<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Respuestas</b></font></p>         <p align="justify"><font face="verdana" size="2">1. La liberaci&oacute;n de mol&eacute;culas terap&eacute;uticas a trav&eacute;s del sistema pulmonar implica una v&iacute;a no invasiva para la administraci&oacute;n de f&aacute;rmacos y agentes bioterap&eacute;uticos para el tratamiento de enfermedades pulmonares (locales) y no pulmonares o sist&eacute;micas.<sup>1</sup></font></p>         <p align="justify"><font face="verdana" size="2">2. La liberaci&oacute;n pulmonar de f&aacute;rmacos ha llamado la atenci&oacute;n como potencial v&iacute;a de administraci&oacute;n de f&aacute;rmacos debido a que presenta ventajas tales como un &aacute;rea superficial amplia (~70&#45;140 m<sup>2</sup>), un epitelio extremadamente delgado (0.1&#45;0.5 &#181;m) y permeable al que se puede acceder f&aacute;cilmente mediante dosis inhaladas;<sup>23</sup> adem&aacute;s de que se ha considerado como una v&iacute;a alternativa a la administraci&oacute;n intravenosa o subcut&aacute;nea.<sup>4</sup></font></p>         <p align="justify"><font face="verdana" size="2">3. La v&iacute;a de liberaci&oacute;n pulmonar funciona a trav&eacute;s de la inhalaci&oacute;n de f&aacute;rmacos o agentes bioterap&eacute;uticos en forma de aerosoles, con su subsecuente deposici&oacute;n en las v&iacute;as a&eacute;reas.<sup>1</sup> Generalmente, los aerosoles consisten en dispersiones o suspensiones estables de f&aacute;rmacos tanto en forma s&oacute;lida como l&iacute;quida. Se ha comprobado que a trav&eacute;s de la aerosolizaci&oacute;n de f&aacute;rmacos se pueden lograr su distribuci&oacute;n y penetraci&oacute;n hasta la regi&oacute;n alveolar.</font></p>         <p align="justify"><font face="verdana" size="2">El origen de la terapia de inhalaci&oacute;n se remonta a 4,000 a&ntilde;os atr&aacute;s, a la medicina Ayuv&eacute;dica, en donde se sabe que la gente utilizaba plantas para tratar el asma y eliminar la tos.<sup>5, 6</sup></font></p>         <p align="justify"><font face="verdana" size="2">4. Los dispositivos m&aacute;s com&uacute;nmente utilizados para la liberaci&oacute;n pulmonar de f&aacute;rmacos incluyen: nebulizadores, inhaladores presurizados (MDI, por sus siglas en ingl&eacute;s) e inhaladores de polvo seco, tambi&eacute;n conocidos como DPI.<sup>2,4,7</sup> Los nebulizadores liberan el f&aacute;rmaco en forma de vapor fino a trav&eacute;s de un tubo o una m&aacute;scara usando aire u ox&iacute;geno presurizado. Los inhaladores presurizados emplean un agente qu&iacute;mico propelente que empuja el f&aacute;rmaco del inhalador. Por otro lado, los inhaladores de polvo seco requieren de la inhalaci&oacute;n profunda y r&aacute;pida del paciente para la liberaci&oacute;n del f&aacute;rmaco.</font></p>     <p align="justify"><font face="verdana" size="2">La elecci&oacute;n del dispositivo adecuado para la liberaci&oacute;n de f&aacute;rmacos depender&aacute; en gran medida del tipo de f&aacute;rmaco, la formulaci&oacute;n, el sitio de acci&oacute;n y la patolog&iacute;a a la que va dirigido.<sup>4,8</sup> Los inhaladores presurizados son originalmente los dispositivo m&aacute;s com&uacute;nmente usados en la terapia de inhalaci&oacute;n,<sup>8</sup> sin embargo; en la actualidad, los inhaladores de polvo seco han ganado mayor popularidad en la liberaci&oacute;n de f&aacute;rmacos y agentes bioterap&eacute;uticos, tales como prote&iacute;nas, p&eacute;ptidos y vacunas.<sup>7</sup></font></p>  	    <p align="center"><font face="verdana" size="2"><a href="/img/revistas/rmcf/v44n3/a11f1.jpg" target="_blank">Figura 1</a></font></p>         <p align="justify"><font face="verdana" size="2">5. La terapia de inhalaci&oacute;n se emplea frecuentemente en enfermedades pulmonares (locales), tales como asma, bronquitis, fibrosis qu&iacute;stica, enfermedad respiratoria obstructiva cr&oacute;nica, neumon&iacute;a, bronquitis cr&oacute;nica, infecci&oacute;n por virus sincitial respiratorio, influenza, c&aacute;ncer pulmonar, entre otras.<sup>3</sup></font></p>         <p align="justify"><font face="verdana" size="2">6. Recientemente se ha demostrado que la liberaci&oacute;n pulmonar de f&aacute;rmacos puede ser una v&iacute;a eficiente para hacer llegar los f&aacute;rmacos hasta circulaci&oacute;n sist&eacute;mica,<sup>9</sup> por lo que en la actualidad existe un gran n&uacute;mero de estudios enfocados en la terapia de inhalaci&oacute;n para el tratamiento de enfermedades sist&eacute;micas; dentro de &eacute;stas se encuentran la hemofilia, la disnea, la terapia contra el dolor, la hepatitis C, la terapia contra tumores, la diabetes, la terapia de reemplazo hormonal, el p&aacute;rkinson, la anafilaxia, la osteoporosis, el SIDA y des&oacute;rdenes neurol&oacute;gicos.<sup>8</sup></font></p>         ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">7. Los f&aacute;rmacos com&uacute;nmente liberados v&iacute;a pulmonar incluyen broncodilatadores tales como nedocromil s&oacute;dico, boclometasona, budesonida, dexametasona, mometasona, fluticasona, albuterol y bromuro de tiotropio;<sup>10,11</sup> antibi&oacute;ticos como la trobamicina, colistimetato de sodio, y el aztreonam; as&iacute; como agentes mucol&iacute;ticos tales como la dornasa alfa;<sup>12</sup> sin embargo, en la actualidad m&uacute;ltiples f&aacute;rmacos y biomol&eacute;culas se encuentras bajo investigaci&oacute;n para lograr un efecto sist&eacute;mico despu&eacute;s de su administraci&oacute;n pulmonar (ver <a href="#t1">Tabla 1</a>).<sup>3,13,14,15,16</sup></font></p>     <p align="center"><font face="verdana" size="2"><a name="t1"></a></font></p>  	    <p align="center"><font face="verdana" size="2"><img src="/img/revistas/rmcf/v44n3/a11t1.jpg"></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Referencias</b></font></p>  	    <!-- ref --><p align="justify"><font face="verdana" size="2">1.
