Desde su descripción en la década de los setenta, ha sido motivo de estudio, ya que su mecanismo fisiopatológico es complejo. Los cambios en el perfil tiroideo parecen ser secundarios al estado agudo de la enfermedad. En todos los casos, existen alteraciones en la actividad de la desionidasa tiroidea, la unión de hormonas tiroideas a proteínas plasmáticas, la transformación de hormonas tiroideas en tejidos periféricos, la actividad del receptor nuclear de hormonas tiroideas y la secreción de la hormona estimuladora de la tiroides (TSH) y de la hormona liberadora de tirotropina (TRH). Se sabe que su presentación y la disminución de T4 a cifras por debajo de 2 &#956;g/dL aumenta hasta en 80% el riesgo de mortalidad en los pacientes en unidades de terapia intensiva. El tratamiento hormonal de sustitución tiroidea con T3 y/o T4 no ha demostrado un beneficio concreto, y su indicación es motivo de polémica. Se realiza la revisión del tema, comprendida por las características del síndrome, su fisiopatología, cuadro clínico, terapéutica y diagnósticos diferenciales.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract: Euthyroid sick syndrome (ESS) is a clinical entity that presents frequently in hospitalized patients, most importantly in critically ill ones. Since it was described, in the 1970s, it has attracted vast interest because it presents itself with a complex physiopathological mechanism. Changes in thyroid function tests (TFT) may be secondary to the acute state of illness. In all cases, we can find alterations in the activity of thyroid deiodinases, union of thyroid hormones to plasmatic proteins and secretion of thyroid stimulating hormone (TSH) and thyrotropin release hormone (TRH). It is known that its presentation, along with the lowering of T4 values to under 2 &#956;g/dL, increases the risk of mortality in up to 80% in intensive care unit (ICU) patients. Hormone substitution with T3, T4, or both has not shown a concrete benefit, its prescription being controversial to this day. This is a systematic review that comprehends the main characteristics of the syndrome, physiopathology, clinical manifestations, treatment and differential diagnosis.]]></p></abstract>
