targeted decolonization therapy was associated with a reduction in periprosthetic infections mediated.]]></p></abstract>
