<body><![CDATA[<p>&nbsp;</p>     <p><font size="2" face="Verdana">Los aneurismas intracraneales contin&uacute;an siendo una de las enfermedades m&aacute;s frecuentes y devastadoras en las que se necesita la presencia de un neurocirujano. Se estima que la incidencia de aneurismas en la poblaci&oacute;n general var&iacute;a considerablemente de 6 a 16 por 100,000 con el mayor &iacute;ndice reportado en Jap&oacute;n y Finlandia <sup>1-4</sup>. La ruptura de un aneurisma intracranial representa aproximadamente entre el 5 y el 15% de los infarto <sup>5</sup>. Se define aneurisma gigante como aquellos con un di&aacute;metro mayor de 2.5 cm y que corresponden del 3 al 5% de los aneurismas intracraniales <sup>6,7</sup>.</font></p>     <p><font size="2" face="Verdana">El tratamiento de los aneurismas intracraneales ha mejorado de manera importante en el paso de pocas d&eacute;cadas, los avances en las t&eacute;cnicas de anestesia, microquir&uacute;rgicas y la introducci&oacute;n de m&eacute;todos endovasculares representan opciones que han disminuido significativamente la morbimortalidad <sup>6</sup>. El clipaje directo del cuello se considera el tratamiento de elecci&oacute;n para los aneurismas intracraneales <sup>7-14</sup>. Sin embargo, para el subgrupo de los aneurismas intracraneales gigantes (en especial para aquellos localizados en la circulaci&oacute;n posterior) el riesgo asociado con el tratamiento quir&uacute;rgico abierto puede en algunas ocasiones alcanzar niveles inaceptables <sup>15,16</sup>.</font></p>     <p><font size="2" face="Verdana">La terapia endovascular neurol&oacute;gica ha sido desarrollado para representar una opci&oacute;n m&aacute;s en el manejo efectivo de pacientes con aneurismas intracraneales considerados como alto riesgo. Reportes previos en la literatura han enfatizado la seguridad y eficacia de la t&eacute;cnica de los <i>   coils </i>    desprendibles de Guglielmi (CDG) para el tratamiento endovascular de aneurismas intracraneales rotos    <sup>17-23</sup>. En este momento este es el &uacute;nico dispositivo endovascular en la actualidad aprobado por la Food and Drug Administration (FDA) en Estados Unidos de Norte&aacute;merica y en Canad&aacute; en la cual involucra espirales de platino de diferentes tama&ntilde;os y longitudes que pueden tomar formas complejas cuando se enrollan dentro del saco aneurism&aacute;tico <sup>5</sup>. En la actualidad m&aacute;s de 16 000 pacientes con aneurismas rotos y no rotos han sido tratados a nivel mundial con el m&eacute;todo de CDG <sup>24</sup>. Reportes publicados de resultados cl&iacute;nicos y angiogr&aacute;ficos sugiere que este m&eacute;todo est&aacute; asociado con menos complicaciones que los asociados con la microneurocirug&iacute;a <sup>25,26</sup>.</font></p>     <p><font size="2" face="Verdana">El papel de la terapia endovascular neurol&oacute;gica en el tratamiento de los aneurismas gigantes en la actualidad todav&iacute;a no se ha establecido. </font></p>     <p><font size="2" face="Verdana">El objetivo de este art&iacute;culo es presentar el caso de una paciente con aneurisma gigante de circulaci&oacute;n posterior que fue tratada de manera exitosa con la colocaci&oacute;n de un solo CDG.</font></p>     <p><font size="2" face="Verdana">    <br>   <b> CASO CL&Iacute;NICO </b></font></p>     <p><font size="2" face="Verdana">El caso corresponde a una mujer de 35 a&ntilde;os quien presenta historia de cefalea s&uacute;bita e intensa, nauseas y v&oacute;mito en junio del 2000, acude al hospital consciente, con rigidez de nuca y par&aacute;lisis completa del III par craneal izquierdo. Se le realiza tomograf&iacute;a computada de cerebro demostrando hemorragia subaracnoidea difusa, predominantemente en la cisterna basal con calificaci&oacute;n de Fisher III, adem&aacute;s de lesi&oacute;n isodensa lateral a la silla turca y a la administraci&oacute;n de medio de contraste yodado intravenoso se observa reforzamiento uniforme de la lesi&oacute;n de aproximadamente 40 x 45 mm sugestiva de un aneurisma (<a href="#a07f1">figura 1</a>). Posterior a ello, se lleva a la paciente a la sala de hemodinamia para la realizaci&oacute;n de angiograf&iacute;a cerebral de 4 vasos en donde se observa aneurisma gigante de la circulaci&oacute;n posterior dependiente de la arteria cerebral posterior izquierda segmento P1 con dimensiones de 40 x 45 x 35 mm. Se le administra anestesia general con intubaci&oacute;n orotraqueal y asistencia ventilatoria. Acto seguido se procede a la realizaci&oacute;n de cateterismo supraselectivo de la arteria vertebral izquierda con microcat&eacute;ter <i>   tracker excell </i> 14 y microgu&iacute;a transend 14 (Boston Scientific Corp.) logrando canalizar el cuello del aneurisma; sin embargo, una vez que el microcat&eacute;ter se encontraba en el cuello al efectuar los disparos a trav&eacute;s del cat&eacute;ter gu&iacute;a se observaba exclusi&oacute;n del aneurisma (<a href="#a07f2">figura 2</a>). Se decide colocar un CDG soft 2-D de 2 X 3 mm, observando exclusi&oacute;n del aneurisma de la circulaci&oacute;n, pero con ausencia proximal de la arteria cerebral posterior izquierda. El grado de obliteraci&oacute;n fue valorado por angiograf&iacute;a postembolizaci&oacute;n inmediata con proyecciones m&uacute;ltiples llevando el resultado final clasificado como oclusi&oacute;n completa (<a href="#a07f3">figura 3</a>). La paciente al d&iacute;a siguientes del procedimiento endovascular, presenta recuperaci&oacute;n de la paresia del III par izquierdo. Se da de alta al cuarto d&iacute;a de estancia hospitalaria sin datos de irritaci&oacute;n men&iacute;ngea o hemianopsia. La tomograf&iacute;a computada de control, antes de su egreso, demuestra la presencia solo de las paredes del aneurisma sin infarto cerebral (<a href="#a07f4">figura 4</a>).</font></p>     <p align="center"><a name="a07f1"></a></p>     ]]></body>

<body><![CDATA[<P align="center"><font size="2" face="Verdana"><img src="/img/revistas/aneuroc/v9n2/n2a07f5.jpg" ></font></p>     <p><font size="2" face="Verdana">    <br>   <b> DISCUSI&Oacute;N </b></font></p>     <p><font size="2" face="Verdana">Es conocida que la historia natural de los aneurismas intracraneales gigantes sintom&aacute;ticos no tratados es pobre y el grado de mortalidad a 2 a&ntilde;os es del 68 al 100% <sup>27,28</sup>. Recientes estudios han indicado que el tratamiento quir&uacute;rgico de los aneurismas gigantes tienen un grado de mortalidad relacionada al procedimiento es del 4 al 15% y con 71 al 86% de los pacientes con un buen o excelente resultado neurol&oacute;gico<sup>8-10,12,16,29</sup>. Cuando estos resultados son comparados con el pobre pron&oacute;stico de los aneurismas gigantes no tratados es evidente que el tratamiento es deseable. Se comparan estos resultados con los datos cl&iacute;nicos publicados recientemente mediante la t&eacute;cnica de CDG indican que existe entre el 3 y el 8.9% de morbilidad permanente y de 0 al 8% de mortalidad relacionado al procedimiento y del 0 al 4% del grado de resangrado <sup>15,30</sup>.</font></p>     <p><font size="2" face="Verdana">Recientes publicaciones han demostrado que del 82 al 95% de aneurismas grandes pueden ser suficientemente ocluidos con CDG y un resultado satisfactorio del 50% de los pacientes con aneurismas gigantes tratados con este m&eacute;todo <sup>23,30</sup>.</font></p>     <p><font size="2" face="Verdana">Reportes recientes han documentado que el tratamiento endosacular con CDG de aneurismas gigantes tiene una recuperaci&oacute;n de buena o excelente en el 73.3% de los casos con un grado de morbilidad relacionado al procedimiento de un 13% y un 6.7% de mortalidad relacionado al procedimiento, con un grado de resangrado del 6.5% y un 2.5% de sangrado anual. Estos resultados indican que el tratamiento con CDG de aneurismas gigantes pueden ser acompa&ntilde;ados con una morbimortalidad relacionado con el procedimiento comparado con aquellos realizados por expertos con microneurocirug&iacute;a <sup>31</sup>.</font></p>     <p><font size="2" face="Verdana">El tratamiento endovascular de los aneurismas intracraneales gigantes hab&iacute;a sido la colocaci&oacute;n de balones inflables <sup>10,15,20,32-40</sup>. Sin embargo, los m&eacute;todos mas recientes son los que utilizan CDG. Existe un caso reportado sobre un paciente que desarroll&oacute; trombosis completa de su aneurisma gigante despu&eacute;s de la colocaci&oacute;n del microcat&eacute;ter por 30 minutos en el <i>   ostium </i> del aneurisma, esta autotrombosis tard&iacute;a permaneci&oacute; estable despu&eacute;s de 8 a&ntilde;os del tratamiento <sup>6</sup>. En la actualidad la utilizaci&oacute;n de t&eacute;cnicas endovasculares combinados como la de remoldeamiento del cuello con bal&oacute;n de colocaci&oacute;n de <i> stents </i> en el vaso portador del aneurisma, pueden representar t&eacute;cnicas con resultados satisfactorios. </font></p>     <p><font size="2" face="Verdana">De los aneurismas gigantes localizados en forma distal ha demostrado que la oclusi&oacute;n de los vasos proximales con o sin <i> by-pass </i> distal a trav&eacute;s de la embolizaci&oacute;n de vasos aferentes es segura y efectiva en prevenir el sangrado avalado con una prueba de oclusi&oacute;n negativa para s&iacute;ntomas neurol&oacute;gicos <sup>6</sup>.</font></p>     <p><font size="2" face="Verdana">Se han reportado complicaciones relacionadas con la embolizaci&oacute;n de aneurismas siendo m&aacute;s importantes los eventos isqu&eacute;micos que consisten en hemiparesia con o sin afasia <sup>22,33,41-44</sup>. La morbilidad es menos del 10%  <sup>13,19,39,45</sup>. En nuestra paciente hubo oclusi&oacute;n proximal de la arteria cerebral posterior izquierda sin manifestaciones cl&iacute;nicas. </font></p>     <p><font size="2" face="Verdana">No hemos encontrado ning&uacute;n reporte en la literatura a nuestro alcance en el cual se pudo ocluir un aneurisma gigante de circulaci&oacute;n posterior a nivel de la arteria cerebral posterior izquierda en su segmento P1, con la utilizaci&oacute;n de un solo <i> coil </i> y controles angiogr&aacute;ficos a los 3 y 6 meses en los que se demuestra exclusi&oacute;n angiogr&aacute;fica completa del aneurisma y que la pacientes se encuentre asintom&aacute;tico.</font></p>     ]]></body>
