Estudios epidemiológicos recientes demuestran que la nefrolitiasis se asocia con otras enfermedades, todas ellas factores de riesgo de daño cardiovascular y renal. Esto sugiere que la enfermedad no es sólo una alteración urinaria sino una enfermedad metabólica sistémica y con repercusiones orgánicas. Se resumen las terapias en el manejo del cólico renal. También se hace mención del nomograma de Rule et al, que es un instrumento que permite predecir la recidiva de cálculos. Con dieta, hidratación y los medicamentos con los que ya contamos, el manejo de la nefrolitiasis es exitoso. Es crítico diagnosticar el trastorno metabólico causal de litiasis, sobre todo en enfermos metabólicamente activos. El depósito intersticial renal de CaP, conocido como placa de Randall, ocurre en presencia de hipercalciuria y es un factor promotor de nucleación y agregación de sales de oxalato de calcio. Se discute el manejo de la hiperuricosuria y su control con alopurinol y febuxostat, el papel del citrato en el tratamiento de la litiasis cálcica y por ácido úrico; y finalmente la litiasis y su manejo en la cistinuria.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract: This review show the high incidence and prevalence of nephrolithiasis in the general population. Recent epidemiological studies demonstrate that renal stone formation is associated with other chronic diseases, all of them, risk factors for cardiovascular and renal damage; these results suggests that stone formation is more a systemic disease with organic repercussions. We discuss the medical therapies for the management of stone passage; and also the recently described Rule et al Nomogram, for the prediction of recurrence of stones episodes. The drugs employed for the treatment of this disease are mentioned, as well as the use of diet and increase in fluid intake. Before treatment, it is critical to establish the metabolic abnormalities in active stone formers. The formation of Randall plaque is described in patients with hypercalciuria as the site of deposition of calcium oxalate crystals. The treatment of hyperuricosuria with alopurinol and flebuxostat and the rol of citrate in the control of calcium an uric acid stones is evaluated; as well as the mechanisms and treatment of renal stones in cystinuria.]]></p></abstract>
