<body><![CDATA[<p align="justify"><font face="verdana" size="2">Existe en la actualidad una corriente conceptual, sustentada en evidencias, de que la prematurez, m&aacute;s que ser un problema del &uacute;ltimo tercio del embarazo, realmente corresponde a una enfermedad cr&oacute;nica que se inicia desde las primeras etapas del embarazo, o a&uacute;n antes, en la que participan factores gen&eacute;ticos y ambientales que pueden ser causales, al menos te&oacute;ricamente, de da&ntilde;os en el feto y de manera relevante en el cerebro de &eacute;ste.<sup>1</sup></font></p>     <p align="justify"><font face="verdana" size="2">La mortalidad de los ni&ntilde;os prematuros, en los &uacute;ltimos lustros, tiene una franca tendencia al descenso, incluso en aqu&eacute;llos con edades gestacionales de 23 a 25 semanas. Esto es consecuencia de mejores estrategias de tratamiento de los problemas respiratorios, infecciosos, metab&oacute;licos y neurol&oacute;gicos, lo que es causa de que la poblaci&oacute;n de sobrevivientes nacidos pret&eacute;rmino sea cada vez mayor y acumulativa. A lo anterior se agrega que en los &uacute;ltimos 25 a&ntilde;os se ha incrementado la incidencia de la prematurez en varios pa&iacute;ses, por ejemplo, en Estados Unidos de Norteam&eacute;rica actualmente ocurre en 12.5% de todos los nacimientos (cerca de medio mill&oacute;n de ni&ntilde;os anualmente). Entre los seis y los ocho meses de la gestaci&oacute;n, etapa en la que el pret&eacute;rmino nace, existe mayor vulnerabilidad del cerebro ante diversas causas de lesi&oacute;n, explicable porque en ella se logra 35% del incremento en el tama&ntilde;o del cerebro, se produce un aumento cinco veces mayor del volumen de la sustancia blanca, se lleva al cabo la maduraci&oacute;n estructural, con su correspondiente incremento en la conectividad de las neuronas, la arborizaci&oacute;n dendr&iacute;tica, la formaci&oacute;n de la uni&oacute;n sin&aacute;ptica, y los procesos paralelos bioqu&iacute;micos y enzim&aacute;ticos de la maduraci&oacute;n.<sup>2</sup> Testimonio de lo anterior es la mayor frecuencia de afecciones neurol&oacute;gicas, de la funci&oacute;n cognitiva y de reducci&oacute;n en las capacidades de desarrollo acad&eacute;mico conforme se nace a menor edad gestacional, lo que se acent&uacute;a en los menores de 33 semanas, y a&uacute;n m&aacute;s en los de 26 &oacute; menos.</font></p>     <p align="justify"><font face="verdana" size="2">Existen multitud de estudios de cohortes de ni&ntilde;os prematuros seguidos en los primeros 12 a 48 meses de edad gestacional corregida, que por el tiempo de observaci&oacute;n y de la metodolog&iacute;a empleada, as&iacute; como por los tama&ntilde;os de muestra, han revelado distintos grados de secuelas neurol&oacute;gicas a corto plazo.<sup>3&#150;5</sup> En otro grupo de trabajos publicados, y que son menos numerosos a los anteriores, con seguimiento hasta los seis a nueve a&ntilde;os de edad y algunos, los menos, hasta la adolescencia, demuestran: afecci&oacute;n del coeficiente intelectual asociado con el per&iacute;metro cef&aacute;lico y otros aspectos,<sup>6,7</sup> mayor prevalencia de alteraciones oft&aacute;lmicas condicionadas por alteraci&oacute;n en la percepci&oacute;n visual, y en los defectos motores y cognoscitivos,<sup>8</sup> problemas agresivos o de comportamiento delincuencial, o de d&eacute;ficit de atenci&oacute;n, independientemente de los pa&iacute;ses evaluados,<sup>9,10</sup> fallas en la funci&oacute;n motora, visoespacial y sensorio&#150;motora, causales de bajo aprovechamiento escolar y de sobreprotecci&oacute;n paternas,<sup>11&#150;17</sup> y un notorio incremento de ni&ntilde;os con par&aacute;lisis cerebral.<sup>18</sup></font></p>     <p align="justify"><font face="verdana" size="2">En los escasos estudios efectuados en adultos j&oacute;venes, se ha encontrado mayor n&uacute;mero de trastornos neurol&oacute;gicos que se manifiestan con funciones neuropsicol&oacute;gicas reducidas,<sup>19&#150;21</sup> afecci&oacute;n de la calidad de vida,<sup>22</sup> y psicopatolog&iacute;a,<sup>23</sup> lo que afecta la vida independiente de un buen n&uacute;mero de casos, la que se logra compensar con adecuados incentivos econ&oacute;micos para la prevenci&oacute;n secundaria de las discapacidades asociadas con el nacimiento pret&eacute;rmino.<sup>24</sup></font></p>     <p align="justify"><font face="verdana" size="2">La raz&oacute;n del t&iacute;tulo de la presente editorial y de los comentarios previos, se sustentan en un reciente trabajo publicado el 17 de julioenlarevista <i>New England Journal of Medicine,<sup>25</sup> </i>en el que se evalu&oacute; la relaci&oacute;n que pudiera existir entre la edad gestacional al momento del nacimiento y las secuelas a largo plazo de una cohorte que se obtiene del registro nacional de nacimientos de Noruega y de otras bases de datos, en las que se incluyeron discapacidades, educaci&oacute;n, ingreso econ&oacute;mico, <i>estatus </i>familiar, beneficios de la seguridad social, compensaciones por desempleo y registros criminales. La cohorte se integr&oacute; al final con 867 692 nacidos vivos, sin malformaciones cong&eacute;nitas, de los a&ntilde;os de 1967 a 1983 y desde las 23 semanas de gestaci&oacute;n hasta el t&eacute;rmino de la misma, que vivieron al menos hasta los 20 a&ntilde;os de edad. Entre los resultados m&aacute;s relevantes se encontr&oacute; un descenso significativo en la mortalidad neonatal por grupos de edad gestacional desde los a&ntilde;os de 1967 a 1983. El riesgo de cursar con discapacidades m&eacute;dicas como par&aacute;lisis cerebral, retardo mental, ceguera y p&eacute;rdida de la audici&oacute;n, se increment&oacute; de manera relevante conforme menor era la edad gestacional. En el seguimiento, desde las edades de 19 hasta los 35 a&ntilde;os, casi uno de cada nueve personas que nacieron entre las 23 a las 27 semanas de gestaci&oacute;n recibieron una pensi&oacute;n por discapacidad, comparado con 1 de 12 en los nacidos de 28 a 30 semanas, 1 de 24 entre las 31 y 33 semanas, 1 de 42 entre las 34 a 36, y 1 de 59 en los nacidos a t&eacute;rmino. En el grupo de menor edad gestacional (23 a 27) hubo un incremento significativo de par&aacute;lisis cerebral. Cuando se excluyeron del an&aacute;lisis a los individuos con discapacidad, a pesar de ello se encontr&oacute; una d&eacute;bil asociaci&oacute;n entre la edad gestacional y la proporci&oacute;n de personas que no lograban educaci&oacute;n superior, trabajos bien remunerados y los beneficios de la seguridad social. A menor edad gestacional se encontr&oacute; menor probabilidad de completar el nivel de preparatoria, o de recibir un grado de postgrado o de tener un elevado ingreso. La fortaleza del estudio de investigaci&oacute;n,<sup>25</sup> es haber evaluado una cohorte que incluy&oacute; todos los nacimientos que se sucedieron entre los a&ntilde;os 1967 a 1983 en Noruega, los que se pudieron correlacionar con las diferentes bases de datos que existen en el pa&iacute;s. Por lo anterior, adem&aacute;s de obtener con precisi&oacute;n los distintos rubros de diagn&oacute;sticos de los padecimientos de los individuos, se evaluaron con gran detalle y confiabilidad todas las condiciones sociales y educativas de cada uno de ellos. Otras fortalezas fueron que las p&eacute;rdidas de casos en el seguimiento a largo plazo fueron m&iacute;nimas, considerando el tama&ntilde;o de muestra original y el haber estudiado todo el espectro de las edades gestacionales al nacer, adem&aacute;s de la relaci&oacute;n entre el grado de prematuridad y varias de las consecuencias.</font></p>     <p align="justify"><font face="verdana" size="2">Aunque en los trabajos previos descritos aqu&iacute; brevemente<sup>18&#150;23</sup> en seguimientos a largo plazo, incluyendo adultos j&oacute;venes, se han encontrado resultados que en t&eacute;rminos generales son parecidos a los de Noruega,<sup>25</sup> la calidad de este &uacute;ltimo, por todos los aspectos se&ntilde;alados en sus fortalezas, le da mayor validez a sus hallazgos. Lo anterior, asociado con el incremento en las tasas de prematuridad y de menor mortalidad, hace pensar que, efectivamente, de continuar las condiciones actuales, llegaremos atener una poblaci&oacute;n de personas con secuelas neurol&oacute;gicas de distinto grado que se ir&aacute;n acumulando en el tiempo. Esto obliga a efectuar en nuestro pa&iacute;s proyectos de investigaci&oacute;n con la calidad y envergadura del trabajo aqu&iacute; comentado, con los matices propios de cada grupo de investigadores. Al mismo tiempo, debemos transitar en forma m&aacute;s acelerada a un sistema nacional de registro &uacute;nico de poblaci&oacute;n que incluya los factores biol&oacute;gicos, psicol&oacute;gicos, sociales y educativos para poder establecer pol&iacute;ticas de salud cada vez m&aacute;s confiables y adecuadamente sustentadas. Las condiciones actuales de nuestro pa&iacute;s, ante la ausencia de este tipo de registros, al menos debe estar orientada entre otros asuntos prioritarios a los de la prematurez y sus consecuencias, ya que al continuar disminuyendo la mortalidad a menores edades gestacionales, sumado al incremento en las tasas de prematuridad, aparecer&aacute;n mayor n&uacute;mero de discapacidades, no s&oacute;lo biol&oacute;gicas sino psicol&oacute;gicas, sociales y educativas, lo que adem&aacute;s de incrementar los costos en salud y en educaci&oacute;n, tienen importantes repercusiones en la din&aacute;mica familiar. De tal forma que en la transici&oacute;n epidemiol&oacute;gica, adem&aacute;s de los padecimientos cronicodegenerativos cl&aacute;sicos, deben incluirse todos los relacionados con la prematuridad y sus consecuencias.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Referencias</b></font></p>     <!-- ref --><p align="justify"><font face="verdana" size="2">1.
