MATERIALES Y MÉTODOS:  Estudio observacional, comparativo, retrospectivo y transversal efectuado en el departamento de reproducción asistida de una institución de tercer nivel. Se analizaron los ciclos de transferencia de embriones en fresco y desvitrificados practicados entre los años 2020 al 2024. Se incluyeron pacientes con infertilidad tratadas mediante fertilización in vitro, inyección intracitoplasmática de espermatozoides o su variante fisiológica. Se aplicaron protocolos individualizados de estimulación ovárica y transferencia según las características de cada paciente. Se evaluaron las variables clínicas, embrionarias, hormonales y reproductivas; los desenlaces incluidos fueron las tasas de embarazo y nacido vivo. La criopreservación fue mediante vitrificación.  RESULTADOS:  Se analizaron 502 ciclos de transferencia embrionaria (284 en fresco, 218 desvitrificados) de pacientes con edad media de 34.3 años; 298 (59.4%) con infertilidad primaria. La tasa general de embarazo clínico fue del 30.2%. El grosor del endometrio mayor de 10 mm y la transferencia en estadio de blastocisto se asociaron con mayor probabilidad de embarazo clínico. Entre ciclos en fresco y desvitrificados no hubo diferencias significativas en las tasas de embarazo o nacido vivo. En los ciclos con embriones desvitrificados, el tiempo de almacenamiento ni el tipo de preparación influyeron de manera significativa en los desenlaces.  CONCLUSIONES:  En ciclos de transferencia en fresco y con embriones desvitrificados el grosor del endometrio y el estadio de desarrollo embrionario son factores decisivos para lograr el embarazo clínico. En los ciclos de transferencia de embriones desvitrificados, el tiempo de almacenamiento no mostró un efecto negativo en los desenlaces reproductivos. Estos hallazgos resaltan la importancia de individualizar el tipo de transferencia y optimizar cada variable en los procedimientos de reproducción asistida.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  BACKGROUND:  Fresh and devitrified embryo transfer are widely used strategies in assisted reproduction. The objective of this study is to compare both modalities and analyze the factors associated with reproductive success.  MATERIALS AND METHODS:  An observational, retrospective, and cross-sectional study was conducted at the Assisted Reproduction Department of a tertiary-level institution, analyzing 502 embryo transfer cycles (284 fresh, 218 devitrified) between 2020 and 2024. Women with infertility treated by IVF, ICSI, or PICSI were included. Ovarian stimulation and embryo transfer protocols were individualized according to each patient&#8217;s characteristics. Clinical, embryological, hormonal, and reproductive variables were analyzed, with pregnancy and live birth rates as outcomes. Cryopreservation was performed using vitrification.  RESULTS:  The mean age was 34.3 years, and 59.4% had primary infertility. The overall clinical pregnancy rate was 30.2%. Endometrial thickness &gt;10 mm and blastocyst stage embryo transfers were significantly associated with higher clinical pregnancy rates. There were no significant differences in pregnancy or live birth rates between fresh and devitrified cycles. In cycles with devitrified embryos, neither storage time nor preparation type significantly influenced the outcomes.  CONCLUSIONS:  This study identified endometrial thickness and embryo developmental stage as key factors for achieving clinical pregnancy.]]></p></abstract>
