We present the case of a patient with an infiltrating tongue base tumor, no ventilatory compromise, but significant limitation to mouth opening. Awake intubation was performed using conscious sedation with dexmedetomidine and fentanyl, atropine as antisialagogue, and local anesthesia through combined spray and spray-as-you-go technique. The intubation was successful on the first attempt, without respiratory or hemodynamic complications. The patient remained cooperative and had no recollection of the procedure. This case highlights the utility of a structured protocol, appropriate sedation, and a trained airway team. The approach adhered to current guidelines and demonstrates that awake intubation can be safely performed with excellent tolerance and high success when patient selection is appropriate.]]></p></abstract>
<abstract abstract-type="short" xml:lang="es"><p><![CDATA[Resumen La intubación con fibrobroncoscopio en paciente despierto es una técnica segura y eficaz para el manejo de la vía aérea difícil anticipada. Presentamos el caso de una paciente con una neoplasia infiltrante de base de lengua, sin limitación funcional ventilatoria, pero con restricción significativa a la apertura oral. Se realizó intubación despierto con fibrobroncoscopio utilizando sedación consciente con dexmedetomidina y fentanilo, antisialogogo (atropina) y anestesia local en técnica combinada (spray y spray-as-you-go). La intubación fue exitosa en el primer intento, sin complicaciones respiratorias ni hemodinámicas. La paciente se mantuvo colaboradora y sin recuerdo del procedimiento. Este caso destaca la utilidad de un protocolo estructurado, el uso de sedación adecuada y la necesidad de equipos entrenados en el manejo de vía aérea difícil. La técnica se mantuvo dentro de las recomendaciones actuales y demuestra que la intubación despierto puede realizarse de forma segura con buena tolerancia y alto grado de éxito cuando se selecciona adecuadamente al paciente.]]></p></abstract>
