Describir la frecuencia de virus respiratorios y características clínicas en niños con cuadros respiratorios de un hospital de tercer nivel en México. MATERIAL Y MÉTODOS. Se incluyeron niños con diagnóstico de infección respiratoria y un resultado positivo por inmunofluorescencia de enero 2004 a octubre 2006. RESULTADOS. De 986 muestras nasofaríngeas, 138 (14%) fueron positivas. La frecuencia fue: 80% virus sincicial respiratorio (VSR), 8% parainfluenza 1, 5% parainfluenza3, 2% adenovirus, 2% influenza A, 1% parainfluenza 2 y 1% influenza B. CONCLUSIONES. La frecuencia de virus respiratorios fue de 14%. El VSR se identificó asociado con más frecuencia, a neumonía y bronquiolitis en menores de 3 años.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[OBJECTIVE. To describe the frequency of respiratory viruses and clinical characteristics in children with respiratory signs and symptoms in a tertiary care center in Mexico. MATERIAL AND METHODS. Patients with a clinical diagnosis of respiratory infection and a positive immunofluorescence result (Light Diagnostics) from January 2004 to October 2006 were included. RESULTS. From the 986 nashopharyngeal samples, 138 (14%) were positive by immunofluorescence. The frequency was: 80% RSV, 8% parainfluenza 1, 5% parainfluenza 3, 2% adenovirus, 2% influenza A, 1% parainfluenza 2 and 1% influenza B. CONCLUSIONS. Respiratory viruses were detected in 14% of samples tested. RSV was the most frequently identified virus and was associated with pneumonia and bronchiolitis in children younger than 3 years old.]]></p></abstract>

Hasta 70&#37; de las infecciones del tracto respiratorio superior    &#40;IRS&#41; son de etiolog&#237;a viral.<sup>1</sup> En 2005, la OMS estim&#243; una mortalidad en ni&#241;os    menores de 5 a&#241;os de 10.6 millones, 10&#37; por neumon&#237;a.<sup>2</sup> En M&#233;xico, la mortalidad    anual por infecciones respiratorias bajas &#40;IRB&#41; en 2006 fue 7.2 por 100000 en    ni&#241;os de 1 a 4 a&#241;os y 109 por 100000 en ni&#241;os menores de 1 a&#241;o. La incidencia    anual de IRB en 2006 fue 36627 por 100000 ni&#241;os menores de 4 a&#241;os.<sup>3,4</sup> A pesar    de la gran morbimortalidad por infecciones respiratorias reportada en M&#233;xico    se conoce poco acerca de su etiolog&#237;a.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Existen informes    de pat&#243;genos virales respiratorios en ni&#241;os en nuestro pa&#237;s,<sup>5-13</sup>    sin embargo la informaci&#243;n es limitada. El objetivo del estudio fue describir    la frecuencia de virus respiratorios y caracter&#237;sticas cl&#237;nicas en    ni&#241;os que acuden a un hospital pedi&#225;trico de tercer nivel en la Ciudad    de M&#233;xico.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Material y m&#233;todos</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se trata de un    estudio transversal descriptivo, retrolectivo con uso de base de datos secundaria.    El estudio fue revisado y aprobado por las comisiones de investigaci&#243;n y &#233;tica    del Hospital Infantil de M&#233;xico Federico G&#243;mez. Debido al uso de base de datos    existente colectada en forma retrospectiva no se requiri&#243; consentimiento informado    de los participantes.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Poblaci&#243;n    de estudio</i>. Se incluyeron pacientes hospitalizados o que acudieron a la    consulta externa del Hospital Infantil de M&#233;xico &#40;un hospital de tercer    nivel&#41; que presentaron s&#237;ntomas respiratorios y a quienes se tom&#243;    una muestra nasofar&#237;ngea para identificaci&#243;n viral, de enero de 2004    a octubre de 2006. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Metodolog&#237;a</i>.    Todos los resultados positivos fueron incluidos en el estudio. Se obtuvieron    las caracter&#237;sticas cl&#237;nicas y demogr&#225;ficas de expedientes cl&#237;nicos.    No se obtuvo informaci&#243;n sobre cultivos bacterianos positivos.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Detecci&#243;n    de virus</i>. Se tom&#243; un exudado nasofar&#237;ngeo, que fue colocado en    medio viral &#40;soluci&#243;n balanceada de Hank's, 0.11&#37; NaHCO3&#41;,    y guardado a 4<sup>o</sup>C en el laboratorio de virolog&#237;a hasta su procesamiento.    Se detectaron siete virus &#40;virus sincicial respiratorio &#40;VSR&#41;, influenza    A y B, parainfluenza 1, 2 y 3 y adenovirus&#41; mediante inmunofluorescencia    indirecta &#40;IFI&#41;. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>An&#225;lisis    estad&#237;stico</i>. Se realiz&#243; un an&#225;lisis univariado para los virus    y caracter&#237;sticas cl&#237;nicas. Se utiliz&#243; t para contrastar la edad,    y ji cuadrada para el sexo y las caracter&#237;sticas cl&#237;nicas de acuerdo    al virus, en pacientes hospitalizados y ambulatorios.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Resultados</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">C<i>aracter&#237;sticas    demogr&#225;ficas</i>. La mediana de edad fue 8 meses &#40;10 d&#237;as a 15    a&#241;os&#41;, 50&#37; masculinos y 50&#37; femeninos.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Frecuencia de    virus detectados</i>. De las 986 muestras enviadas para detecci&#243;n de virus,    138 &#40;14&#37;&#41; fueron positivas: 58/290 &#40;20&#37;&#41; en 2004, 61/453    &#40;13&#37;&#41; en 2005 y 19/284 &#40;7&#37;&#41; hasta octubre de 2006; 64&#37;    en ni&#241;os menores de 1 a&#241;o y 83&#37; en ni&#241;os menores de 3 a&#241;os.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> La frecuencia    de virus respiratorios fue: 80&#37; VSR, 8&#37; parainfluenza 1, 5&#37; parainfluenza 3,    2&#37; adenovirus, 2&#37; influenza A, 1&#37; parainfluenza 2 y 1&#37; influenza B. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> En 2004, hubo    dos casos de coinfecci&#243;n viral: uno con adenovirus, VSR y parainfluenza    1 y uno con VSR y parainfluenza 1. Se encontr&#243; VSR en 85&#37; de menores    de 1 a&#241;o y 83&#37; en menores de 3 a&#241;os; el diagn&#243;stico m&#225;s    frecuente fue neumon&#237;a en 38&#37;, seguido de bronquiolitis en 22&#37;,    e IRS en 2&#37;; con mayor incidencia en invierno, pero con casos todo el a&#241;o.    En 2005, se detectaron tres casos de influenza A y un caso de influenza B en    menores de 5 a&#241;os, de abril a octubre &#40;<a href="/img/revistas/spm/v52n6/a08fig1.jpg">figura    1</a>&#41;.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Estacionalidad.    La mayor incidencia fue en invierno &#40;46&#37;&#41; y oto&#241;o &#40;36&#37;&#41;, seguida de primavera    &#40;12&#37;&#41; y verano &#40;5&#37;&#41;.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Caracter&#237;sticas    cl&#237;nicas</i>. El diagn&#243;stico, signos y s&#237;ntomas, enfermedades    subyacentes y tratamiento se describen en el <a href="/img/revistas/spm/v52n6/a08qua1.jpg">cuadro    I</a>. El 70&#37; de los pacientes estaban hospitalizados y 30&#37; fueron ambulatorios.    La evoluci&#243;n fue favorable en la mayor&#237;a de los casos, sin embargo    se reportaron 11 muertes &#40;8&#37;&#41;, tres de ellas con detecci&#243;n    positiva para adenovirus.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>An&#225;lisis    bivariado</i>. No se observaron diferencias significativas en la edad, el sexo    o las caracter&#237;sticas cl&#237;nicas de acuerdo con el virus detectado.    Se encontr&#243; un mayor porcentaje de s&#237;ntomas cl&#237;nicos en pacientes    hospitalizados, en comparaci&#243;n con los ambulatorios &#40;<a href="/img/revistas/spm/v52n6/a08qua1.jpg">cuadro    I</a>&#41;.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Discusi&#243;n</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">La frecuencia de    virus respiratorios fue de 14&#37;, comparable con el 11&#37; de aislamientos en una    muestra de todo el pa&#237;s de 1995 a 2000 reportado por el InDRE14 y 18&#37; en pacientes    con bronquiolitis del Hospital Infantil Privado,<sup>13</sup> y en contraste con 65&#37; en    estudios en comunidades en M&#233;xico,<sup>5,10</sup> y 39 a 60&#37; en centros de segundo y tercer    nivel que registra la literatura.<sup>6,13-17</sup> Este bajo porcentaje puede ser explicado    por el tipo de pacientes del hospital, por la referencia de pacientes con infecciones    no complicadas a segundo nivel, por la regular eficiencia diagn&#243;stica de IFI18    o por la falla para detectar virus no incluidos en el panel viral, como metaneumovirus    humano,<sup>11,12,19-23</sup> bocavirus,<sup>24-26</sup> o coronavirus.<sup>22,27,28</sup></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Similar a lo que    se ha descrito en muchos informes internacionales,<sup>20,22,27,29-33</sup> y en algunos    mexicanos,<sup>5-8,10,13</sup> as&#237; como en un informe previo de este hospital, con 36&#37;,<sup>34</sup>    el virus m&#225;s frecuentemente detectado fue VSR, asociado a neumon&#237;a en 39&#37;    y bronquiolitis en 22 por ciento de los casos.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> En el presente    estudio se encontr&#243; parainfluenza en 14&#37; de detecciones virales, lo que representa    la segunda causa de infecci&#243;n viral; esto concuerda con la literatura.<sup>6,22,29,31,32</sup>    Por otra parte, se encontr&#243; 2&#37; de influenza A y 1&#37; de influenza B en 2005,    sin detecciones en 2004 ni 2006; infecciones que informes internacionales describen    de 4 a 20&#37;<sup>22,31,35,36</sup> y estudios nacionales de 7 a 12 por ciento.<sup>7,9</sup> Este bajo    porcentaje de identificaci&#243;n pudo estar condicionado por falta de identificaci&#243;n    mediante otros m&#233;todos como inoculaci&#243;n en embriones de pollo, cultivo celular    o PCR,<sup>37,38</sup> y es probable que la incidencia sea mayor en la comunidad.<sup>39</sup> El    sistema Flunet reporta en el pa&#237;s 66 casos de influenza en 2004, 297 casos    en 2005 y 97 casos en 2006, lo que podr&#237;a correlacionar con la baja incidencia    en el Hospital.<sup>40</sup></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Se pueden mencionar    como limitaciones del estudio que &#233;ste es de tipo retrospectivo y s&#243;lo se    utiliz&#243; IFI, m&#233;todo que a pesar de su buena sensibilidad y especificidad<sup>37,41,42</sup>    reporta una eficiencia diagn&#243;stica de 57&#37;, en comparaci&#243;n con 72&#37; del cultivo    m&#225;s IFI y con 92&#37; si se utiliza PCR.<sup>18</sup> Esto pudo haber conducido a subestimar    la incidencia de virus respiratorios.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> En conclusi&#243;n,    se document&#243; que VSR fue el virus m&#225;s frecuente asociado a neumon&#237;a y    bronquiolitis en menores de 3 a&#241;os de edad en este hospital.
