<self-uri xlink:href="http://www.scielo.org.mx/scielo.php?script=sci_arttext&amp;pid=S1870-72032016000100025&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://www.scielo.org.mx/scielo.php?script=sci_abstract&amp;pid=S1870-72032016000100025&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://www.scielo.org.mx/scielo.php?script=sci_pdf&amp;pid=S1870-72032016000100025&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="es"><p><![CDATA[Resumen: Se establecen los objetivos de monitorización e intervención terapéutica durante las primeras 24 horas de vida en recién nacidos con peso menor de 1,200 g, los cuales son: diagnosticar bajo flujo sistémico y por órganos mediante detección de antecedentes y datos clínicos como asfixia perinatal, sepsis, sangrado, ventilación mecánica, hipotensión, hipoperfusión tisular, acidosis metabólica y falla renal. Establecer manejo ventilatorio adecuado obteniendo metas como pH 7.25-7.40, PCO2 mayor de 35 hasta 55 mmHg y PO2 entre 30 y 60 mmHg en muestras capilares. Mantenimiento de la perfusión cerebral adecuada mediante la obtención de presión arterial media óptima (15% por encima de la edad gestacional en semanas) a través de medidas como pinzamiento tardío u ordenamiento de cordón umbilical y el uso de fármacos vasoactivos en caso necesario. Monitorización de alteraciones cerebrales tempranas mediante electroencefalograma de amplitud integrada y ultrasonografía Doppler cerebral. La monitorización y conocimiento de límites fisiológicos de las constantes vitales y la estratificación de variaciones en este periodo modifica el curso clínico mediante el reconocimiento y manejo temprano de las alteraciones ventilatorias, hemodinámicas y de perfusión.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract: In newborns weighing less than 1,200 g, the objectives of monitoring and therapeutic intervention are established during the first 24 hours of life, which are: to diagnose low systemic flow and by organs by means of detecting background and clinical data such as perinatal asphyxia, sepsis, bleeding, mechanical ventilation, hypotension, hypoperfusion, metabolic acidosis and renal failure. To obtain adequate ventilatory management reaching goals as pH 7.25-7.40, pCO2 higher than 35 and up to 55 mmHg and PO2 between 30 and 60 mmHg in capillary samples. Maintaining adequate cerebral perfusion by obtaining optimal mean blood pressure (15% above the gestational age in weeks) through measures such as late clamping or arrangement of the umbilical cord and the use of vasoactive drugs if necessary. Monitoring early brain changes by amplitude-integrated electroencephalogram and cerebral Doppler ultrasonography. Monitoring and knowledge of physiological limits of vital signs and the stratification of variations in this period, changes the clinical course by early recognition and management of ventilatory, hemodynamic and perfusion problems.]]></p></abstract>
