De acuerdo con Gattinoni la neumonía por COVID-19 tiene dos fenotipos diferentes, los cuales son distintos en cuanto a su fisiopatología que se puede diferenciar desde su ingreso. El fenotipo 1 se caracteriza por representar 70% de los casos, con una complianza pulmonar mayor de 50 mL/cmH2O. El fenotipo 2 representa 20% de los casos y una complianza menor de 50 mL/cmH2O. Se insiste en que éstos deben ser identificados tan pronto como sea posible con tomografía computarizada, la complianza de igual forma se debe medir en cuanto se conecta al paciente a la ventilación mecánica. Y con esto iniciar acciones terapéuticas y de protección pulmonar. El fenotipo 1, entre otras cosas, se debe manejar con ciertos parámetros como volumen corriente de 6 mL/kg, presión positiva al final de la espiración entre 8-10 mmHg, mientras que el fenotipo 2 se trata de cierta forma como un síndrome de distrés respiratorio agudo severo. En este trabajo tratamos de mostrar de forma secuencial este pensamiento en forma de algoritmo, con la finalidad de que todos los médicos que en algún momento estén en contacto con este tipo de pacientes puedan seguir las directrices aquí mencionadas.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract The first cases of a novel virus, called COVID-19, were documented in Wuhan, on December 2019. According to Gattinoni, two different phenotypes can be found in COVID-19-related pneumonia. Both phenotypes show a different Pathophysiology, which can be detected from the moment the patient is admitted to the hospital. 70% of those cases featured Phenotype 1, where pulmonary compliance was higher than 50 mL/cmH2O, while 20% were presented with Phenotype 2 with a pulmonary compliance lower than 50 mL/cmH2O. The use of a CT scan is paramount for early detection and pulmonary compliance measurements must be performed, as soon as the patient is connected to a mechanical ventilator. These procedures allow physicians involved to start treatment and protect the lungs. Among other parameters, a tidal volume of 6 mL/kg and a PEEP of 8-10 mmHg should be used when dealing with Phenotype 1, whereas Phenotype 2 should be treated to certain point as an acute severe respiratory distress syndrome. This article strives to show the sequential algorithm for this methodology, so that these guidelines are available for all treating Physicians that could encounter this type of patients in the future.]]></p></abstract>
