Existe una correlación positiva entre el índice de masa corporal y el asma; el riesgo de padecer asma aumenta con el incremento de la masa corporal. La probabilidad de desarrollar asma de un escolar obeso puede ser hasta de 50%. La asociación entre la obesidad y el asma se ha descrito con más frecuencia en mujeres, particularmente en estudios de adultos. La obesidad puede afectar directamente el fenotipo del asma por efectos mecánicos en la vía aérea, por reflujo gastroesofágico, por la producción de citocinas proinflamatorias en el tejido adiposo (interleucina 6, factor de necrosis tumoral, leptina, adiponectina), por la activación de genes comunes o por el aumento en la producción de estrógenos. La obesidad puede agravar los síntomas del asma y ser causante de su pobre control; la disminución del peso mejora los síntomas y la función pulmonar y reduce el uso de medicamentos antiasmáticos. There is a positive correlation between body mass index and asthma; the risk of suffering from asthma is greater as body mass increases. The probability for developing asthma in an obese school-age child may be as high as 50%. An association between obesity and asthma has been described more frequently in females, particularly in adult surveys. Obesity may directly affect asthma phenotype by mechanical effects in the respiratory tract, gastroesophageal reflux, production of proinflammatory cytokines in fat tissue (interleukin 6, tumor necrosis factor, leptin, adiponectin), activation of common genes, or by increased estrogen production. Obesity may worsen asthma symptoms as well as causing poor control of the condition. Weight loss improves symptomatology and pulmonary function, along with reducing the use of antiasthmatic medication. Therefore, it is necessary that management of obese asthmatic patients includes a weight control program.</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Keywords:</b> obesity, asthma, body mass index, phenotype.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2">En las &uacute;ltimas d&eacute;cadas los casos de asma y obesidad han aumentado notablemente en diversos pa&iacute;ses.<sup>1,2</sup> Esta situaci&oacute;n representa un problema de salud p&uacute;blica por la probabilidad de una muerte temprana en un gran n&uacute;mero de individuos.<sup>3,4</sup> La Organizaci&oacute;n Mundial de la Salud (OMS) incluye a ambos padecimientos dentro de las principales enfermedades cr&oacute;nicas.<sup>5,6</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Obesidad</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La obesidad es el exceso de grasa acumulada que afecta a la salud.<sup>7</sup> La causa fundamental es un desequilibrio entre el ingreso y el gasto de energ&iacute;a. Est&aacute; estrechamente ligada a un estilo de vida occidental donde hay una disminuci&oacute;n de la actividad f&iacute;sica y una inadecuada alimentaci&oacute;n.<sup>8</sup> De acuerdo a la Encuesta Nacional de Salud y Nutrici&oacute;n 2006 (ENSANUT) con los criterios de la <i>Internacional Obesity Task Force</i> (IOTF)<sup>9</sup> en el grupo de 12 a 18 a&ntilde;os de edad, los varones presentaron una prevalencia de sobrepeso de 21.2% y de obesidad de 10.0% y las mujeres de 23.3% y 9.2%, respectivamente.<sup>4</sup></font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">En general, de acuerdo a la ENSANUT 2006, 70% de los adultos en M&eacute;xico padecen sobrepeso y obesidad, mientras que en los adolescentes del &aacute;rea metropolitana, la obesidad y el sobrepeso est&aacute;n presentes en 28% de los varones y 30% de las mujeres de 10 a 17 a&ntilde;os.<sup>4</sup> Si bien no hubo un cambio significativo para este &uacute;ltimo grupo etario con respecto al reporte de la ENSANUT 2000,<sup>10</sup> la prevalencia es alta y esto preocupa ya que la obesidad y el sobrepeso est&aacute;n relacionados con el desarrollo de alteraciones pulmonares, ortop&eacute;dicas, gastroenterol&oacute;gicas, neurol&oacute;gicas, la resistencia a la insulina, dislipidemia, hipertensi&oacute;n y diabetes tipo 2 (<a href="/img/revistas/bmim/v68n3/a2f1.jpg" target="_blank">Figura 1</a>).<sup>11&#45;13</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Para establecer el diagn&oacute;stico del sobrepeso y la obesidad en la pr&aacute;ctica cl&iacute;nica uno de los &iacute;ndices m&aacute;s accesibles y pr&aacute;cticos, para estimar el exceso de grasa, es el &Iacute;ndice de Masa Corporal (IMC), que es el valor del peso (en kilogramos) dividido entre el cuadrado del valor de la talla (en metros).</font></p>  	    <p align="justify"><font face="verdana" size="2">El comit&eacute; de expertos para la evaluaci&oacute;n, la prevenci&oacute;n y el tratamiento del ni&ntilde;o y del adolescente con sobrepeso y obesidad recomienda aplicar el IMC en ni&ntilde;os de 2 a 18 a&ntilde;os de edad.<sup>7</sup> El sobrepeso se define cuando el valor del IMC es &#8805; al percentil 85 y la obesidad cuando es &#8805; al percentil 95 para la edad y el sexo (en base a las tablas percentiladas de los CDC),<sup>14</sup> mientras que la obesidad m&oacute;rbida se establece cuando el IMC es &#8805; al percentil 99 o, en adolescentes, cuando hay un IMC &#8805; 35 kg/m<sup>2</sup>. Este valor de corte se defini&oacute; con base en la cohorte de Bogalusa, donde se identificaron a los sujetos con muy alto riesgo de tener mayores anormalidades bioqu&iacute;micas asociadas al desarrollo temprano de la diabetes, de la enfermedad cardiovascular y a la obesidad grave del adulto.<sup>12,14</sup> En esta &uacute;ltima se han observado, de forma m&aacute;s temprana, manifestaciones de deslizamiento de la cabeza femoral (10%), esteatosis hep&aacute;tica (30%), s&iacute;ndrome metab&oacute;lico (35%), trastornos respiratorios del sue&ntilde;o (70%) y alta frecuencia de resistencia a la insulina (65%).<sup>15,16</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Asma</b></font></p>  	    <p align="justify"><font face="verdana" size="2">El asma es un desorden inflamatorio cr&oacute;nico de las v&iacute;as a&eacute;reas con la participaci&oacute;n de muchas c&eacute;lulas y elementos de ellas (c&eacute;lulas cebadas, eosin&oacute;filos, neutr&oacute;filos, linfocitos T, macr&oacute;fagos y c&eacute;lulas epiteliales) que originan episodios recurrentes de tos de predominio nocturno, sibilancias, dificultad respiratoria y sensaci&oacute;n de opresi&oacute;n tor&aacute;cica. Estos s&iacute;ntomas se asocian generalmente con una extensa pero variable obstrucci&oacute;n bronquial, que es a menudo reversible espont&aacute;neamente o con el tratamiento.<sup>17</sup> Con relaci&oacute;n al asma en la edad pedi&aacute;trica la prevalencia var&iacute;a ampliamente a escala mundial; incluso en nuestro pa&iacute;s, por ejemplo en M&eacute;rida, Tabasco y Ciudad Victoria, se presenta en un alto porcentaje de escolares (12%), mientras que en el norte del Distrito Federal se presenta en 9.9% de los adolescentes y en 8.5% de los escolares.<sup>18,19</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Nuestro objetivo en esta revisi&oacute;n es mostrar las caracter&iacute;sticas principales de la obesidad y el asma y la relaci&oacute;n causal entre ellas.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Relaci&oacute;n entre el asma y la obesidad</b></font></p>  	    <p align="justify"><font face="verdana" size="2">El incremento paralelo en la prevalencia del asma y la obesidad en diversas regiones ha dado origen al postulado de que ambas entidades tienen una relaci&oacute;n causal.<sup>20&#45;22</sup> Aunque esta relaci&oacute;n no es del todo clara, probablemente por lo complejo de esta epidemia, ambos padecimientos tienen en com&uacute;n el proceso inflamatorio cr&oacute;nico.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Se ha descrito que la obesidad incrementa la incidencia del asma en adultos.<sup>23&#45;38</sup> Sin embargo, existe una gran heterogeneidad en la magnitud e importancia de esta relaci&oacute;n ya que hay diferencias en la duraci&oacute;n del seguimiento, en el n&uacute;mero de pacientes, en la distribuci&oacute;n por sexos y edades y en el IMC, adem&aacute;s de otras variables que podr&iacute;an explicar las diferencias en las estimaciones puntuales.<sup>28,39</sup> Por ejemplo, la obesidad en las mujeres pospuberales antecede al asma y se ha demostrado que la relaci&oacute;n entre la obesidad y el asma es significativa; sin embargo, en algunos estudios se ha encontrado que esta relaci&oacute;n es similar para ambos sexos<sup>40,41</sup> y en otros, por el contrario, que esta situaci&oacute;n s&oacute;lo se presenta en varones.<sup>42</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En varios estudios transversales se ha encontrado un incremento en la prevalencia del asma en pacientes obesos<sup>22</sup> aunque, para detectar mejor los efectos de la obesidad sobre el desarrollo del asma, los estudios prospectivos han sido de mayor valor.<sup>43</sup> A partir del a&ntilde;o 2000, con un mejor dise&ntilde;o de los estudios, se ha demostrado un riesgo mayor en los obesos, que va de 1.1 a 3 veces, para desarrollar asma.<sup>24,40,44,45</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Despu&eacute;s de encontrar 2006 referencias en la literatura, Beuther y Sutherland realizaron el meta&#45;an&aacute;lisis de tan s&oacute;lo siete estudios que cumplieron con los criterios de inclusi&oacute;n. Fueron evaluados adultos eutr&oacute;ficos con sobrepeso y obesidad para el desarrollo de asma (n = 333,102). Cuando se compar&oacute; el IMC entre estos, el riesgo de asma (OR) fue de 1.5 (IC 95% 1.27&#45;1.80) para los de mayor IMC (IMC &gt; 25) con un efecto dosis respuesta de IMC en la incidencia de asma, tanto en hombres como en mujeres.<sup>46</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Nosotros encontramos en trabajadores del Hospital General de M&eacute;xico una relaci&oacute;n significativa de s&iacute;ntomas de asma en mujeres con un IMC &#8805; 85% y una cintura mayor de 80 cm.<sup>31</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Estudios en la poblaci&oacute;n pedi&aacute;trica</b></font></p>  	    <p align="justify"><font face="verdana" size="2">Los estudios que se han llevado a cabo en la poblaci&oacute;n pedi&aacute;trica son m&aacute;s heterog&eacute;neos tanto en t&eacute;rminos de fuerza de sus resultados como en la direcci&oacute;n de la relaci&oacute;n asma&#45;obesidad. Gold y cols., en 9828 ni&ntilde;os entre 6 y 14 a&ntilde;os de edad con un seguimiento durante cinco a&ntilde;os, reportaron un riesgo 2.2 veces mayor para asma, sobre todo en ni&ntilde;as, con exceso de peso.<sup>47</sup> En otro estudio, en 3792 ni&ntilde;os, se encontr&oacute; que el sobrepeso y la obesidad incrementaban el riesgo de asma, m&aacute;s en ni&ntilde;os que en ni&ntilde;as.<sup>48</sup> Mannino y cols. realizaron el seguimiento de 4393 ni&ntilde;os sin asma durante 14 a&ntilde;os. En sus primeros 2 a&ntilde;os de vida mostraron que el grupo con IMC elevados (arriba del percentil 85) tuvo un riesgo 2.4 veces mayor de desarrollar asma que el grupo que ten&iacute;a IMC m&aacute;s bajos.<sup>49</sup> En el metaan&aacute;lisis de Flaherman y Rutherford, donde se analizaron 12 estudios,<sup>50</sup> se observ&oacute; que en cuatro de ellos hubo un riesgo 4 veces mayor para asma en los escolares con obesidad.<sup>51,52</sup> El efecto del peso al nacimiento y el desarrollo de asma en nueve de los 12 estudios fue 1.2 mayor cuando el peso era &gt; 3,800 g,<sup>50</sup> aunque otros autores han demostrado que no s&oacute;lo el peso alto al nacer (&gt; 3500 g) es un factor de riesgo, sino tambi&eacute;n un peso &lt; 2,500 g.<sup>53</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En el <a href="/img/revistas/bmim/v68n3/a2c1.jpg" target="_blank">cuadro I</a> se resumen dos metaan&aacute;lisis, uno de ni&ntilde;os y otro de adultos, con riesgo de 1.5 e IC 95% de 1.2 a 1.8.<sup>46,50</sup> Contrario a lo descrito hasta aqu&iacute;, no se detect&oacute; ning&uacute;n efecto en un n&uacute;mero menor de estudios.<sup>51,52</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La hip&oacute;tesis de Godfrey y Barker plantea que hay una programaci&oacute;n fetal que puede originar el subsecuente desarrollo de enfermedades cr&oacute;nicas como obesidad y asma.<sup>54</sup> El resultado programado de un est&iacute;mulo da&ntilde;ino durante un periodo cr&iacute;tico (sensible) en el desarrollo fetal temprano puede verse reflejado en la etapa adulta.<sup>55</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En los obesos se ha observado una relaci&oacute;n del fenotipo de asma no al&eacute;rgico con s&iacute;ntomas m&aacute;s intensos: el mayor uso de medicamentos antiasm&aacute;ticos y la mala respuesta a antiinflamatorios esteroides inhalados.<sup>56</sup> Otra forma de evaluar la relaci&oacute;n entre el asma y la obesidad es el efecto ben&eacute;fico que tiene la disminuci&oacute;n del peso en la reducci&oacute;n de los s&iacute;ntomas, en el uso de medicamentos y en el n&uacute;mero de visitas a urgencias por crisis.<sup>57,58</sup> Adem&aacute;s de lo anterior, los pacientes obesos presentan una combinaci&oacute;n de efectos mec&aacute;nicos e inflamatorios que pueden producir discapacidad pulmonar.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Las pruebas de funci&oacute;n pulmonar m&aacute;s empleadas en la cl&iacute;nica son la espirometr&iacute;a forzada y la pletismograf&iacute;a. Para la primera, se requiere de un espir&oacute;metro que muestra de manera num&eacute;rica y gr&aacute;fica una curva de flujo/volumen y de volumen/tiempo con el aire espirado e inspirado, la cual determina el volumen y la velocidad del aire que se moviliza desde los pulmones hacia el exterior durante una maniobra de espiraci&oacute;n forzada. Con esta prueba se calculan los siguientes datos: la capacidad vital forzada (FVC), el volumen espiratorio forzado en el primer segundo (FEV1), el flujo espiratorio forzado obtenido de 25&#45;75% de la capacidad vital exhalada (FEF 25&#45;75%) y la relaci&oacute;n FEV1/%FVC (&iacute;ndice de Tiffeneau). Existen principalmente dos patrones espirom&eacute;tricos, el restrictivo y el obstructivo. En el restrictivo existe una limitaci&oacute;n de la expansi&oacute;n del pulm&oacute;n que puede ser originada por alteraciones del par&eacute;nquima, de la pleura, de la pared tor&aacute;cica o del aparato neuromuscular. En el obstructivo hay aumento de la resistencia al flujo de aire y est&aacute;n reducidos el FEV1 y la relaci&oacute;n FEV1/%FVC.</font></p>  	    <p align="justify"><font face="verdana" size="2">La pletismograf&iacute;a es un procedimiento m&aacute;s sofisticado y costoso. La t&eacute;cnica se basa en las leyes de los gases. Adem&aacute;s de determinar los vol&uacute;menes y flujos (igual que la espirometr&iacute;a), mide las capacidades pulmonares. De &eacute;stas, las de mayor utilidad cl&iacute;nica son la capacidad pulmonar total (TLC), la capacidad residual funcional (FRC) y el volumen residual (RV).</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Principales alteraciones en la funci&oacute;n pulmonar por la obesidad</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La obesidad puede causar s&iacute;ntomas respiratorios como: disnea al ejercicio, aumento del esfuerzo respiratorio y alteraciones en la capacidad pulmonar; estos podr&iacute;an ser interpretados como s&iacute;ntomas sugestivos de asma (sibi&#45;lancias) sin ser asma, por lo que es necesario, mediante pruebas de reto pulmonar, confirmar si existen alteraciones de la reactividad bronquial sumadas a los s&iacute;ntomas respiratorios cr&oacute;nicos de tos, sibilancias y opresi&oacute;n tor&aacute;cica.<sup>59</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En la obesidad se presenta el endurecimiento de las v&iacute;as a&eacute;reas debido a una combinaci&oacute;n de los efectos sobre los pulmones y el trabajo de la pared tor&aacute;cica para respirar. La distendibilidad pulmonar est&aacute; disminuida y parece ser exponencial con relaci&oacute;n al IMC.<sup>60</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Tambi&eacute;n puede presentarse un mayor volumen sangu&iacute;neo pulmonar que, junto con la alteraci&oacute;n del cierre de la v&iacute;a a&eacute;rea por los vol&uacute;menes disminuidos, ocasione peque&ntilde;as &aacute;reas de atelectasia o de aumento de la tensi&oacute;n superficial alveolar debido a la reducci&oacute;n de la FRC.<sup>60</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La gran cantidad de trabajo que cuesta mover un cuerpo obeso, m&aacute;s la disminuci&oacute;n de la distendibilidad o <i>compliance</i> de la pared tor&aacute;cica por la infiltraci&oacute;n de grasa en los m&uacute;sculos accesorios de la respiraci&oacute;n, llevan a una sensaci&oacute;n subjetiva de disnea. Esta "batalla para respirar" puede ocasionar debilidad en los m&uacute;sculos respiratorios y disminuir la presi&oacute;n inspiratoria m&aacute;xima a diferencia de sujetos no obesos. Por estas razones no es dif&iacute;cil entender que, con m&uacute;sculos d&eacute;biles, pobre <i>compliance</i> o distendibilidad de la pared tor&aacute;cica y una masa corporal grande, la tolerancia al ejercicio sea pobre.<sup>61</sup> Adem&aacute;s, la adiposidad central incrementa la presi&oacute;n intraabdominal, que desplaza al diafragma. Esto origina un s&iacute;ndrome compartimental abdominal cr&oacute;nico que da lugar a una disminuci&oacute;n en los vol&uacute;menes y en las din&aacute;micas pulmonares.<sup>60</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">De acuerdo con el grado de adiposidad puede o no haber alteraciones de la funci&oacute;n pulmonar; el patr&oacute;n respiratorio que predomina en la obesidad es el restrictivo, pero puede incluso ser mixto (restrictivo y obstructivo).</font></p>  	    <p align="justify"><font face="verdana" size="2">El tejido adiposo alrededor de la caja tor&aacute;cica y del abdomen (grasa visceral) origina una carga sobre la pared tor&aacute;cica y reduce la FRC. Esta reducci&oacute;n y la del volumen de reserva espiratorio (ERV) son detectables incluso con un aumento modesto de peso.<sup>60</sup></font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">En cuanto a la intensidad de la alteraci&oacute;n del calibre de la v&iacute;a a&eacute;rea se ha visto una discreta disminuci&oacute;n de los vol&uacute;menes pulmonares; sin embargo, rara vez se encuentran por debajo de los par&aacute;metros normales, incluso en los pacientes extremadamente obesos.<sup>60</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Una baja FRC aumenta el riesgo de limitaci&oacute;n del flujo espiratorio y del cierre de las v&iacute;as a&eacute;reas. La marcada disminuci&oacute;n del ERV puede conducir a alteraciones en la ventilaci&oacute;n, la distribuci&oacute;n, el cierre de las v&iacute;as a&eacute;reas en las zonas dependientes del pulm&oacute;n y originar diferencias en la ventilaci&oacute;n perfusi&oacute;n que traen un cambio en la balanza de las presiones de inflado y desinflado del pulm&oacute;n (<a href="/img/revistas/bmim/v68n3/a2f2.jpg" target="_blank">Figura 2</a>).<sup>60</sup> Existe una relaci&oacute;n exponencial entre el IMC y la FRC con una reducci&oacute;n de &eacute;sta, incluso en personas con sobrepeso, la cual puede llegar a ser tan marcada que sea igual al volumen residual.<sup>62</sup> Sin embargo, los efectos de la obesidad en los vol&uacute;menes pulmonares, en la TLC y en el RV pueden ser modestos.<sup>60</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Se ha reportado una asociaci&oacute;n entre el exceso de peso y la disminuci&oacute;n de la TLC, aunque los cambios son peque&ntilde;os; por lo general se mantienen por encima del l&iacute;mite inferior de la normalidad, incluso en la obesidad grave.<sup>62</sup> El RV est&aacute; generalmente bien conservado y la relaci&oacute;n RV&#45;TLC se mantiene normal o ligeramente mayor.<sup>56,62,</sup><sup>63</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La reducci&oacute;n de la TLC probablemente se deba al efecto mec&aacute;nico de la grasa, que reduce el descenso del movimiento del diafragma por aumento de la masa abdominal y limita el margen de pulm&oacute;n para expandirse durante el inflado. Todo esto se corrige cuando hay p&eacute;rdida de peso.<sup>60,64</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Cuando se presenta una ligera reducci&oacute;n de la TLC y un buen estado de conservaci&oacute;n del RV, la reducci&oacute;n de la FRC se manifiesta por un aumento de la capacidad inspiratoria y una disminuci&oacute;n muy marcada del ERV.<sup>62</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Con respecto a los vol&uacute;menes pulmonares, el FEV1 y la FVC pueden verse o no afectados; si se alteran, la afectaci&oacute;n es muy leve en adultos y ni&ntilde;os<sup>59,65</sup> y la relaci&oacute;n FEV1/%FVC est&aacute; generalmente bien conservada o, incluso, aumentada.<sup>65&#45;67</sup> Sin embargo, hoy en d&iacute;a hay controversias que han servido para sustentar que en ni&ntilde;os con obesidad s&iacute; puede haber un patr&oacute;n obstructivo con una disminuci&oacute;n del FEV1/%FVC;<sup>68</sup> esto llama la atenci&oacute;n ya que es un hallazgo frecuente en el asma.<sup>69</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">A pesar de las consideraciones sobre la modesta reducci&oacute;n en los vol&uacute;menes pulmonares, en caso de haber una reducci&oacute;n del di&aacute;metro de la v&iacute;a a&eacute;rea perif&eacute;rica por infiltraci&oacute;n grasa, habr&aacute; cambios en la funci&oacute;n del m&uacute;sculo liso bronquial con la incoordinaci&oacute;n subsiguiente en el ciclo de entrecruzamiento de actina y miosina, que potencialmente incrementa la obstrucci&oacute;n y altera la respuesta ante un est&iacute;mulo como el ejercicio o el reto bronquial, claves en el diagn&oacute;stico de la hiperreactividad bronquial (HRB) (<a href="/img/revistas/bmim/v68n3/a2f2.jpg" target="_blank">Figura 2</a>).<sup>70</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Entre los estudios en favor del desarrollo de HRB en obesos se encuentra el de Kaplan y Montana, quienes corroboraron HRB al ejercicio en ni&ntilde;os obesos <i>vs.</i> ni&ntilde;os sanos.<sup>71</sup> De igual forma, Gennuso y cols. describen una mayor frecuencia de HRB en asm&aacute;ticos obesos <i>vs.</i> asm&aacute;ticos sin obesidad.<sup>72</sup> En la Encuesta de Salud Respiratoria de la Comunidad Europea se observ&oacute; un incremento en la HRB en hombres obesos<sup>73</sup> y en un estudio longitudinal <i>(Normative Aging Study)</i> hubo tambi&eacute;n una asociaci&oacute;n entre el IMC y la HRB conunaraz&oacute;nde momios (OR) reportada de 7.<sup>74</sup>En nuestra experiencia, en un grupo de ni&ntilde;os obesos sometidos a una prueba de reto al ejercicio, observamos patrones de HRB similares a los ni&ntilde;os asm&aacute;ticos eutr&oacute;ficos y una ca&iacute;da mayor del FEV1 en ni&ntilde;os asm&aacute;ticos con sobrepeso y obesos <i>vs.</i> los asm&aacute;ticos no obesos.<sup>75</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La reducci&oacute;n en los vol&uacute;menes pulmonares con restricci&oacute;n de la pared tor&aacute;cica y el aumento del consumo de ox&iacute;geno por la respiraci&oacute;n contribuyen a condiciones com&oacute;rbidas como lo son el reflujo gastroesof&aacute;gico y la apnea del sue&ntilde;o (SAOS).<sup>76&#45;78</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Los pacientes obesos tienen un riesgo seis veces mayor de manifestar apnea obstructiva del sue&ntilde;o. Esta condici&oacute;n puede, por s&iacute; misma, predisponer al riesgo del desarrollo de hipertensi&oacute;n arterial, de enfermedad cardiovascular, de trastornos del comportamiento, de bajo rendimiento escolar y de una pobre calidad de vida.</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Trastornos respiratorios en la obesidad</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La p&eacute;rdida de peso ha mostrado que mejora la funci&oacute;n pulmonar y que disminuye los s&iacute;ntomas del asma, pero no necesariamente en la obstrucci&oacute;n del flujo a&eacute;reo o en la HRB.<sup>79&#45;81</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En ni&ntilde;os se ha observado que la limitaci&oacute;n respiratoria de la obesidad no m&oacute;rbida est&aacute; m&aacute;s relacionada con su percepci&oacute;n sintom&aacute;tica que con el agravamiento del asma. De igual manera, los niveles de actividad y la potencia aer&oacute;bica m&aacute;xima est&aacute;n m&aacute;s bajos.<sup>82</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">En un estudio de ni&ntilde;os asm&aacute;ticos de 3 a 5 a&ntilde;os de edad se encontr&oacute; una asociaci&oacute;n entre el aumento del IMC con mayor ausentismo escolar, m&aacute;s consultas de urgencia y mayor limitaci&oacute;n de su actividad f&iacute;sica.<sup>83</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Tambi&eacute;n se ha observado que en una unidad de terapia intensiva las crisis asm&aacute;ticas fueron m&aacute;s en ni&ntilde;os obesos, adem&aacute;s de que tuvieron una estancia m&aacute;s prolongada y requirieron un curso m&aacute;s largo de suplementos de ox&iacute;geno, de salbutamol continuo y de esteroides por v&iacute;a intravenosa.<sup>84</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Influencia del g&eacute;nero y su relaci&oacute;n hormonal</b></font></p>  	    <p align="justify"><font face="verdana" size="2">Como se mencion&oacute; al inicio, la conexi&oacute;n entre la obesidad y el asma ha sido m&aacute;s favorecida en mujeres pospuberales que en hombres, mientras que en los ni&ntilde;os algunos estudios lo demuestran y otros no.<sup>24,28,31,34,</sup><sup>85</sup> En el estudio cohorte de Tucson, Castro&#45;Rodr&iacute;guez y cols. encontraron que la prevalencia de s&iacute;ntomas de asma fue mayor en aquellas ni&ntilde;as obesas que ten&iacute;an menarca temprana (antes de los 11&nbsp; a&ntilde;os de edad) que entre aqu&eacute;llas que la presentaban en edades posteriores.<sup>86</sup> Una probable explicaci&oacute;n es que esto se debe a los estr&oacute;genos.<sup>87</sup>,<sup>88</sup> En la obesidad existe un aumento de la enzima aromatasa, que est&aacute; en el tejido adiposo y convierte los andr&oacute;genos en estr&oacute;genos, los cuales tienen una acci&oacute;n broncoconstrictora y se asocian con la menarca precoz y con un retraso de la pubertad en los varones.<sup>88</sup> A partir de este hecho se ha postulado que la obesidad altera la producci&oacute;n de las hormonas relacionadas con la pubertad; una producci&oacute;n incrementada de las hormonas femeninas (o de su sensibilidad) altera el desarrollo pulmonar y la regulaci&oacute;n del tono de la v&iacute;a a&eacute;rea en las ni&ntilde;as p&uacute;beres. Sin embargo, este mecanismo no se conoce con exactitud.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Factores gen&eacute;ticos</b></font></p>  	    <p align="justify"><font face="verdana" size="2">Biol&oacute;gicamente es posible que ciertos genes que est&eacute;n relacionados con una enfermedad particular puedan estar relacionados con otras. Se ha determinado que ciertas regiones espec&iacute;ficas del genoma humano est&aacute;n relacionadas tanto con el asma como con la obesidad, por ejemplo los <i>loci</i> 5q, 6, 11q13 y 12q (<a href="/img/revistas/bmim/v68n3/a2c2.jpg" target="_blank">Cuadro II</a>).<sup>68</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>La influencia de la dieta</b></font></p>  	    <p align="justify"><font face="verdana" size="2">Se ha reportado que la dieta influye en la prevalencia del asma. Por ejemplo, los antioxidantes (vitaminas C y E), los carotenos, la riboflavina y la piridoxina pueden tener un efecto importante al incrementar la funci&oacute;n inmune, reducir los s&iacute;ntomas de asma/eccema y mejorar la funci&oacute;n pulmonar. Romieu y cols. han reportado que mujeres adultas que consum&iacute;an frutas y vegetales tuvieron una prevalencia baja de asma.<sup>89</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Asma, obesidad y ambiente</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La influencia de los ambientes intra y extradomiciliarios como factores desencadenantes de las crisis asm&aacute;ticas est&aacute; ampliamente documentada.<sup>90</sup> En las <a href="/img/revistas/bmim/v68n3/a2f3.jpg" target="_blank">figuras 3</a> y <a href="/img/revistas/bmim/v68n3/a2f4.jpg" target="_blank">4</a> se muestran las principales relaciones entre el asma, la obesidad y los factores ambientales.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Asma, obesidad y atopia</b></font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Existe una diversidad de factores que pueden desencadenar el asma; uno de ellos es el mediado por la inmunoglobulina E (IgE) pero este no se ha relacionado con mayor IMC. Aunque el fenotipo del asma que se ha relacionado con la obesidad es de tipo grave &eacute;ste, muy probablemente, podr&iacute;a estar mediado por otro mecanismo.<sup>91</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Obesidad y mediadores de la inflamaci&oacute;n</b></font></p>  	    <p align="justify"><font face="verdana" size="2">La inflamaci&oacute;n silenciosa y da&ntilde;ina que produce la obesidad puede verse incrementada cuando coexiste con el asma. El incremento en la funci&oacute;n del tejido adiposo en los sujetos obesos conlleva a un estado proinflamatorio sist&eacute;mico en el que las concentraciones s&eacute;ricas de citocinas, de fracciones solubles de sus receptores y de quimiocinas se encuentran aumentadas.<sup>92</sup> Muchos de estos mediadores son sintetizados y secretados por c&eacute;lulas del tejido adiposo y se les ha dado el nombre gen&eacute;rico de adipocinas; se incluyen en este grupo a la IL&#45;6, la IL&#45;10, la eotaxina, el factor de necrosis tumoral (TNF), el factor de crecimiento transformante beta (TGF&#45;&#946;1), la prote&iacute;na C reactiva, la leptina y la adiponectina.</font></p>  	    <p align="justify"><font face="verdana" size="2">El TNF se encuentra en los adipocitos, se relaciona directamente con la grasa corporal, se eleva en el asma y est&aacute; relacionado con la producci&oacute;n de citocinas T<sub>H</sub>2 (IL&#45;4, IL&#45;6) en el epitelio bronquial. Los niveles s&eacute;ricos de IL&#45;6 est&aacute;n elevados en sujetos obesos y se asocian con la gravedad del asma.<sup>93</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Uno de los mayores avances es el conocimiento de la v&iacute;a de se&ntilde;alizaci&oacute;n de la leptina; hay ciertas mutaciones de inactivaci&oacute;n que afectan los genes que codifican esta v&iacute;a, lo que condiciona un peque&ntilde;o porcentaje de la presencia de obesidad grave de inicio temprano.<sup>94</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La leptina se produce en el tejido adiposo y se une a su receptor en el n&uacute;cleo arcuato. Su concentraci&oacute;n aumenta con el incremento de la masa grasa, en cambio, en los individuos con masa grasa baja (p. ej. s&iacute;ndromes de lipodistrofia o anorexia nerviosa) los niveles de leptina son bajos. El ayuno disminuye la concentraci&oacute;n de leptina de manera aguda y su deficiencia provoca una se&ntilde;al que estimula la b&uacute;squeda de alimento, las conductas de consumo y el gasto de energ&iacute;a. La restauraci&oacute;n de los niveles normales lleva a limitar la ingesta de alimentos y cambia las regiones de activaci&oacute;n involucradas en el control del apetito.<sup>94</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Muchos trabajos orientados hacia la investigaci&oacute;n de la relaci&oacute;n entre el asma y la obesidad se han enfocado en el papel de la leptina porque se cree que act&uacute;a como lipostato; cuando las cantidades de grasa almacenada en los adipocitos se incrementan se libera al torrente circulatorio. Esto constituye una se&ntilde;al de retroalimentaci&oacute;n negativa para el hipot&aacute;lamo, que responde con la liberaci&oacute;n de p&eacute;ptidos anorexig&eacute;nicos y suprime la producci&oacute;n de p&eacute;ptidos orexig&eacute;nicos. El gasto energ&eacute;tico se incrementa al igual que la tasa del metabolismo basal y la temperatura corporal. Al mismo tiempo se modifica el punto de equilibrio basal para la reducci&oacute;n de la lipog&eacute;nesis y se incrementa la lip&oacute;lisis en el tejido adiposo.<sup>95</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">Otro papel que desempe&ntilde;a la leptina es que posee una considerable homolog&iacute;a estructural con las citocinas de cadena larga, como la IL&#45;6, y es capaz de regular la proliferaci&oacute;n y la activaci&oacute;n de los linfocitos T, promover la angiog&eacute;nesis y el reclutamiento de monocitos y macr&oacute;fagos activados.<sup>96</sup> Adem&aacute;s, es importante para el desarrollo normal de los pulmones y act&uacute;a como un mediador en la diferenciaci&oacute;n entre lipofibroblastos y fibroblastos y en la s&iacute;ntesis de surfactante pulmonar.<sup>97</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">A diferencia de otras adipocinas, los niveles s&eacute;ricos de adiponectina est&aacute;n reducidos en sujetos obesos y m&aacute;s a&uacute;n en obesos asm&aacute;ticos.<sup>98</sup> Se ha demostrado que esta hormona posee propiedades antiinflamatorias, incluso en las v&iacute;as a&eacute;reas.<sup>99</sup></font></p>  	    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Un incremento sistem&aacute;tico de la eotaxina se ha encontrado en individuos obesos. Parte de esta quimiocina es sintetizada en los adipocitos y sugiere un papel potencial de riesgo de asma en pacientes obesos.<sup>100</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Obesidad y alteraciones metab&oacute;licas</b></font></p>  	    <p align="justify"><font face="verdana" size="2">Aparte de las alteraciones en la funci&oacute;n pulmonar en el obeso asm&aacute;tico se pueden encontrar las complicaciones inherentes de la obesidad como son las alteraciones en la glucemia, en la presi&oacute;n arterial, en los l&iacute;pidos y en la resistencia a la insulina.<sup>101</sup> En comparaci&oacute;n con los trastornos metab&oacute;licos que origina la obesidad existen pocos estudios que eval&uacute;en al mismo tiempo asma y obesidad. En un grupo de adolescentes obesos, con y sin asma, y asm&aacute;ticos sin obesidad, as&iacute; como controles sanos, nuestro grupo encontr&oacute; que los varones obesos asm&aacute;ticos tuvieron mayor frecuencia de s&iacute;ndrome metab&oacute;lico.<sup>102</sup></font></p>  	    <p align="justify"><font face="verdana" size="2">La mayor&iacute;a de los estudios epidemiol&oacute;gicos prospectivos indican que la obesidad puede aumentar la prevalencia y la incidencia del asma, con predominio en mujeres posp&uacute;beres. La probabilidad de desarrollar asma en un escolar obeso puede ser de hasta 50%. Existe una clara relaci&oacute;n entre el mayor IMC y el asma, lo que sugiere que el riesgo aumenta a medida que aumenta el peso. Esto se confirma con el efecto de las intervenciones para perder peso, que se asocian con una disminuci&oacute;n en los s&iacute;ntomas del asma. Las complejas relaciones entre estos dos padecimientos son un ejemplo de la interacci&oacute;n, en su patog&eacute;nesis, entre la gen&eacute;tica y el ambiente. Es muy probable que est&eacute; involucrado m&aacute;s de un mecanismo biol&oacute;gico (<a href="/img/revistas/bmim/v68n3/a2f5.jpg" target="_blank">Figura 5</a>).</font></p>  	    <p align="justify"><font face="verdana" size="2">La obesidad puede afectar directamente el fenotipo del asma por efecto mec&aacute;nico, por cierre de la v&iacute;a a&eacute;rea, por modulaci&oacute;n de citocinas del tejido adiposo a trav&eacute;s de genes comunes o de regiones gen&eacute;ticas o por otros efectos que incluyen a los estr&oacute;genos. Se necesitan m&aacute;s investigaciones para elucidar estos dos fen&oacute;menos y las m&uacute;ltiples interrelaciones existentes. Es claro que la disminuci&oacute;n del peso mejora la salud respiratoria del asm&aacute;tico por lo que el tratamiento de los asm&aacute;ticos obesos debe incluir un programa de control de peso, con la premisa de que la obesidad altera la respuesta normal al tratamiento antiasm&aacute;tico.</font></p>  	    <p align="justify"><font face="verdana" size="2">Es posible que el asma est&eacute; sobrediagnosticada en la poblaci&oacute;n de pacientes obesos y sea fenot&iacute;picamente diferente a la que padecen los individuos con peso normal. Sin embargo, no se debe olvidar que la obesidad por s&iacute; sola causa impedimentos fisiol&oacute;gicos de la funci&oacute;n pulmonar con predominio de un patr&oacute;n restrictivo, como el reflujo gastroesof&aacute;gico y la apnea del sue&ntilde;o que aumentan el consumo de ox&iacute;geno, y que el exceso de infiltraci&oacute;n grasa contribuya a la comorbilidad.</font></p>  	    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>  	    <p align="justify"><font face="verdana" size="2"><b>Puntos clave</b></font></p>  	    <blockquote> 		    ]]></body>
