Por la persistencia de la sintomatología sin mejoría, acudió con un tercer facultativo quien realizó un electrocardiograma (ECG), el cual documentó lesión subepicárdica inferior; fue trasladado para realizar coronariografía, la cual mostró oclusión trombótica funcional del segmento proximal de la coronaria derecha, misma que fue tratada exitosamente con intervención coronaria percutánea primaria e implante de stent medicado. Este caso muestra que el infarto agudo del miocardio asociado con trauma cerrado de tórax es una patología de difícil diagnóstico.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[This is a case of the 44 year old male patient without history or risk factors for coronary disease, with blunt chest trauma in the anterior region after training of an Israeli martial art, right after he had typical retroesternal chest pain, he went to different doctors who prescribed different antiinflamatory drugs, because the persistence of the pain he decided to check with a third doctor who took an EKG and found an inferior epicardial lesion. Coronary angiography showed functional thrombotic occlusion of proximal segment of the right coronary. Successful primary angioplasty and stenting was done. The case shows that myocardial infarction after blunt chest trauma is challenging in terms of diagnosis and treatment.]]></p></abstract>

The case shows that myocardial infarction after blunt chest trauma is challenging in terms of diagnosis and treatment.</font></p>     <p align="justify"><font face="verdana" size="2"><b>Key words:</b> Myocardial infarction, chest pain, blunt trauma.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>INTRODUCCI&Oacute;N</b></font></p>     <p align="justify"><font face="verdana" size="2">Dentro de las etiolog&iacute;as no ateroscler&oacute;ticas de infarto agudo del miocardio (IAM), el secundario a trauma no penetrante de t&oacute;rax es una entidad muy rara.<sup>1-13</sup></font></p>     <p align="justify"><font face="verdana" size="2">La dificultad para realizar el diagn&oacute;stico se enmascara por el propio contexto traum&aacute;tico del evento, pues se confunde f&aacute;cilmente con la cl&iacute;nica del propio traumatismo. Por esta raz&oacute;n, en general el IAM no se considera como una causa de dolor precordial despu&eacute;s de trauma tor&aacute;cico, sobre todo en pacientes j&oacute;venes sin factores de riesgo cardiovascular en donde el dolor se atribuye al impacto <i>per se</i>.</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Presentamos el caso de un hombre previamente sano que posterior a trauma cerrado de t&oacute;rax se encontr&oacute; una oclusi&oacute;n tromb&oacute;tica funcional de la coronaria derecha.</font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>CASO CL&Iacute;NICO</b></font></p>     <p align="justify"><font face="verdana" size="2">Paciente masculino de 44 a&ntilde;os de edad, previamente sano, sin antecedentes de importancia, que presenta dolor tor&aacute;cico secundario a m&uacute;ltiples traumatismos de t&oacute;rax por enfrentamiento corporal que recibi&oacute; durante un entrenamiento de un programa de artes marciales israel&iacute; (Krav Maga). El dolor tor&aacute;cico en la regi&oacute;n retroesternal era opresivo de intensidad 10/10, motivo por el cual fue visto por el m&eacute;dico general quien prescribi&oacute; analg&eacute;sicos y antiinflamatorios.</font></p>     <p align="justify"><font face="verdana" size="2">Dos a tres horas despu&eacute;s sigui&oacute; con dolor y acudi&oacute; a un servicio de urgencias, en el que el m&eacute;dico le dio antiinflamatorios parenterales y fue dado de alta a su domicilio. Al d&iacute;a siguiente present&oacute; dos episodios de s&iacute;ncope y persistencia del dolor opresivo en regi&oacute;n retroesternal sin irradiaciones, con mareo intenso, acompa&ntilde;ado de descarga adren&eacute;rgica y neurovegetativa; decidi&oacute; acudir con un tercer facultativo, que en la exploraci&oacute;n f&iacute;sica lo encontr&oacute; p&aacute;lido, con bradicardia (50x') e hipotensi&oacute;n arterial (80/45 mmHg). El ECG mostr&oacute; lesi&oacute;n subepic&aacute;rdica y necrosis inferior (<a href="#a7f1" target="_self">Figura 1</a>). Los biomarcadores cardiacos fueron positivos, por lo que se estableci&oacute; el diagn&oacute;stico de un s&iacute;ndrome isqu&eacute;mico coronario agudo tipo infarto agudo del miocardio con elevaci&oacute;n del segmento ST Inferior (Killip Kimball II) de ocho horas de evoluci&oacute;n. Debido a que se encontr&oacute; en un centro hospitalario sin facilidades para hemodin&aacute;mica, se decidi&oacute; su traslado en ambulancia para realizar una coronariograf&iacute;a urgente. Se le administr&oacute; aspirina y aminas vasoactivas. A su llegada a la sala de hemodin&aacute;mica, a&uacute;n con dolor, se decidi&oacute; puncionar la arteria radial derecha, evalu&aacute;ndose la coronaria derecha con disecci&oacute;n tipo E y lesi&oacute;n suboclusiva 99% tromb&oacute;tica en el segmento proximal, con flujo epic&aacute;rdico TIMI I (<a href="#a7f2" target="_self">Figura 2</a>). El sistema izquierdo sin lesiones angiogr&aacute;ficas significativas. Se decidi&oacute; realizar una intervenci&oacute;n coronaria percut&aacute;nea primaria, carga de tienopiridina (prasugrel), utiliz&aacute;ndose extractor de trombos, con apertura del vaso y con defecto de llenado pero con flujo distal TIMI II; se implant&oacute; de forma directa un stent medicado en el segmento proximal de la coronaria derecha con buen resultado angiogr&aacute;fico, flujo epic&aacute;rdico TIMI III y mioc&aacute;rdico TIMI II (<a href="#a7f3" target="_self">Figura 3</a>).</font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p><a name="a7f1"></a></p>    <p>&nbsp;</p>    <p align="center"><img src="../img/revistas/rmc/v25n2/a7f1.jpg"></p>    ]]></body>

<body><![CDATA[<p>&nbsp;</p>    <p><font size="2" face="Verdana"></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>DISCUSI&Oacute;N</b></font></p>     <p align="justify"><font face="verdana" size="2">La oclusi&oacute;n coronaria tromb&oacute;tica en una complicaci&oacute;n muy rara del trauma tor&aacute;cico.<sup>14,15</sup> Se han reportado 24 casos en la literatura mundial y casi todos recibieron tratamiento conservador, sin embargo, la disecci&oacute;n de una arteria coronaria como complicaci&oacute;n de trauma cerrado del t&oacute;rax se ha reportado con m&aacute;s frecuencia en tiempos recientes,<sup>16</sup> lo que nos puede indicar que esta entidad no ha sido diagnosticada o que su incidencia ha aumentado.</font></p>     <p align="justify"><font face="verdana" size="2">Parmely report&oacute; que de 546 autopsias realizadas a pacientes fallecidos despu&eacute;s de un trauma de t&oacute;rax &uacute;nicamente nueve ten&iacute;an ruptura de una arteria coronaria y s&oacute;lo un caso de laceraci&oacute;n de la &iacute;ntima; ninguno de estos casos mostr&oacute; datos de oclusi&oacute;n de arteria coronaria.<sup>17</sup></font></p>     <p align="justify"><font face="verdana" size="2">Ocasionalmente se ha demostrado <i>in vivo</i> la trombosis coronaria postraum&aacute;tica; los hallazgos patol&oacute;gicos incluyen disecci&oacute;n, ruptura, desgarros, lesi&oacute;n de la &iacute;ntima, hemorragia subintimal, trombosis intraluminal y espasmo<sup>4,13,18,19</sup> que se presenta en una lesi&oacute;n ateromatosa previa.<sup>8,9-12</sup></font></p>     <p align="justify"><font face="verdana" size="2">En el paciente se puede presentar con un gran componente de estimulaci&oacute;n adren&eacute;rgica secundario a la compresi&oacute;n del estern&oacute;n contra la superficie anterior del coraz&oacute;n y la desaceleraci&oacute;n s&uacute;bita del coraz&oacute;n, con la compresi&oacute;n del miocardio y de las arterias coronarias contra la pared tor&aacute;cica. Se puede presentar: a) ruptura de la placa ateromatosa previa, b) ruptura de un n&uacute;cleo de calcio de la placa, si la placa esta calcificada; c) puede haber una erosi&oacute;n superficial de la placa, d) una hemorragia dentro de la placa o e) un espasmo de la arteria coronaria; todas ellas con la exposici&oacute;n de col&aacute;geno, factor tisular, factor de Von Willebrand, calcio y ADP, lo que activa la agregaci&oacute;n plaquetaria subsecuente<sup>20</sup> y desarrollo de una oclusi&oacute;n tromb&oacute;tica.<sup>7</sup></font></p>     <p align="justify"><font face="verdana" size="2">Por la posici&oacute;n anat&oacute;mica de la arteria coronaria descendente anterior y su proximidad con la pared anterior del t&oacute;rax se convierte en el vaso que se afecta con m&aacute;s frecuencia con este tipo de mecanismo,<sup>21-23</sup> principalmente, en el segmento proximal y es responsable del 66-71.4% de 5 los IAM postraum&aacute;ticos; la arteria coronaria derecha representa el 19-25% y la arteria circunfleja de un 3.2-9%.<sup>5,6,12,21,24-27</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">El diagn&oacute;stico cl&iacute;nico puede ser dif&iacute;cil, ya que la sintomatolog&iacute;a puede estar enmascarada por otras lesiones en el t&oacute;rax, incluso en algunos casos el ECG puede ser normal.<sup>28</sup> En la mayor&iacute;a, el ECG muestra cambios,<sup>29,30</sup> puede haber bloqueo de rama derecha, elevaci&oacute;n del segmento ST, p&eacute;rdida de la onda R y aparici&oacute;n de la onda Q.<sup>31-33</sup> Por ecocardiograf&iacute;a se pueden demostrar anormalidades de la movilidad segmentaria de la pared en caso de isquemia o de IAM,<sup>29,31,34</sup> as&iacute; como hemopericardio o problemas valvulares y/o disecci&oacute;n a&oacute;rtica.<sup>35</sup></font></p>     <p align="justify"><font face="verdana" size="2">El examen diagn&oacute;stico de mayor utilidad no invasivo es el electrocardiograma, sin embargo, la literatura reporta s&oacute;lo un 2% de pacientes con patrones de IAM, por lo que en este tipo de pacientes los biomarcadores cardiacos son de mucha utilidad,<sup>32</sup> aunque tienen la desventaja de que no se puede diferenciar el da&ntilde;o cardiaco o contusi&oacute;n del miocardio de la oclusi&oacute;n coronaria postraum&aacute;tica.</font></p>     <p align="justify"><font face="verdana" size="2">El manejo resulta en extremo complicado por la severidad de las lesiones acompa&ntilde;antes. El tratamiento del IAM postrauma cerrado del t&oacute;rax incluye: a) manejo de forma conservadora con inotr&oacute;picos que ha resultado con secuelas post-IAM con fracci&oacute;n de expulsi&oacute;n disminuida y datos de falla de bomba,<sup>36</sup> b) fibrinol&iacute;ticos administrados despu&eacute;s de trauma leve, sin embargo, muchos de ellos no son candidatos a este tipo de tratamiento por el riesgo de hemorragia de las lesiones coexistentes,<sup>12,37</sup> c) intervenci&oacute;n percut&aacute;nea primaria con o sin el implante de <i>stents</i> con revascularizaci&oacute;n exitosa y reversi&oacute;n de la elevaci&oacute;n del segmento ST,<sup>38,39</sup> d) revascularizaci&oacute;n con puentes aortocoronarios en el evento agudo, as&iacute; como despu&eacute;s de varias semanas y e) embolectom&iacute;a coronaria.<sup>40,41</sup> La literatura reporta m&uacute;ltiples casos de tratamiento endovascular exitoso, por lo que la primera opci&oacute;n terap&eacute;utica de elecci&oacute;n es, sin duda, la angioplastia coronaria primaria.<sup>11,12,25,42-50</sup> Es importante un estudio minucioso de estos casos, ya que se ha reportado que el IAM postraum&aacute;tico puede ocurrir en forma inmediata o hasta cinco semanas despu&eacute;s del evento inicial.<sup>51,52</sup> La mortalidad depende de la gravedad de las lesiones extracardiacas asociadas, as&iacute; como de la disfunci&oacute;n sist&oacute;lica residual del ventr&iacute;culo izquierdo, por lo que esta patolog&iacute;a debe ser sospechada en pacientes con lesiones al estern&oacute;n.<sup>7</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>CONCLUSI&Oacute;N</b></font></p>     <p align="justify"><font face="verdana" size="2">El infarto agudo del miocardio secundario a trauma cerrado de t&oacute;rax es una entidad rara que representa un gran reto diagn&oacute;stico debido a que suele ser enmascarado por la cl&iacute;nica de las lesiones concomitantes. La sospecha de este padecimiento es la base del manejo de esta patolog&iacute;a ya que el diagn&oacute;stico temprano nos permite iniciar el tratamiento correcto.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2"><b>REFERENCIAS</b></font></p>    <!-- ref --><p align="justify"><font face="verdana" size="2">1.
