Diversos países han implementado nuevas estrategias de vacunación con la finalidad de reducir su transmisión y significado clínico. En México, la tos ferina es un problema de salud pública vigente, y su control presenta algunos obstáculos, como la sospecha clínica fuera de la etapa del lactante, la confirmación del diagnóstico, los esquemas de vacunación tardíos o incompletos y la dificultad para limitar su transmisibilidad. La introducción de nuevas estrategias de vacunación en adolescentes y adultos, así como en las mujeres embarazadas, contribuirían al control de la enfermedad y limitarían sus complicaciones.]]></p></abstract>

<body><![CDATA[<p align="justify"><font face="verdana" size="2">Tos ferina es una enfermedad de distribuci&oacute;n mundial<sup>1</sup> que sigue provocando un gran n&uacute;mero de muertes, a pesar de que la vacunaci&oacute;n contra <i>B. pertussis</i> se inici&oacute; hace m&aacute;s de 50 a&ntilde;os. En 2008, la Organizaci&oacute;n Mundial de la Salud estim&oacute; 195,000 defunciones anuales atribuidas a la enfermedad. En relaci&oacute;n al resto de las enfermedades prevenibles por vacunaci&oacute;n, se ubica como la quinta causa de muerte en los menores de 5 a&ntilde;os, solamente precedida por la enfermedad neumoc&oacute;cica, sarampi&oacute;n, rotavirus y las enfermedades invasivas por <i>Haemophilus influenzae</i> tipo B.<sup>2&#45;4</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Es considerada una enfermedad reemergente, ya que su incidencia ha aumentado a nivel mundial, a&uacute;n en pa&iacute;ses con adecuadas coberturas de vacunaci&oacute;n.<sup>5&#45;8</sup> Los avances en el reporte y en el diagn&oacute;stico, la p&eacute;rdida de la inmunidad adquirida por la vacunaci&oacute;n y los cambios en el genoma de <i>B. pertussis</i> son algunos de los factores a los que se atribuye el incremento de los casos. El grupo de los adolescentes ha sido el m&aacute;s afectado, sobre todo en pa&iacute;ses industrializados.<sup>8&#45;11</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>La dificultad para el diagn&oacute;stico</b></font></p>  	    <p align="justify"><font face="verdana" size="2">En M&eacute;xico, la tos ferina y el s&iacute;ndrome coqueluchoide son enfermedades de notificaci&oacute;n inmediata, a trav&eacute;s del sistema de vigilancia y seguimiento epidemiol&oacute;gico de las enfermedades inmunoprevenibles. Este sistema tiene como objetivo primordial detectar y estudiar de forma pasiva los casos probables y su seguimiento hasta establecer el diagn&oacute;stico etiol&oacute;gico. Los pacientes que cumplen con las definiciones operacionales ingresan al sistema a trav&eacute;s de notificaci&oacute;n inmediata (local, municipal, federal) (<a href="/img/revistas/bmim/v69n4/a10c1.jpg" target="_blank">Cuadro 1</a>).<sup>12</sup> El diagn&oacute;stico final de tos ferina se realiza por el cuadro cl&iacute;nico, por laboratorio y asociaci&oacute;n epidemiol&oacute;gica. La vigilancia de la mortalidad se realiza a trav&eacute;s del Sistema Epidemiol&oacute;gico y Estad&iacute;stico de las Defunciones (SEED).<sup>12,13</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La enfermedad obedece a ciclos epid&eacute;micos cada tres a cuatro a&ntilde;os. La incidencia basal promedio (a&ntilde;os no epid&eacute;micos) se reporta entre 1 y 2 casos confirmados por mill&oacute;n de habitantes. Los periodos con mayor incidencia en los &uacute;ltimos 10 a&ntilde;os han sido 2005 (3 casos/mill&oacute;n de habitantes) y 2009 (5 casos/mill&oacute;n de habitantes) (<a href="/img/revistas/bmim/v69n4/a10f1.jpg" target="_blank">Figura 1</a>).<sup>14</sup> La regi&oacute;n m&aacute;s afectada durante el &uacute;ltimo a&ntilde;o epid&eacute;mico (2009) fue la frontera norte; durante este a&ntilde;o se documentaron 1,959 casos sospechosos o s&iacute;ndrome coqueluchoide y se confirmaron 579 casos de tos ferina. Los estados de Nuevo Le&oacute;n, Sonora y Tamaulipas presentaron las tasas de incidencia m&aacute;s altas: 44.5, 42.6 y 16.6/ mill&oacute;n de habitantes, respectivamente. Correspondieron al 62% del total de casos confirmados durante ese a&ntilde;o en todo el pa&iacute;s.<sup>14</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La carga real de la enfermedad est&aacute; subestimada, tanto por la baja sospecha diagn&oacute;stica como por el bajo porcentaje de confirmaci&oacute;n. En nuestro pa&iacute;s, se diagnostican 2 casos de tos ferina por cada 10 casos de s&iacute;ndrome coqueluchoide que se estudian. En los &uacute;ltimos 10 a&ntilde;os, el porcentaje de confirmaci&oacute;n se ha mantenido uniforme (alrededor del 20%) (<a href="/img/revistas/bmim/v69n4/a10f1.jpg" target="_blank">Figura 1</a>). Las dificultades propias del diagn&oacute;stico de laboratorio por cultivo comprenden la toma inadecuada de muestra, la toma de muestra posterior al inicio de antibi&oacute;ticos y la toma de muestra tard&iacute;a (tercera semana del inicio de la tos). A pesar de la heterogeneidad en la vigilancia epidemiol&oacute;gica de tos ferina a nivel mundial, las diferentes definiciones operacionales y los m&eacute;todos de diagn&oacute;stico utilizados,<sup>15</sup> los porcentajes de confirmaci&oacute;n de pa&iacute;ses como Australia,<sup>16</sup> Estados Unidos,<sup>6</sup> Canad&aacute;,<sup>17</sup> Espa&ntilde;a<sup>18</sup> y Suiza<sup>19</sup> se reportan en 90%, 55%, 25.6%, 57.6% y 24.3%, respectivamente.</font></p>  	    <p align="justify"><font face="verdana" size="2">El grupo de edad m&aacute;s afectado es el de los menores de un a&ntilde;o.<sup>14</sup> En 2009, este grupo represent&oacute; el 70.4% del total de los casos reportados. En el caso de los adolescentes y los adultos, la baja sospecha cl&iacute;nica y el subregistro parecen explicar su baja incidencia (<a href="/img/revistas/bmim/v69n4/a10f2.jpg" target="_blank">Figura 2</a>).<sup>14</sup> Existe evidencia de que en M&eacute;xico los adolescentes son un reservorio importante de la enfermedad. La encuesta seroepidemiol&oacute;gica realizada en 1987 muestra que la seroprevalencia de anticuerpos contra <i>B. pertussis</i> disminuye con la edad.<sup>20</sup> Tom&eacute; y colaboradores aislaron <i>B. pertussis</i> en 32.8% de los 61 adolescentes estudiados con tos cr&oacute;nica.<sup>21</sup> Ruiz Palacios y colaboradores encontraron una prevalencia mayor de tos ferina en adolescentes con tos cr&oacute;nica (5.4/1000) que la reportada de forma pasiva por nuestro sistema de vigilancia epidemiol&oacute;gica.<sup>22</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Estrategias de vacunaci&oacute;n en M&eacute;xico</b></font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">En 1954 inici&oacute; la aplicaci&oacute;n de la vacuna contra difteria, tos ferina y t&eacute;tanos (DPT), la cual se realiz&oacute; de forma masiva hasta 1973. En 1999 se introdujo la vacuna pentavalente de c&eacute;lulas completas (DPT+HB+Hib), y desde 2007 se sustituy&oacute; por la vacuna pentavalente acelular (DPaT/VIP+Hib). El esquema actual de vacunaci&oacute;n est&aacute; dirigido &uacute;nicamente a los lactantes y preescolares y comprende la aplicaci&oacute;n de vacuna pentavalente acelular (DPaT/VIP+Hib) a los 2, 4, 6 y 18 meses de edad y refuerzo con DPT a los 4 a&ntilde;os de edad.<sup>23</sup> A partir del 2009, posterior al brote de los estados fronterizos del norte de nuestro pa&iacute;s y por el beneficio de disminuir oportunidades perdidas de vacunaci&oacute;n, el Consejo Nacional de Vacunaci&oacute;n (CONAVA), &oacute;rgano coordinador de las pol&iacute;ticas p&uacute;blicas en materia de vacunaci&oacute;n, aprob&oacute; la aplicaci&oacute;n temporal del esquema acelerado de vacunaci&oacute;n a las 6 semanas, 3, 4 y 18 meses con pentavalente acelular y refuerzo con DPT en la etapa preescolar a los 4 a&ntilde;os de edad, con el objetivo de proteger lo m&aacute;s tempranamente posible a los menores de 6 meses, especialmente en los estados con brotes recientes.</font></p>  	    <p align="justify"><font face="verdana" size="2">Las coberturas de vacunaci&oacute;n contra <i>B. pertussis</i> en el grupo de un a&ntilde;o de edad se han incrementado en nuestro pa&iacute;s a trav&eacute;s de los a&ntilde;os. Seg&uacute;n la Encuesta Nacional de Vacunaci&oacute;n de 1990, la cobertura nacional de vacunaci&oacute;n con DPT3 en el grupo de un a&ntilde;o de edad era del 53%, y hacia 2008 la cobertura estimada se report&oacute; en 96%, lo que representa un incremento del 46% con respecto a 1990.<sup>2425</sup> Desde 1991, la cobertura nacional con dichos inmun&oacute;genos se ha mantenido cercana a 90% al a&ntilde;o de edad;<sup>24,25</sup> sin embargo, la dificultad primordial ha sido la aplicaci&oacute;n tard&iacute;a de la serie de inmun&oacute;genos. Seg&uacute;n la Encuesta Nacional de Cobertura de Vacunaci&oacute;n (ENCOVA) realizada entre los meses de abril y junio de 2010 por el Instituto Nacional de Salud P&uacute;blica, entre ni&ntilde;os menores de 2 a&ntilde;os con cartilla de vacunaci&oacute;n, la cobertura con pentavalente acelularen el grupo de &#8804; 2 a&ntilde;os fue 90.8%; sin embargo, disminuy&oacute; a 70% en el grupo de &lt; 1 a&ntilde;o y hasta 65% en las cohortes de 2 y 4 meses,<sup>26</sup> lo que representa una vacunaci&oacute;n retrasada. La efectividad de la vacuna (EV) contra <i>B. pertussis</i> aumenta paulatinamente conforme se completa la serie inicial de inmun&oacute;genos en los lactantes. Bisgard y colaboradores<sup>27</sup> reportaron una EV para prevenir la enfermedad apenas del 50.5% (IC95% 71.1&#45;86.3) tras la primera dosis de vacuna, que aumenta al 80% (IC95% 41.3&#45;93.2), 93.3% (IC95% 83.1&#45;97.4) y 95.1% (IC95% 85.1&#45;98.4) con dos, tres y cuatro dosis, respectivamente (<a href="/img/revistas/bmim/v69n4/a10f3.jpg" target="_blank">Figura 3</a>). De igual forma, Juretzo y colaboradores<sup>28</sup> describieron una EV para prevenir hospitalizaciones por la enfermedad del 68.7% (IC95% 46.3&#45;81.7), 91.8%(IC 95% 84.7&#45;95.7) y 99.8%(IC95% 98.3&#45;100), posterior al esquema con una, dos y tres dosis, respectivamente. En este sentido, los lactantes menores de 3 meses vacunados oportunamente con una dosis de biol&oacute;gico (efectividad de la vacuna del 50.5%), aquellos con esquemas tard&iacute;os o incompletos (vacunados tard&iacute;amente) y los menores de 6 semanas que no son candidatos a iniciar la vacunaci&oacute;n (program&aacute;ticamente no prevenibles) son la poblaci&oacute;n m&aacute;s vulnerable a adquirir la enfermedad, sus complicaciones y morir.<sup>22,23</sup> Durante el 2009, se confirmaron 32 defunciones por tos ferina y se asociaron 44 (s&iacute;ndrome coqueluchoide). El total de defunciones se present&oacute; en menores de un a&ntilde;o de edad y el 85% se concentr&oacute; en los menores de 3 meses.</font></p>  	    <p align="justify"><font face="verdana" size="2">La duraci&oacute;n de la inmunidad posterior al esquema completo contra <i>B. pertussis</i> disminuye paulatinamente a trav&eacute;s de los a&ntilde;os; se ha descrito que dura entre 4 a 12 a&ntilde;os,<sup>29&#45;36</sup> sin haber aparentemente diferencias significativas entre la vacuna de c&eacute;lulas completas y la acelular.<sup>31</sup> Te&oacute;ricamente, en M&eacute;xico, la vacunaci&oacute;n ha permitido mantener inmunes a las cohortes de lactantes, preescolares y escolares mediante la inmunizaci&oacute;n sistem&aacute;tica desde 1973. Sin embargo, los adolescentes y adultos que pierden la inmunidad son portadores, contraen la enfermedad<sup>4,11</sup> o son transmisores de la enfermedad a los lactantes,<sup>37,38</sup> adem&aacute;s de que no se sospecha ni diagnostica en ellos la enfermedad. En el estudio de contactos de lactantes enfermos menores de 6 meses se ha logrado identificar a los miembros de la familia como la fuente de transmisi&oacute;n de 43 a 80% de los casos.<sup>3842</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Estrategias actuales de Prevenci&oacute;n.</b></font></p>  	    <p align="justify"><font face="verdana" size="2">En el a&ntilde;o 2005, el biol&oacute;gico con toxoide tet&aacute;nico, dift&eacute;rico y pertussis acelular (Tdap) fue regulado en los Estados Unidos para su uso en adolescentes y adultos; la composici&oacute;n es similar a la vacuna DTaP pedi&aacute;trica, pero con una menor concentraci&oacute;n de ant&iacute;genos.<sup>43</sup> Ward y colaboradores la describieron como una vacuna segura con una eficacia del 92% (IC 95% 32&#45;99%).<sup>44</sup> El seguimiento a 5 a&ntilde;os en adolescentes posterior al refuerzo con Tdap ha mostrado una respuesta adecuada, tanto en la inmunidad humoral como en la mediada por c&eacute;lulas.<sup>45</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Tanto la Iniciativa Global de Pertussis,<sup>15,46</sup> como el Comit&eacute; Asesor en Vacunas de los Estados Unidos de Am&eacute;rica (ACIP, por sus siglas en ingl&eacute;s)<sup>47</sup> han recomendado un refuerzo con Tdap en adolescentes y adultos, as&iacute; como la vacunaci&oacute;n en el personal de Salud y mujeres embarazadas, sustentada en la p&eacute;rdida de la inmunidad adquirida mediante la vacunaci&oacute;n espec&iacute;fica en estos grupos de edad, el incremento en su incidencia,<sup>10&#45;12,31</sup> su papel en la transmisi&oacute;n de la enfermedad<sup>37,38</sup> y como medida potencial para reducir la morbimortalidad, principalmente en lactantes.<sup>48</sup> Pa&iacute;ses como Estados Unidos, Canad&aacute;, Australia, Francia, Costa Rica, Argentina han reconocido e implementado estas nuevas estrategias de vacunaci&oacute;n a sus esquemas.<sup>8,49</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La introducci&oacute;n de Tdap como refuerzo en la poblaci&oacute;n adolescente ha demostrado ser eficaz para reducir la incidencia de tos ferina en este grupo de edad. Kandola y colaboradores evaluaron el impacto de la introducci&oacute;n de Tdap en adolescentes de 14 a 16 a&ntilde;os, encontrando una incidencia de tos ferina antes de la introducci&oacute;n de la vacuna de 18/100,000 habitantes comparada con 0.2/100,000 habitantes posterior a su introducci&oacute;n.<sup>50</sup> Quinn y colaboradores demostraron el impacto de la vacunaci&oacute;n con Tdap en poblaci&oacute;n adolescente en la reducci&oacute;n de los casos notificados de tos ferina en el grupo de los adolescentes y en los menores de 6 meses de edad.<sup>16</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Por su parte, la vacunaci&oacute;n en el embarazo est&aacute; orientada a brindar protecci&oacute;n espec&iacute;fica a los lactantes susceptibles antes del inicio del esquema de vacunaci&oacute;n habitual,<sup>51</sup> con la aplicaci&oacute;n de biol&oacute;gicos a las mujeres embarazadas, consideradas en su mayor&iacute;a poblaci&oacute;n cautiva en la consulta prenatal. Las encuesta serol&oacute;gicas reportan que solo 14&#45;34% de las mujeres embarazadas tienen anticuerpos contra <i>B. pertussis.<sup>51,51</sup></i> Diversos estudios han fundamentado el paso de anticuerpos maternos tipo IgG a trav&eacute;s de la placenta,<sup>53&#45;55</sup> as&iacute; como el incremento de anticuerpos espec&iacute;ficos en sangre de cord&oacute;n de madres vacunadas con respecto a las no vacunadas.<sup>53,54</sup> La vida media de los anticuerpos maternos ha sido estimada en 6 semanas<sup>55</sup> y estos no parecen interferir con la respuesta inmunol&oacute;gica a la serie primaria de vacunaci&oacute;n contra tos ferina cuando se administra vacuna acelular a los lactantes.<sup>55</sup> La efectividad de la vacunaci&oacute;n con Tdap en el embarazo para prevenir la enfermedad en los lactantes no ha sido bien establecida; sin embargo, los anticuerpos maternos contra <i>B. pertussis</i> confieren protecci&oacute;n y modifican la severidad de la enfermedad en los lactantes<sup>56,57</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En el Centro Nacional para la Salud de la Infancia y la Adolescencia (CeNSIA) reconocemos la importancia de que las Entidades Federativas mantengan por arriba de 95% el porcentaje de cobertura con pentavalente acelular, as&iacute; como garantizar la vacunaci&oacute;n oportuna de las primeras dosis de biol&oacute;gicos a la poblaci&oacute;n menor de 6 meses mediante el esquema acelerado de vacunaci&oacute;n. Con la finalidad de fortalecer la campa&ntilde;a permanente de vacunaci&oacute;n e invitar a la poblaci&oacute;n a que acuda oportunamente a la unidad de salud m&aacute;s cercana a demandar la vacunaci&oacute;n, a partir de 2010 el CeNSIA inici&oacute; una importante campa&ntilde;a de comunicaci&oacute;n social masiva en forma original, interactiva y eficiente a trav&eacute;s de <i>marketing</i> m&oacute;vil, vinculaci&oacute;n con el sector privado y organizaciones no gubernamentales. Gracias a los productores de pa&ntilde;ales se distribuyeron m&aacute;s de 36 millones de pa&ntilde;ales con el logo de "Vac&uacute;name a tiempo", se produjeron 1 mill&oacute;n de collares para adherirse a los productos para beb&eacute;, se capacitaron m&aacute;s de 400 hospitales con un alcance de m&aacute;s de 7,000 enfermeras, as&iacute; como difusi&oacute;n a trav&eacute;s de folletos y posters en hospitales. Sin embargo, es evidente que la vacunaci&oacute;n eficaz a los lactantes y preescolares no ser&aacute; una medida suficiente para controlar la enfermedad. Es necesario implementar nuevas estrategias de vacunaci&oacute;n en la poblaci&oacute;n adolescente y adulta, como estrategia para reducir la transmisi&oacute;n a los menores de 6 meses y la morbimortalidad por la enfermedad en este grupo de edad.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">En M&eacute;xico, tos ferina es a&uacute;n un problema importante de salud p&uacute;blica, el cual se presenta como en otros pa&iacute;ses, a pesar de contar con buenas coberturas de vacunaci&oacute;n. Los principales obst&aacute;culos para su control han sido la subestimaci&oacute;n de la carga real de la enfermedad, el bajo porcentaje de confirmaci&oacute;n, la pobre sospecha diagn&oacute;stica en adolescentes y adultos, la baja efectividad del ant&iacute;geno, los esquemas de vacunaci&oacute;n tard&iacute;os o incompletos y la p&eacute;rdida de la inmunidad adquirida por la vacunaci&oacute;n a trav&eacute;s de los a&ntilde;os. En la actualidad, siguen existiendo en nuestro pa&iacute;s brotes importantes, y su principal trascendencia radica en el impacto a los menores de un a&ntilde;o, que corren el riesgo de morir o de quedar con secuelas neurol&oacute;gicas.</font></p>  	    <p align="justify"><font face="verdana" size="2">Las estrategias para control de la enfermedad deben estar encaminadas a mejorar los m&eacute;todos diagn&oacute;sticos, as&iacute; como sensibilizar a los m&eacute;dicos cl&iacute;nicos de su diagn&oacute;stico en lactantes, adolescentes y adultos. Mantener las coberturas de vacunaci&oacute;n por arriba del 95% en poblaci&oacute;n menor de 5 a&ntilde;os mantendr&aacute; inmunes a los escolares y evitar&aacute; la transmisi&oacute;n por este grupo de edad. La difusi&oacute;n y sensibilizaci&oacute;n de los padres para que acudan a las unidades de salud a demandar los biol&oacute;gicos desde las 6 semanas de vida ser&aacute; una pieza fundamental para el control de la enfermedad.</font></p>  	    <p align="justify"><font face="verdana" size="2">Sin embargo, adem&aacute;s de utilizar el esquema acelerado para vacunaci&oacute;n temprana, es necesario proteger a los lactantes menores de 6 meses, al reducir la fuente de contagio de <i>B. pertussis,</i> desde un familiar cercano portador o enfermo.
