La causa más común de los adquiridos es la endocarditis aórtica. Actualmente la ecocardiografía se ha convertido en el método diagnóstico de elección. Se presentan dos pacientes en edad pediátrica con aneurismas del seno de Valsalva no roto y manifestaciones de falla cardíaca secundaria a insuficiencia aórtica aguda. Se compararon las imágenes ecocardiográficas bidimensionales transtorácicas y transesofágicas y en uno de ellos tridimensionales, analizando la calidad de la imagen, localización y detalles anatómicos del aneurisma, obstrucción o compresión de estructuras adyacentes y la presencia de otros defectos asociados. Se demuestra la utilidad de la valoración ecocardiográfica tridimensional en el abordaje diagnóstico de estas lesiones.]]></p></abstract>

<body><![CDATA[<p align="justify"><font face="verdana" size="2">Los aneurismas de los senos de Valsalva a&oacute;rticos son malformaciones raras que se presentan en menos del 0.1% de los pacientes con cardiopat&iacute;as cong&eacute;nitas y en 0.05% de los casos de autopsia.<sup>1</sup> Por lo general, sonde origen cong&eacute;nito y en ocasiones asociados a endocarditis bacteriana. La utilidad de la ecocardiograf&iacute;a bidimensional transtor&aacute;cica y transesof&aacute;gica en el diagn&oacute;stico de estas lesiones est&aacute; bien establecida, sin embargo el advenimiento de t&eacute;cnicas nuevas como la ecocardiograf&iacute;a tridimensional han permitido definir con mayor precisi&oacute;n la morfolog&iacute;a de diversas cardiopat&iacute;as cong&eacute;nitas.<sup>2</sup></font></p>     <p align="justify"><font face="verdana" size="2">En este trabajo se presentan dos pacientes en edad pedi&aacute;trica con aneurismas de seno de Valsalva no rotos asociados a endocarditis infecciosa, en quienes el diagn&oacute;stico se realiz&oacute; con ecocardiograf&iacute;a bidimensional y en uno de ellos se complement&oacute; la valoraci&oacute;n morfol&oacute;gica con ecocardiograma tridimensional.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Caso l</b></font></p>     <p align="justify"><font face="verdana" size="2">Paciente masculino de 9 a&ntilde;os, asintom&aacute;tico hasta un mes antes de su ingreso cuando se le detect&oacute; soplo card&iacute;aco. Ingresa por insuficiencia card&iacute;aca descompensada. Al examen f&iacute;sico con pulsos amplios y soplo mesotelesist&oacute;lico y diast&oacute;lico largo en el foco a&oacute;rtico. La radiograf&iacute;a de t&oacute;rax con cardiomegalia grado I e hipertensi&oacute;n venocapilar pulmonar. Elecocardiogramatranstor&aacute;cico (ETT) mostr&oacute; en la aproximaci&oacute;n paraesternal en ejes largo y corto un aneurisma no roto del seno de Valsalva no coronariano que protru&iacute;a hacia la aur&iacute;cula izquierda, insuficiencia a&oacute;rtica severa y dilataci&oacute;n del ventr&iacute;culo izquierdo con compromiso de la funci&oacute;n ventricular izquierda <i>(<a href="/img/revistas/acm/v75n2/a4f1.jpg" target="_blank">Fig. 1</a>). </i>Las im&aacute;genes transesof&aacute;gicas (ETE) en corte transversal a nivel de la base confirmaron la presencia de un aneurisma no roto del seno de Valsalva no coronariano con flujo sistolodiast&oacute;lico y destrucci&oacute;n de la valva, el cual comprim&iacute;a el atrio izquierdo sin condicionar compromiso hemodin&aacute;mico. El aneurisma se continuaba hacia la aorta ascendente <i>(<a href="/img/revistas/acm/v75n2/a4f2.jpg" target="_blank">Fig. 2</a>). </i>La adquisici&oacute;n de im&aacute;genes ecocardiogr&aacute;ficas transesof&aacute;gicas para reconstrucci&oacute;n tridimensional se realizaron con un transductor multiplanar de 5&#150;7 MHz y se procesaron con sistema Echo&#150;Scan (versi&oacute;n 4.0 TomTec Gmb H Munich; Germany). Las im&aacute;genes tridimensionales en cortes sagitales y coronales mostraron con detalle el sitio de origen del aneurisma, su extensi&oacute;n y compresi&oacute;n del atrio izquierdo adem&aacute;s de la destrucci&oacute;n de la valva no coronariana <i>(<a href="#f3">Figs. 3</a> y <a href="/img/revistas/acm/v75n2/a4f4.jpg" target="_blank">4</a>). La </i>cirug&iacute;a confirm&oacute; los hallazgos. Se realiz&oacute; cambio valvular por pr&oacute;tesis de St. Jude de 19 mm y resecci&oacute;n del aneurisma con buenos resultados. El estudio anatomopatol&oacute;gico mostr&oacute; tejido fibroso reparativo con peque&ntilde;a vegetaci&oacute;n fibrinoide compatible con endocartitis infecciosa en fase de resoluci&oacute;n.</font></p>     <p align="center"><font size="2" face="verdana"><a name="f3"></a></font></p>     <p align="center"><font face="verdana" size="2">&nbsp;<img src="/img/revistas/acm/v75n2/a4f3.jpg"></font></p>     <p align="justify"><font face="verdana" size="2"><b>Caso 2</b></font></p>     <p align="justify"><font face="verdana" size="2">Masculino de 6 a&ntilde;os de edad, con historia de fiebre y manifestaciones de insuficiencia card&iacute;aca, se estableci&oacute; el diagn&oacute;stico de endocarditis a&oacute;rtica. Al examen f&iacute;sico se encontr&oacute; con pulsos amplios, fr&eacute;mito supraesternal, chasquido de apertura, soplo sist&oacute;lico y mesotelediast&oacute;lico enfoco a&oacute;rtico, irradiado a vasos del cuello. El electrocardiograma con hipertrofia y dilataci&oacute;n del ventr&iacute;culo izquierdo. Las im&aacute;genes bidimensionales transtor&aacute;cicas en ejes paraesternal largo y corto mostraron un aneurisma del seno de Valsalva coronariano derecho no roto que comprim&iacute;a el atrio derecho sin condicionar alteraciones hemodin&aacute;micas, con vegetaciones en la v&aacute;lvula a&oacute;rtica y aorta ascendente e insuficiencia a&oacute;rtica severa <i>(<a href="/img/revistas/acm/v75n2/a4f5.jpg" target="_blank">Fig. 5</a>). </i>En el ETE se observ&oacute; v&aacute;lvula a&oacute;rtica bivalva con insuficiencia severa, vegetaciones en la valva coronariana derecha, as&iacute; como aneurisma del seno de Valsalva coronariano derecho no roto de 20 x 30 mm, con compresi&oacute;n del atrio y ventr&iacute;culo derechos <i><a href="/img/revistas/acm/v75n2/a4f6.jpg" target="_blank">(Fig. 6)</a>. </i>Los hallazgos quir&uacute;rgicos corroboraron el diagn&oacute;stico. Se realiz&oacute; cambio valvular con implante de pr&oacute;tesis biol&oacute;gica INC (porcina con tronco de aorta y sin anillo) No. 22 y resecci&oacute;n del aneurisma. El estudio anatomopatol&oacute;gico concluy&oacute; endocarditis bacteriana aguda y subaguda. Desarroll&oacute; bloqueo AV completo por lo que se le coloc&oacute; marcapaso endoc&aacute;rdico. La evoluci&oacute;n posterior fue satisfactoria.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Discusi&oacute;n</b></font></p>     <p align="justify"><font face="verdana" size="2">Se ha considerado a los aneurismas de los senos de Valsalva como malformaciones cong&eacute;nitas ya que la patolog&iacute;a fundamental de este tipo de anomal&iacute;a es la separaci&oacute;n entre la capa media de la aorta y el sitio de inserci&oacute;n de las valvas a&oacute;rticas en su porci&oacute;n posterior.<sup>3</sup> La capa media de la aorta se retrae y no forma parte del seno de Valsalva dilatado, el cual est&aacute; formado de fibras de col&aacute;geno densas sin fibras musculares o tejido el&aacute;stico. Tambi&eacute;n se presentan secundarios a otras enfermedades sist&eacute;micas como s&iacute;filis, ateroesclerosis y endocarditis bacteriana. Cuando son adquiridos por lo general involucran a m&aacute;s de un seno de Valsalva y raramente se asocian a otras malformaciones cong&eacute;nitas, sin embargo, en ocasiones puede ser imposible determinar si la endocarditis es la causa o consecuencia del aneurisma. El seno coronariano anterior o derecho es el m&aacute;s frecuentemente involucrado, el no coronariano o posterior derecho est&aacute; afectado en el 20% aproximadamente y el menos afectado es el seno coronariano izquierdo.<sup>4</sup> Los aneurismas no rotos pueden comprimir las cavidades card&iacute;acas, las estructuras vasculares adyacentes o entrar al espacio peric&aacute;rdico. Se han reportado casos de obstrucci&oacute;n a la v&iacute;a de salida del ventr&iacute;culo derecho,<sup>5,6</sup> del ventr&iacute;culo izquierdo,<sup>7 </sup>disecci&oacute;n de septum interventricularu obstrucci&oacute;n coronaria.<sup>8</sup> La aorta ascendente por lo general se encuentra dilatada. </font></p>     <p align="justify"><font face="verdana" size="2">Los pacientes con aneurismas no rotos son asintom&aacute;ticos en las primeras d&eacute;cadas de la vida,<sup>9&#150;11</sup> s&oacute;lo presentan s&iacute;ntomas por las alteraciones hemodin&aacute;micas que condicionan por la compresi&oacute;n de estructuras adyacentes o anomal&iacute;as de la v&aacute;lvula a&oacute;rtica como engrosamiento de las valvas o insuficiencia la cual es frecuente como resultado del prolapso de la c&uacute;spide o distorsi&oacute;n del aparato valvular,<sup>12</sup> tal como se manifest&oacute; en los 2 casos que se presentan.La ecocardiogr&aacute;fica bidimensional transtor&aacute;cica es el m&eacute;todo diagn&oacute;stico inicial de elecci&oacute;n,<sup>5,9,13</sup> proporciona informaci&oacute;n valiosa acerca del seno involucrado y las caracter&iacute;sticas anat&oacute;micas del aneurisma como se muestra en uno de nuestros casos, determina si existe obstrucci&oacute;n o ruptura a estructuras adyacentes y eval&uacute;a las anomal&iacute;as asociadas; excluye adem&aacute;s diagn&oacute;sticos diferenciales como el t&uacute;nel aorta&#150;ventr&iacute;culo izquierdo, sin embargo, el abordaje transesof&aacute;gico est&aacute; indicado como m&eacute;todo complementario<sup>11,14&#150;16</sup> en donde se valora la dilataci&oacute;n de los senos a&oacute;rticos asociados con los aneurismas y la presencia de la insuficiencia a&oacute;rtica.</font></p>     <p align="justify"><font face="verdana" size="2">Con el advenimiento de la ecocardiograf&iacute;a tridimensional se ha logrado una mejor definici&oacute;n anat&oacute;mica en diversas cardiopat&iacute;as cong&eacute;nitas, incluyendo las alteraciones de la v&aacute;lvula a&oacute;rtica.<sup>2,17</sup> La ecocardiograf&iacute;a tridimensional ofrece una mejor evaluaci&oacute;ny entendimiento del defecto con detalles anat&oacute;micos de la localization e integridad de los aneurismas y su relaci&oacute;n con estructuras adyacentes, confirmando que este m&eacute;todo es de gran utilidad para definir la cardiopat&iacute;a ya que proporciona detalles anat&oacute;micos &uacute;tiles en la planeaci&oacute;n del abordaje y tratamiento quir&uacute;rgico, es un m&eacute;todo novedoso con aplicaci&oacute;n en el estudio preoperatorio de los pacientes con &eacute;ste y otros defectos card&iacute;acos. El tratamiento quir&uacute;rgico est&aacute; indicado aun en los pacientes asintom&aacute;ticos, ya que pueden ser fuentes embol&iacute;genas<sup>8,18,19</sup> y por el riesgo de ruptura.<sup>20 </sup>No se conoce la historia natural de los aneurismas no rotos ya que como se ha comentado son lesiones que no son detectadas por ser asintom&aacute;ticas.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Referencias</b></font></p>     <!-- ref --><p align="justify"><font face="verdana" size="2">1.&nbsp; &nbsp; Anderson RH, Macartney FJ, Shinebourne EA, Tynan M, Bus A: <i>The child with a continuous murmur. </i>In: Anderson RH, Macartney FJ, Shinebourne EA, Tinan M, eds.
