METODOLOGIA: Revisión sistemática de artículos que refieren la suplementación con vitamina D en pacientes con diabetes gestacional, con apego a los criterios PRISMA y PICO de Cochrane. Se analizaron ensayos clínicos aleatorizados de las bases de datos PubMed, LILACS, Cochrane, Scielo y Springer Link (2012-2024). Se evaluaron cinco estudios mediante las escalas PEDro y JADAD.  RESULTADOS: La suplementación con vitamina D mostró cambios significativos en las concentraciones de 25-hidroxivitamina D en todos los estudios. Las concentraciones de glucosa en ayuno e índice HOMA-IR mejoraron en el grupo de intervención comparado con el control, en dos de los cinco estudios. Sin embargo, los reportes de la hemoglobina glucosilada no fueron consistentes. Cuando la suplementación se administró de manera continua en dosis moderadas mostró efectos positivos.  CONCLUSIONES: Los hallazgos no son concluyentes para la toma rutinaria de vitamina D en pacientes con diabetes mellitus gestacional. La heterogeneidad en las dosis y la duración de la suplementación entre los estudios limita la capacidad de comparación. La variabilidad genética y el estado basal son factores que pueden influir en la eficacia de los suplementos, así como la formulación por sí misma. En este sentido, la evidencia actual es insuficiente para establecer recomendaciones definitivas. Se necesitan estudios adicionales para determinar las dosis óptimas, la duración adecuada del tratamiento y sus efectos en indicadores metabólicos, con el fin de desarrollar guías basadas en evidencia para su implementación efectiva.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  OBJECTIVE: To evaluate the scientific evidence regarding the effects of vitamin D supplementation on metabolic variables in patients with gestational diabetes mellitus.  METHODOLOGY: A systematic review was conducted on articles referring to vitamin D supplementation in patients with gestational diabetes, adhering to the Cochrane, PRISMA, and PICO criteria. Randomized clinical trials from PubMed, LILACS, Cochrane, SciELO, and SpringerLink (2012-2024) databases were analyzed. Five studies were evaluated using the PEDro and JADAD scales.  RESULTS: Vitamin D supplementation showed significant changes in 25-hydroxyvitamin D concentrations in all studies. Fasting glucose concentrations and HOMA-IR improved in the intervention group compared to the control group in two of the five studies. However, reports of glycated hemoglobin were inconsistent. Positive effects were observed when supplementation was administered continuously at moderate doses.  CONCLUSIONS: The findings are inconclusive regarding the routine intake of vitamin D in patients with gestational diabetes mellitus. Heterogeneity in the doses and duration of supplementation across studies limits comparability. Factors such as genetic variability, baseline vitamin D status, and the formulation itself may influence the efficacy of supplements. In this regard, current evidence is insufficient to establish definitive recommendations. Additional studies are needed to determine the optimal dosage and treatment duration and to assess the effects on metabolic indicators in order to develop evidence-based guidelines for effective implementation.]]></p></abstract>
